The CGC evaluator test summary form is a one-page document you send to the AKC after every Canine Good Citizen testing event, whether you tested one dog or fifty. It records your evaluator information, the host club and location, the date, and the number of dogs tested, passed, and failed, broken out by program level. Submission is required after every S.T.A.R. Puppy, CGC, AKC Community Canine, or Urban CGC test, and the form creates the AKC’s official record of your testing activity.
Where to Find the Form
The Test Summary Form lives on the AKC’s CGC evaluator resources page and can be completed directly in the embedded online form. If the embedded version does not load, the AKC provides a direct link through its JotForm portal. There is no separate PDF form number to look up; the document is simply the “Evaluator Test Summary Form” or “Test Summary Form.”
What the Form Asks You to Report
The summary is short by design. You’ll provide:
- Your full name and the evaluator identification number assigned through AKC Canine College.
- The name of the club or organization hosting the test and the physical testing location.
- The exact date of the event.
- Totals for dogs tested, passed, and failed, reported separately for each program: S.T.A.R. Puppy, standard CGC, AKC Community Canine (CGCA), and Urban CGC (CGCU).
If you kept clean notes during the event, filling it out takes only a few minutes.
Why the Counts Have to Be Separated by Program
Each program level carries its own title suffix on a dog’s AKC record. Standard CGC passes earn the “CGC” designation; Community Canine passes earn “CGCA”;1American Kennel Club. Test Items for Community Canine Urban CGC passes earn “CGCU.”2American Kennel Club. About AKC Urban Canine Good Citizen If the tallies on your form get mixed, dogs can end up with the wrong title attached to their record.
How to Submit
The AKC expects the summary form after every event, with no exceptions for small tests.3American Kennel Club. CGC Evaluator Guide The simplest route is to complete and submit the online version on the evaluator resources page.
Individual CGC Test Registration Forms for each passing dog go in separately through the AKC’s secure online portal so owners can track status, and the AKC’s Title Application Portal (TAP) handles CGC title applications specifically.3American Kennel Club. CGC Evaluator Guide

The Deadline That Actually Matters
There is no rigid 30-day window for the summary form itself, but there is a hard outside limit on the paperwork chain: if an owner waits longer than one year to send their paperwork to the AKC, the dog has to be retested.3American Kennel Club. CGC Evaluator Guide Sending your summary form in soon after each event protects everyone from running into that cutoff.
Processing Fees Owners Pay
Owners, not evaluators, pay the per-dog processing fee. The amount depends on the program and whether the owner wants an official title or only a certificate:
- CGC as a title: $32. The dog receives the certificate, the “CGC” suffix is added to its name, and the title is logged on the permanent record. The dog must have an AKC registration number, a Purebred Alternative Listing (PAL) number, or a Canine Partners number.5American Kennel Club. AKC Canine Good Citizen Becomes a Title
- CGC certificate only, with no title added to the record: $25.5American Kennel Club. AKC Canine Good Citizen Becomes a Title
- AKC Community Canine (CGCA) title: $25.1American Kennel Club. Test Items for Community Canine
- Urban CGC (CGCU) title: $20.6American Kennel Club. AKC Urban Canine Good Citizen Evaluator Guide
The online portal takes credit card payment. For mailed submissions, checks or money orders should be made payable to the AKC. Mixed-breed dogs are not excluded from the title route; they qualify through the Canine Partners program.2American Kennel Club. About AKC Urban Canine Good Citizen
Records You Keep on Your Side
The summary form isn’t the only piece of paperwork you’re responsible for. At every event, complete a “Record of All Dogs Tested” listing each owner name, dog name, and pass/fail result, and keep it for at least one year. If an owner later misplaces their paperwork, the AKC may contact you to verify results.7American Kennel Club. ATT Evaluator Resources
The CGC Test Registration Form is a three-part form, and the yellow copy stays with you for one year.3American Kennel Club. CGC Evaluator Guide Keeping both the yellow copy and the Record of All Dogs Tested lets you resolve any later dispute without putting the owner through a retest.
Dismissals and How They Affect Your Counts
A few testing rules directly shape what you put on the summary form. Dogs are tested one at a time, not in groups, and tests cannot be held at the owner’s home or yard; a distraction dog and a “crowd” must be present.3American Kennel Club. CGC Evaluator Guide
Any dog that growls, snaps, bites, or attempts to attack a person or another dog must be dismissed from the test immediately. If the event is on AKC show grounds, notify the Show Superintendent and the AKC Field Representative. Aggressive behavior observed before, during, or after testing, including after a dog has already passed, calls for a written report to the AKC CGC Department.3American Kennel Club. On the summary form, a dismissed dog counts in your total tested but not in your passed count. Writing down what happened at the event itself makes the form straightforward and gives you documentation if the AKC follows up.
