Authorization Letter to Collect Passport: Contents and Sample

An authorization letter to collect a passport is a short signed statement telling a passport office, embassy, or visa center that you permit a named person to pick up your passport for you. It works best when it names you, names your representative exactly as their ID reads, identifies the passport or application, and carries your signature and the date. The letter travels with your representative to the counter, along with IDs and any appointment paperwork.

Before drafting anything, check whether the releasing office publishes its own form. Many U.S. embassies use a standard authorization letter template, and offices like India’s Passport Seva have their own authority letter format. If a form exists, use it. A self-drafted letter is accepted when no template is provided.

What the Letter Must Include

Every authorization letter, whether on an official template or drafted from scratch, needs the same core details. Missing any one of them gives the clerk grounds to refuse the pickup.1U.S. Embassy. Authorization Letter

  • Your full legal name, written exactly as it appears on your passport or application. A middle-name abbreviation that doesn’t match can cause problems.
  • Your passport or application number, plus any tracking receipt or reference code that ties the letter to your file.
  • Your representative’s full legal name, matching the government-issued ID they will present at the counter. No nicknames or shortened forms.2U.S. Embassy & Consulates in Turkiye. Congratulations Your Visa Application Is Approved
  • A direct statement of authorization, such as “I authorize [representative’s name] to collect my passport from [office name] on my behalf.”
  • The date and your signature. Some offices also require your representative to sign the letter so the clerk can compare that signature against the ID at the window.3Government of India. Authority Letter – Passport Seva Project
  • A phone number where you can be reached if the clerk has any doubts.

Keep the language direct. Vague wording like “I authorize my friend to pick up my documents,” without specifying which documents, which office, or the representative’s full legal name, gives the clerk nothing to verify.

Sample Authorization Letter

When no official form is provided, this format covers what most passport offices expect:

[Your Full Legal Name]
[Your Address]
[Your Phone Number]
[Date]

To: [Name of Passport Office / Embassy / Consulate]

I, [Your Full Legal Name], holder of passport number [Passport Number], hereby authorize [Representative’s Full Legal Name] to collect my passport from [Office Name and Location] on my behalf.

My representative’s government-issued identification number is [ID Number], and a copy of their photo ID is attached to this letter.

I am unable to collect the passport in person due to [brief reason, e.g., medical condition, work travel, residence in another city].

[Your Signature]
[Your Printed Name]
[Date]

[Representative’s Signature]
[Representative’s Printed Name]
[Date]

[Notary block, if required]

Attach a photocopy of both your ID and your representative’s ID. Keep a copy for your own records.

What Your Representative Should Bring

The letter by itself is never enough. Your representative should walk in with a small packet of documents. The exact list varies by office, but nearly every embassy and consulate that allows third-party collection asks for the same things.

  • The original authorization letter. Photocopies are almost always rejected; the U.S. embassy system specifically requires the original.2U.S. Embassy & Consulates in Turkiye. Congratulations Your Visa Application Is Approved
  • The representative’s government-issued photo ID, with the name matching the authorization letter exactly.4USTravelDocs. Letter of Authority for Passport Pickup
  • A photocopy of your ID so the clerk can verify your signature. India’s Passport Seva explicitly requires the representative to carry an identity document with a photo, with a copy attached to the authority letter.3Government of India. Authority Letter – Passport Seva Project
  • The appointment confirmation, interview waiver, or application receipt. Some offices list this as recommended rather than required, but having it speeds the pickup considerably.4USTravelDocs. Letter of Authority for Passport Pickup

Put everything in a single folder before your representative leaves the house. Clerks at busy counters process dozens of pickups a day, and hunting through a bag doesn’t help the process along.

Do You Need to Notarize It?

That depends on the office releasing the passport. Many embassies and visa collection centers accept a plain signed letter. The Philippine Embassy in Ottawa, for example, requires an original authorization letter with the applicant’s original signature but does not specify notarization.5Philippine Embassy in Ottawa. Passports Other offices, particularly those handling higher-security documents or minors, require a notarized signature.

If you cannot confirm the requirement, notarizing the letter is cheap insurance. A notarized letter will not be rejected for being too formal; an unnotarized letter can be rejected where the seal is required. Use black ink, sign in the notary’s physical presence, and make sure the notary is not a family member of either you or your representative.

Why These Letters Get Rejected

Most rejections come from mismatched details rather than missing ones. Someone used a nickname, so the name on the letter doesn’t match the ID at the counter. The applicant’s name is spelled slightly differently than on the application. The letter presented is a photocopy instead of the original. All of these are avoidable with a careful read-through before your representative leaves.

The other frequent failure is turning up without the supporting documents. A flawless letter will not help if your representative forgot their own ID or doesn’t have a copy of yours. Have them lay out every document at home and cross-check every name, number, and date against the letter itself.

Situations This Letter Does Not Cover

A pickup authorization letter is not the right tool for two related situations.
