The BACB Monthly Fieldwork Verification Form is the record you and your supervisor sign each month to confirm the fieldwork hours you completed toward BCBA or BCaBA certification. You do not send it to the Behavior Analyst Certification Board when you finish it. You keep it on file for at least seven years, and the BACB requests it only during an audit or review.1Behavior Analyst Certification Board. Monthly Fieldwork Verification Form: Individual Supervisor To make the month’s hours count, every field has to be complete, the totals have to meet the BACB’s supervision minimums, and both signatures have to be on the form by the last day of the following calendar month.
Pick the Right Version and Open It Correctly
The BACB publishes two versions of the M-FVF. Use the Individual Supervisor form when one BCBA supervises you. Use the Organization form when more than one supervisor within the same agency or practice oversees your work.2Behavior Analyst Certification Board. Monthly Fieldwork Verification Form: Organization You fill out one form per supervisor (or per organization) for each month, so a trainee with two individual supervisors completes two forms every month.
Download the current form from the BACB website. The dropdown menus only work in Adobe Acrobat Reader on a desktop; a browser will not save dropdown selections or dates reliably.3Behavior Analyst Certification Board. Final Fieldwork Verification Form: Individual Supervisor If Acrobat isn’t available, print the form and complete it by hand.
Complete Every Field
The top of the M-FVF identifies you, your supervisor, and the reporting period. Enter your full legal name as it appears in your BACB account, your BACB ID number, the calendar month and year of the fieldwork, and the state and country where the work physically took place. Then enter your supervisor’s full legal name and either their BCBA certification number or BACB ID.
The hours section splits the month’s work into two categories.1Behavior Analyst Certification Board. Monthly Fieldwork Verification Form: Individual Supervisor Independent hours are time you spent performing behavior-analytic activities without your supervisor present. Supervised hours are time your supervisor was directly involved in oversight, including meetings and observations. Within the supervised hours, you also record how many minutes your supervisor spent observing you work directly with a client. The form calculates total fieldwork hours and the percentage supervised.
Every field must be filled in. An incomplete form means the entire month’s fieldwork hours are lost.
Know Which Activities Count
Not every task belongs on the form. The BACB divides qualifying fieldwork into unrestricted and restricted activities and excludes several kinds of work entirely.
Unrestricted activities are the higher-level tasks a certified behavior analyst typically performs: intake interviews, designing data collection systems, writing behavior intervention plans, conducting functional analyses and preference assessments, training staff and caregivers, graphing and analyzing data, literature review tied to a current client, and giving feedback to RBTs on their implementation. BCBA candidates must complete at least 60 percent of total fieldwork hours in unrestricted activities. BCaBA candidates need at least 40 percent.4Behavior Analyst Certification Board. Board Certified Behavior Analyst Handbook There is no cap on unrestricted hours.5Behavior Analyst Certification Board. Fieldwork: Getting It Right
Restricted activities are mostly hands-on delivery of therapeutic and instructional procedures with clients. For BCBA fieldwork these can make up no more than 40 percent of your total hours; there is no minimum.4Behavior Analyst Certification Board. Board Certified Behavior Analyst Handbook
Some work does not count as fieldwork at all: attending conferences, course homework, meetings with little behavior-analytic content, billing and administrative tasks, and podcasts. Only client-specific behavior-analytic activity is eligible.
Meet the Monthly Minimums or the Month Doesn’t Count
Each M-FVF covers exactly one calendar month, which the BACB treats as one supervisory period. The month’s totals have to fall within the hour range for your track and meet the supervision percentages, or the hours you report won’t qualify.
Under the 2027 BCBA requirements, both tracks allow between 20 and 160 hours per calendar month.6Behavior Analyst Certification Board. 2027 BCBA Requirements Supervised Fieldwork requires 2,000 total hours across the experience, at least 5 percent of each month’s hours supervised, and a cumulative 60 minutes of client observation per month. Concentrated Supervised Fieldwork requires 1,500 total hours, at least 7.5 percent of each month’s hours supervised, and a cumulative 90 minutes of client observation.7Behavior Analyst Certification Board. Guidance for Meeting the BCBA Requirements During the 2027 Transition
If you are still working under the pre-2027 BCBA Handbook standards, the monthly cap is 130 hours rather than 160.4Behavior Analyst Certification Board. Board Certified Behavior Analyst Handbook Check your BACB account and confirm with your supervisor which set of requirements applies before you fill out your first form. BCaBA candidates follow similar structures with different totals and percentages, laid out in the BCaBA Handbook.8Behavior Analyst Certification Board. BCaBA Handbook
At least half of your supervised hours each month must be individual (one-on-one) meetings. Group supervision, with up to 10 trainees, can fill the rest but cannot exceed 50 percent of the month’s supervised time.9Behavior Analyst Certification Board. When Supervising Fieldwork Doesn’t Go as Planned The required client observation can be in person, by synchronous video, or asynchronously by recorded video reviewed later.10Behavior Analyst Certification Board. BCBA Fieldwork Requirements
A shortfall on any of these figures for a given month means the month’s hours are lost. The BACB does not permit retroactive makeup of a supervision gap.
Confirm Your Supervisor Is Eligible Before They Sign
A signature from an ineligible supervisor invalidates the hours on that form. Supervisors of BCBA fieldwork must hold active BCBA certification. Supervisors of BCaBA fieldwork must be a BCBA or a licensed or registered psychologist. Every supervisor must also have completed the 8-hour training based on the BACB’s Supervisor Training Curriculum Outline (2.0) through an authorized ACE provider before providing supervision.11Behavior Analyst Certification Board. Supervision, Assessment, Training, and Oversight
If your supervisor’s certification lapses or the training requirement wasn’t met, they cannot sign your M-FVF for that period. Verify status when you begin working together, and check again from time to time throughout your experience.
Sign by the End of the Following Month
Both you and your supervisor sign and date the form. The deadline is firm: signatures must be on the form by the last day of the calendar month after the month of supervision. Fieldwork done in March must be signed by April 30. Miss the deadline and the hours are permanently lost.1Behavior Analyst Certification Board. Monthly Fieldwork Verification Form: Individual Supervisor
Signatures have to comply with the BACB’s Acceptable Signatures Policy, published separately on the BACB website.12Behavior Analyst Certification Board. Documenting Fieldwork: Helpful Answers to Your FAQs A typed name is not a valid signature. If you use a digital signature tool, confirm it meets the policy before you rely on it for months of records.
Retain the Form, Don’t Send It
Do not submit the M-FVF to the BACB when it is signed. You and your supervisor each keep a copy for at least seven years. A dedicated digital folder with backups, organized by month and supervisor, is the easiest way to make forms findable if you are audited. Where supporting documentation includes client information, storage has to be HIPAA-compliant.
Your M-FVFs are the source records behind the Final Fieldwork Verification Form (F-FVF). When you finish your experience with a supervisor, you complete one F-FVF per supervisor that summarizes hours across every month. Signing the F-FVF, both of you attest that it and all corresponding M-FVFs are true and correct.3Behavior Analyst Certification Board. Final Fieldwork Verification Form: Individual Supervisor The F-FVF is what you actually send to the BACB with your certification application, so every number on it has to match the underlying monthly forms.
The BACB may audit fieldwork records during the application process or years later, which is why the seven-year retention rule exists. Common audit problems include unsigned or incomplete verification forms, hour totals that don’t reconcile between the M-FVFs and the F-FVF, and missing supervision contracts. The BACB publishes a Fieldwork Checklist for BCBA and BCaBA Supervisors that your supervisor can use throughout the experience to keep documentation clean.11Behavior Analyst Certification Board.
