Early Warning Consumer Identification Form: Filing, Costs, and Follow-Up

The Early Warning Consumer Identification Form is a one-page identity verification document you send to Early Warning Services before the company will release your banking history file or act on a dispute. Early Warning is a nationwide specialty consumer reporting agency owned by Bank of America, Capital One, JPMorgan Chase, PNC Bank, Truist, U.S. Bank, and Wells Fargo, and it tracks checking and savings account activity that banks consult when you apply to open an account.1Early Warning. Consumer Report You can download the form from earlywarning.com and submit it by mail, fax, or a secure online portal.2Early Warning. FAQs for Requesting Your File Disclosure

When You’d Use This Form

Two situations pull most people to the form. The first is a request for your file disclosure, which is the record Early Warning keeps about your checking and savings history. The Fair Credit Reporting Act gives you the right to see what any consumer reporting agency has on you, including specialty agencies like this one.3Consumer Financial Protection Bureau. The second is a bank denial. When a bank turns you down for an account based on a consumer reporting agency’s data, it must send you an adverse action notice naming the agency and reminding you that you have 60 days to request a free copy of the report used against you.4Office of the Law Revision Counsel. 15 USC 1681m – Duties of Users Taking Adverse Actions on the Basis of Information Contained in Consumer Reports If that notice names Early Warning, this form is how you start the process.

You’ll also need the form before Early Warning will open a dispute about something already on your file. The company won’t investigate or release data until it has verified your identity.

What to Gather Before You Start

The form is short. The prep is where people stumble. Pull the following together first:

  • Your full legal name, including middle name and any suffix, spelled exactly as it appears on your government ID.
  • Your Social Security number, all nine digits.
  • Your date of birth.
  • Your current physical address, plus your previous address if you’ve moved recently, so Early Warning can locate every record tied to you.2Early Warning. FAQs for Requesting Your File Disclosure
  • A clear photocopy of one government-issued photo ID: driver’s license, state ID, U.S. passport, or similar.

No notary is required. Your signature on the certification section is what makes the form valid; it’s your attestation that the information you’ve given is complete and accurate.5Early Warning. Consumer Identification and Certification Form

Filling Out the Form

Download the Consumer Identification and Certification Form from earlywarning.com.6Early Warning. Your Early Warning File Disclosure It’s a single-page PDF. If you’re writing by hand, use block letters so nothing gets misread.

Enter your name, Social Security number, date of birth, and current address in the labeled fields. Add your prior address where indicated if you’ve moved. Check the spelling of your name against your photo ID character by character. “Catherine” on the form and “Katherine” on the license will trigger a delay or outright rejection.

Photocopy your photo ID. The copy must show your full name, photo, and ID number clearly. If the address on your license doesn’t match the address you wrote on the form, add a second document showing your current address, such as a recent utility bill or bank statement. The FAQ doesn’t list this as required, but including it heads off a follow-up request.

Finish with the certification: sign, date, and print your name under the signature.

  • You’re requesting within 60 days of an adverse action notice from a bank that used Early Warning data.
  • You certify in writing that you’re unemployed and plan to apply for work within 60 days.

What Happens After You Submit

Early Warning must respond to a standard file disclosure request within 15 days of receiving your complete identification package.7Consumer Financial Protection Bureau. Early Warning Services, LLC The clock starts on receipt, not on the date you mailed it. The portal cuts several days off the overall turnaround.

If you’re also filing a dispute, federal law gives Early Warning 30 days from receipt to investigate by contacting the bank that reported the information. If you send supporting documents during that window, the investigation can be extended by up to 15 days, unless Early Warning has already determined the information to be inaccurate, incomplete, or unverifiable.9Office of the Law Revision Counsel. 15 USC 1681i – Procedure in Case of Disputed Accuracy You’ll get results within five business days of the investigation closing.10Consumer Financial Protection Bureau. If your identification is insufficient, Early Warning will mail a notice asking for more documentation and will pause the timeline until you reply. Clean paperwork the first time is the single biggest factor in how fast you hear back. All responses, including file disclosures and requests for more information, come by postal mail to the address on your form.

If Identity Theft Is Behind the Request

If someone opened accounts in your name and those accounts are on your Early Warning file, add two pieces to the package you send.

First, file a report with your local police department or with the FTC at identitytheft.gov. For blocking purposes under federal regulation, the report must allege identity theft with as much detail as you can provide and must be filed with a federal, state, or local law enforcement agency.11Consumer Financial Protection Bureau. Definitions Filing a false report is a crime, which is what gives the document its legal weight.

Second, send a copy of that report along with your identification form. A detailed police report with the officer’s signature and badge number is usually enough on its own. A thin automated printout may prompt Early Warning or the reporting bank to ask you to complete a notarized Identity Theft Affidavit; the CFPB and FTC each publish acceptable versions.11Consumer Financial Protection Bureau. Fraud is also a standalone basis for a free file disclosure, even if you’ve already used your annual free copy.8Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures
