Capital One doesn’t publish a single “suspicious activity form” for customers to download and submit. Fraud reports go through the mobile app or website, through dedicated phone lines for credit and debit cards, or by mailing a dispute template to the bank’s Salt Lake City address. Which channel is right depends on what kind of account was hit and how quickly you need the card shut down.
Reporting an Unauthorized Credit Card Charge
The fastest option is the app or capitalone.com. Open your recent transactions, tap the charge you don’t recognize, choose “Report a problem,” and answer the prompts. Capital One says there is no time limit for filing a credit card fraud claim.1Capital One. Credit Card Fraud Process Reporting quickly still matters, because it stops additional charges and gets the investigation moving.
Reporting an Unauthorized Debit Card Charge
Debit card fraud has its own number: 1-888-464-0727. Call as soon as you spot a transaction you didn’t make and aren’t an authorized user for.3Capital One. Report Debit Card Fraud The charge has to have posted before a formal fraud claim opens, but if a suspicious transaction is still pending and you believe your card information has been stolen, call anyway so the bank can lock the card.
Speed matters more here than with a credit card. The money is already out of your checking account, and federal liability rules get less forgiving the longer you wait to call.
Filing a Dispute by Mail

Photocopy everything before it goes in the envelope. Mail is the slowest channel, so the investigation won’t start until the form arrives and is logged.
What to Have Ready Before You Report
The same core details work for every channel. Pull up your statement or the transaction list in the app and note the exact date, dollar amount, and merchant name for each charge you want to dispute. If there’s more than one, list them in date order so the pattern is clear.
Be ready to explain the circumstances. Did you still have your card when the charge appeared? Any phishing emails or texts asking for account information? Were you traveling? If you don’t know how the compromise happened, say so; guesses don’t help the investigator.
For identity theft that reaches beyond a single charge, a police report strengthens your claim, and many creditors and credit bureaus expect one before fully resolving the dispute.6My Florida Legal. Your out-of-pocket exposure depends on the type of card and how quickly you report.
Credit Cards

Debit Cards and Checking Accounts
Debit card protections under the Electronic Fund Transfer Act are tighter and are tied to your reporting speed:
- Report within 2 business days of learning about the loss or theft, and liability tops out at $50 or the total unauthorized transfers before you notified the bank, whichever is less.9eCFR. 12 CFR 1005.6 – Liability of Consumer for Unauthorized Transfers

The gap between $50 and unlimited liability comes down to a phone call made a couple of days sooner.
What Happens After You Report
Different federal rules govern the investigation depending on the account type.
Credit Card Claims
Under Regulation Z, Capital One must acknowledge your billing error notice in writing within 30 days and resolve the dispute within two complete billing cycles, capped at 90 days.10eCFR. 12 CFR 1026.13 – Billing Error Resolution While the investigation is open, the bank can’t try to collect the disputed amount or report it as delinquent. If the charge is confirmed fraudulent, it comes off your account for good.
Debit Card and Checking Account Claims
Regulation E gives the bank 10 business days from receiving your error notice to investigate and reach a decision, three business days after that to report the results, and one business day after confirming an error to correct it.11eCFR. 12 CFR 1005.11 – Procedures for Resolving Errors
The bank can extend the investigation to 45 days, but only if it provisionally credits your account within the first 10 business days.11eCFR. 12 CFR 1005.11 – Procedures for Resolving Errors That credit covers the full disputed amount, though up to $50 may be withheld if the bank reasonably believes an unauthorized transfer occurred. You get full use of those funds during the investigation. If the bank confirms fraud, the provisional credit becomes permanent. If it concludes no error occurred, it can reverse the credit, but must explain its findings and provide copies of the documents it relied on.
Updating Recurring Payments After a New Card Is Issued
A replacement card arrives with a different card number, expiration date, and security code, typically within about three to five business days.1Capital One. Credit Card Fraud Process The old card is deactivated, so every subscription, automatic bill pay, and stored payment method tied to it will fail on the next billing cycle unless you update it.
Capital One posts a list of your recurring charges and merchants that had the old card on file, so you can work through what needs updating.12Capital One. Replacement Cards Utilities, insurance premiums, streaming services, and gym memberships are the easy ones to miss, and a missed payment can trigger late fees or service cutoffs unrelated to the fraud itself.
When the Problem Goes Beyond One Account
If the suspicious activity suggests someone has your personal information rather than just a card number, take a few extra steps in the first day or two.
A fraud alert tells lenders to verify your identity before opening new accounts. An initial fraud alert lasts one year, and you only need to contact one of the three major bureaus; that bureau notifies the others.13Equifax. The FTC’s IdentityTheft.gov site generates a personalized recovery plan covering credit issues, debt collectors, compromised government IDs, and other downstream problems.15Federal Trade Commission. Keep an eye on your statements weekly for the next several months. Fraudsters who got your information once often run smaller test charges before trying anything bigger.
