Before any piercing at Claire’s, you fill out the Claire’s piercing registration form in the store. It’s a short intake document that records your contact details, screens for medical conditions that could complicate the procedure, and captures your signed acknowledgment of the risks. If the person being pierced is a minor, a parent or legal guardian signs on their behalf and must be present in the store.
Where and When You Fill It Out
The form is handled on-site, not online. Claire’s takes walk-ins during regular store hours and also lets you book an appointment in advance through its website,1Claire’s. Professional Ear Piercing Guide and Types of Piercing but either way the registration paperwork is completed in person at the piercing station. A store associate or Piercing Specialist hands it to you when you arrive. There is no remote submission option.
What the Form Asks For
The first section collects basic contact information: your legal name, address, and phone number. This becomes the store’s record of who received the service, so the details need to match your ID.
A medical screening section follows. Expect questions about nickel sensitivity, which is common with earring alloys, blood-thinning medications such as aspirin or warfarin, diabetes, and other conditions that can slow wound healing. Pregnancy and nursing should be disclosed as well, because they carry additional infection risk and the store may decline to perform the piercing.
The form also includes a liability waiver. Signing it means you acknowledge the inherent risks of piercing, including infection, irritation, and scarring. Answer every question honestly. Omitting a known allergy or medication gives the specialist no chance to catch a problem before the needle goes in, and it undermines the protection the form is meant to give you.
Identification You Need to Bring
Claire’s requires valid government-issued photo ID before any piercing, and accepts four forms:1Claire’s. Professional Ear Piercing Guide and Types of Piercing
- Driver’s license
- Passport
- Military ID
- Government-issued ID, such as a state ID card
The name on your ID has to match the name on the form. The specialist checks it to confirm your age and identity before starting.
Signing for a Minor
A minor cannot be pierced unless a parent or legal guardian is physically present in the store to sign.1Claire’s. Professional Ear Piercing Guide and Types of Piercing The signing adult needs to bring their own valid photo ID from the accepted list. Claire’s does not publish detailed guardianship documentation rules on its website, so if you are a legal guardian rather than a birth parent, bring your court-issued guardianship paperwork along with your ID so you aren’t turned away at the station.
Age thresholds vary by piercing type and by location. Claire’s will pierce babies as young as three months old if the child has had a DTaP vaccination, and if the baby is under three months you need documentation of the DTaP shot date.1Claire’s. Professional Ear Piercing Guide and Types of Piercing Cartilage and nose piercings have higher minimum age requirements than standard lobe piercings, and the specific cutoffs can differ by region. Call your local store before bringing a younger child in for anything beyond a lobe piercing.
What the Piercing Costs
There is no separate service charge for the piercing itself. It’s included free with the purchase of a starter kit that bundles the earrings and aftercare solution.1Claire’s. Professional Ear Piercing Guide and Types of Piercing Kits start at $29.99 and go up based on the earring material and style. You choose the kit during the consultation, before the registration form is finalized, so the jewelry decision is part of the same intake rather than a transaction afterward.
What Happens After You Sign
Once the form is complete and your ID has been checked, the Piercing Specialist walks you through a consultation, cleanses the piercing site, and marks the placement with a dot for your approval before piercing.1Claire’s. Professional Ear Piercing Guide and Types of Piercing The procedure itself takes only a few minutes. Before you leave, the specialist goes over aftercare with you.

- Wash your hands with antibacterial soap before touching the piercing.
- Clean the piercing three times a day with Claire’s Aftercare Solution on a cotton ball, applied front and back.

- Keep shampoo, hairspray, and perfume away from the site.
- Rinse after swimming, showering, or exercise, then reapply the aftercare solution.
- Leave the starter earrings in for the full recommended healing period.
