Form 1A Medical Certificate: Eligibility, Filing, and Validity

The Form 1A medical certificate is a fitness document, signed by a State Government-appointed medical practitioner, that you must submit when applying for or renewing certain Indian driving licences. Under Rule 5 of the Central Motor Vehicles Rules, 1989, it is mandatory for any transport vehicle licence and for any non-transport licence renewal once the holder has turned 40.1Mizoram Transport Department. The Central Motor Vehicles Rules, 1989 The certificate confirms that a doctor has personally examined you and found you physically fit to drive.

Who Needs to Submit Form 1A

Two things decide whether the form applies to you: the class of licence and your age.

If your application involves a transport vehicle, meaning a commercial truck, bus, taxi, or goods carrier, Form 1A is required regardless of age. This covers a new learner’s licence, a fresh driving licence, and any renewal.1Mizoram Transport Department. The Central Motor Vehicles Rules, 1989

For non-transport vehicles like private cars and motorcycles, a younger applicant only needs Form 1, a self-declaration. Form 1A becomes mandatory at renewal once the licence holder has attained the age of 40. Section 15 of the Motor Vehicles Act, 1988 requires the same medical certificate at that point as is required for a transport vehicle applicant.2India Code. The Motor Vehicles Act, 1988

One catch worth knowing before you go to the RTO: even when Form 1A is required, you still attach a completed Form 1 alongside it. The medical certificate itself notes that the applicant’s self-declaration in Form 1 should be filed with it. A signed Form 1 cannot substitute for Form 1A where the rules require the medical certificate, and submitting the wrong form will result in rejection.3Ministry of Road Transport and Highways. What the Medical Examination Covers

The examining doctor works through the form and evaluates the areas of physical fitness that matter for driving.3Ministry of Road Transport and Highways. Form 1A – Medical Certificate

  • Vision. You must be able to read a motor vehicle number plate at 25 metres in daylight. The doctor also screens for night blindness and, using an Ishihara chart, for colour blindness. Severe or total colour blindness is disqualifying because traffic signals cannot be reliably distinguished.
  • Hearing. The doctor confirms you can hear ordinary sound signals such as horns and sirens.
  • Limbs and joints. Your arms, legs, hands, and joints are examined for any defect, deformity, or loss that would interfere with operating a vehicle.
  • Loss of consciousness. Any history of epilepsy, fainting, or similar conditions that could cause sudden incapacitation is assessed.

Blood group is optional on the form. If you want it printed on your driving licence, you can supply it, but leaving it blank does not invalidate the certificate.3Ministry of Road Transport and Highways. Form 1A – Medical Certificate

Drivers applying to carry dangerous or hazardous goods face additional testing. The doctor must personally check reaction time, side vision, and glare recovery in addition to the standard evaluation.3Ministry of Road Transport and Highways. Form 1A – Medical Certificate

Which Doctors Can Sign the Form

Not every physician qualifies. Section 8(3) of the Motor Vehicles Act, 1988 requires the certificate to be signed by a registered medical practitioner appointed for the purpose by the State Government, or by a person the State Government has authorised through an official gazette notification.4Indian Kanoon. Section 8 in The Motor Vehicles Act, 1988 A certificate from a family doctor who has not been designated by the State may be refused at the RTO. Confirm the practitioner’s authorisation before booking the examination.

The completed form must carry the doctor’s signature, name, designation, official seal, and registration number. A passport-sized photograph of the applicant is affixed to the form, and the doctor signs across it so that part of the signature falls on the photograph and part on the certificate itself. This prevents anyone from swapping photos after the examination.3Ministry of Road Transport and Highways. Form 1A – Medical Certificate

How to Upload Form 1A on the Sarathi Portal

Once the doctor has completed and signed the certificate, you upload a scanned copy to the Sarathi Parivahan portal so the RTO can review it with the rest of your application.6Parivahan Sewa. FAQ – Document Management Services

  • Go to sarathi.parivahan.gov.in and select your state.
  • Click “Upload Document” and then “Upload Document/Scanned images.”
  • Upload a single JPEG or PDF of the signed and attested Form 1A.

Document management services are customised by each state’s transport department, so the exact workflow can vary. If the file is rejected as too large, compress it before trying again. Error messages usually point to a problem with the file size or file name.

Keep the original physical copy after uploading. The RTO can ask for it during manual verification at your appointment or driving test, and any mismatch between the scan and the hard copy can hold up your application. Make sure the scan is legible and complete.

How Long Form 1A Is Valid

A Form 1A medical certificate is valid for one year from the date of issue.1Mizoram Transport Department. The Central Motor Vehicles Rules, 1989 If your licence process runs past that window, you will need a fresh examination and a new certificate. Commercial drivers who renew on a regular cycle should schedule the medical close to the renewal date rather than months ahead.

If You Are Found Medically Unfit

If the examination reveals a condition that makes driving dangerous, the licensing authority will refuse to issue or renew your licence under Section 8(4) of the Motor Vehicles Act. There is one exception: where the authority is satisfied that you can safely operate an adapted vehicle designed for your condition, it may issue a licence limited to that vehicle type.4Indian Kanoon. Section 8 in The Motor Vehicles Act, 1988

A refusal or revocation must come with written reasons, and Section 17 gives you 30 days from receiving that order to file an appeal with the prescribed appellate authority.2India Code. The Motor Vehicles Act, 1988
