ASHA Certification Maintenance Record Keeping Form: Logging, Submission, Audits

The ASHA certification maintenance record keeping form is a downloadable PDF log where CCC-A and CCC-SLP holders track every professional development activity completed during their three-year maintenance interval. You fill it in as you go, keep it with your certificates of completion, and hold onto it in case ASHA selects you for an audit. The form is not routinely submitted; what you submit at the end of the cycle is a separate online compliance attestation, due by December 31 of your interval’s final year.1American Speech-Language-Hearing Association. What the Form Is For

The record-keeping form is your personal evidence file. It lists each qualifying activity so that, if ASHA audits you, you can match every line to a certificate, transcript, or sign-in sheet. The online compliance form you file through your ASHA account is a simple attestation that you met the 30-hour standard; you do not upload the log or your certificates when you submit it.2American Speech-Language-Hearing Association. If every one of your 30 Professional Development Hours was earned through an ASHA-approved CE provider and recorded in the CE Registry, ASHA submits the compliance form on your behalf automatically.3American Speech-Language-Hearing Association. Certification Maintenance Frequently Asked Questions The record-keeping form matters most for activities earned outside the registry, where no one is reporting to ASHA for you: employer in-services, independent coursework, state association conferences, and courses from non-ASHA providers.

What Your Log Needs to Reflect

Over each three-year interval you need at least 30 PDHs.4American Speech-Language-Hearing Association. Requirements for Maintaining Your ASHA Certification Hours beyond 30 do not roll over, so there is no benefit to logging extra.5American Speech-Language-Hearing Association. Professional Development Hours for ASHA Certification Maintenance and Reinstatement Applications Within the 30, specific hours must sit in designated content areas, and your log needs to flag them clearly:6American Speech-Language-Hearing Association. Professional Development Requirements for Certification Maintenance of the ASHA CCC-A and CCC-SLP

  • 1 PDH in ethics.
  • 2 PDHs in cultural competency and collaboration. Effective January 1, 2026, these hours must cover topics that help you provide services aligned with the histories, values, and circumstances of the individuals you serve, or that strengthen self-reflection and collaboration with colleagues, students, Clinical Fellows, and care partners.

If you supervise students, externs, or Clinical Fellows, you also need at least 2 PDHs in supervision, clinical instruction, or mentorship. This is a one-time post-certification requirement and does not repeat in future intervals, but your log needs to show it the cycle you first complete it.7American Speech-Language-Hearing Association. Requirements for ASHA-Certified Clinical Educators, Supervisors, and Clinical Fellowship Mentors Those two hours count toward the 30, not on top of them.

One ASHA Continuing Education Unit equals ten contact hours of instruction, so a half-day workshop reported as 0.3 CEUs is 3 PDHs on your log.8American Speech-Language-Hearing Association. Continuing Education Credit

Which Activities You Can Log

An activity qualifies when four conditions are met: you are the learner, the content is within your scope of practice, the activity builds or refines skills for independent clinical work, and a third party can verify your attendance.5American Speech-Language-Hearing Association. Professional Development Hours for ASHA Certification Maintenance and Reinstatement Applications That last point is the one most people get wrong. Reading a journal article at home does not count unless it is part of a structured journal club with documented participation.

Activities that qualify include:5American Speech-Language-Hearing Association. Professional Development Hours for ASHA Certification Maintenance and Reinstatement Applications

  • Employer-sponsored in-services, including Medical Grand Rounds, special education workshops on topics such as ADA requirements, and formal manufacturer training on diagnostic or treatment equipment.
  • Structured journal clubs with documented participation.
  • State association conferences, even when they do not carry ASHA CEUs.
  • Graduate or undergraduate coursework, including formal online non-credit courses.
  • Foreign language study in a language spoken by the population you serve.
  • Business and management training relevant to running a private practice.
  • Industry or career training on reimbursement, clinical supervision, and service delivery models.

Save the certificate of completion, sign-in sheet, or transcript for every activity you log. If you cannot produce documentation during an audit, the hours do not stand.

Filling In Each Row

Download the current PDF from ASHA’s certification resources page.9American Speech-Language-Hearing Association. Requirements for Maintaining Your ASHA Certification The form is a grid, one row per activity. For each entry, record:

  • The specific calendar date of the activity.
  • The activity title, written to match the title on your certificate of completion exactly. If the certificate says “Ethical Decision-Making in Telepractice,” use that phrasing rather than a paraphrase.
  • The provider or sponsor, using the official organization name rather than an abbreviation a reviewer might not recognize.
  • The number of PDHs earned. If the provider reported in CEUs, multiply by ten.
  • The content area, when the activity satisfies one of the required categories: ethics, cultural competency and collaboration, or supervision.

Fill in the form as you go, ideally within a week of each activity, rather than reconstructing it near the deadline. Entering activities while the paperwork is fresh prevents mismatched dates, misspelled titles, and missing certificates. Keep a running total and check it periodically. People often reach the final weeks of a cycle a few hours short because an activity they counted on turned out to lack third-party verification. Catching a gap in October gives you time to register for something; catching it on December 30 does not.

Store the completed form with your supporting documentation in one folder, digital or physical, organized by activity date so each row matches a corresponding certificate.

Submitting the Compliance Form at the End of the Cycle

Once your 30 hours are complete, log in to your ASHA account and submit the compliance form, affirming that you met the standard, including the required content-area hours.2American Speech-Language-Hearing Association. After submission, the system generates a confirmation notice in your member portal and sends an email receipt. Check that your certification end date in ASHA’s online directory has advanced to reflect your new three-year cycle.

If You Are Selected for an Audit

After the compliance form is in, ASHA may select you for a random certification maintenance audit. You will be notified and given instructions for submitting your documentation.2American Speech-Language-Hearing Association. Submitting Certification Maintenance Compliance Forms This is the moment the record-keeping form earns its keep. Each line should correspond to a certificate, sign-in sheet, or transcript with a matching activity title; discrepancies between what you logged and what your documentation says will slow the review and may require explanation.

Retain all documentation for at least one full year after your maintenance interval ends, since audit notifications can arrive well after the December 31 compliance deadline. Cloud backup avoids losing paper records to an office move or hard-drive failure.

What a Lapse Looks Like

Missing the compliance deadline, falling short on PDHs, or lapsing on annual fees does not trigger permanent revocation. Your status changes to “Not Current,” and if unresolved, the certification eventually expires.10American Speech-Language-Hearing Association.
