A fax cover sheet example has the same core layout whether it goes out from an accounting office or a hospital records room: a header identifying sender and recipient, a subject line, a page count, a short message, and (when the contents are sensitive) a confidentiality notice at the bottom. Two filled-in samples are below, one for standard business use and one for healthcare or other confidential transmissions, along with the fields you need to plug into either.
Fields Every Cover Sheet Needs
Miss one of these and your fax is likely to sit unrouted on someone’s desk.
- To: Recipient’s full name, title, and department.
- From: Your full name, title, and organization.
- Fax number: The recipient’s fax number, plus your own so they can reply.
- Phone number: A direct callback line for both parties.
- Date: The date of transmission.
- Subject (RE:): A short description of what’s attached.
- Page count: Total pages including the cover sheet. Five pages of content means you write “6 pages.” This is how the recipient confirms the whole transmission came through.
- Message: A brief note with any instructions or context.
Microsoft Word and Google Docs both ship with fax cover sheet templates. Search “fax” in the template gallery and fill in the blanks. Keep the layout clean, use a legible font, and make the page count impossible to miss.
Standard Business Fax Cover Sheet
A business cover sheet is built for quick scanning. Anyone processing incoming faxes should be able to route yours from the top third of the page alone.
FACSIMILE TRANSMITTAL
- TO: John Doe, Regional Manager
- FROM: Jane Smith, Accounts Payable
- DATE: March 10, 2026
- FAX: (555) 012-0199
- PHONE: (555) 012-0100
- RE: Invoice #44521 Processing
- PAGES: 3 (including cover)

Two sentences in the message, no more. The cover sheet is for routing and verification. If the recipient needs background, it belongs in the attached documents, not here.
Confidential and Healthcare Fax Cover Sheet
Sensitive faxes need a confidentiality notice at the bottom of the cover sheet. Healthcare, legal, and financial offices all use one. A misrouted fax carrying patient records or account data can create real liability, and the disclaimer is the standard way to show a good-faith safeguard is in place.
For healthcare specifically, federal privacy rules at 45 CFR 164.530 require covered entities to maintain “appropriate administrative, technical, and physical safeguards to protect the privacy of protected health information.”1eCFR. 45 CFR 164.530 – Administrative Requirements The regulation doesn’t dictate specific cover sheet wording, but a confidentiality disclaimer has become the accepted way to satisfy that safeguard requirement when faxing protected health information.
CONFIDENTIAL FACSIMILE TRANSMITTAL
- TO: Dr. Sarah Chen, Cardiology
- FROM: Mark Rivera, Patient Records, Valley Medical Group
- DATE: March 10, 2026
- FAX: (555) 012-0250
- PHONE: (555) 012-0200
- RE: Patient Referral Documentation
- PAGES: 5 (including cover)
Message: Attached are the requested referral records. Please confirm receipt by phone.
CONFIDENTIALITY NOTICE: This fax contains information that is privileged and confidential, intended only for the individual named above. If you are not the intended recipient, please notify the sender immediately at the phone number listed and destroy all copies of this transmission. Unauthorized review, distribution, or use of this information is prohibited.
Put the disclaimer in a clearly separated block at the bottom of the page. Bold the heading so anyone handling the document sees it. Law offices use nearly identical language to protect attorney-client privilege during document exchanges.
Why the Disclaimer Matters
HIPAA penalties are tiered by the level of negligence involved. Under 45 CFR 160.404, the base statutory penalties for violations a covered entity didn’t know about start at $100 per violation; for willful neglect that goes uncorrected, the floor rises to $50,000 per violation with an annual cap of $1.5 million for identical violations.2eCFR. 45 CFR 160.404 – Amount of a Civil Money Penalty These amounts are adjusted for inflation each year. A confidentiality notice on the cover sheet won’t erase a mistake, but it documents the kind of good-faith safeguard that keeps you out of the worst penalty tiers.
Priority Labels for Time-Sensitive Faxes
If a fax needs quick handling, add a priority label near the top of the cover sheet where a busy office can’t miss it.
- Urgent: Immediate attention. Medical referrals, time-sensitive contracts, same-day deadlines.
- For Review: Needs the recipient’s input but isn’t an emergency.
- For Your Records: Informational. No action needed.
- Please Reply: The sender is waiting on a response or signature.
Put the label at the top of the page or directly under the transmittal header. An unlabeled fax in a busy office goes to the bottom of the pile.
Confirming the Fax Was Received
Every fax machine and online fax service produces a confirmation report after transmission. It shows the date, time, recipient number, and number of pages successfully sent. Keep it. This is your proof of delivery if the recipient later claims nothing arrived, and if the report shows an error or a short page count, resend right away.
Healthcare organizations subject to HIPAA should retain fax audit logs for at least six years. Other businesses generally keep confirmations in the three-to-seven-year range for legal and financial records, following their own retention policy. If transmission fails repeatedly, a quick phone call to the recipient to verify the fax number is active saves more time than another round of retries.
