FSU Membership Justification Form: Fields, Signatures, and Payment

To get FSU to pay your professional organization dues, complete the FSU Membership Justification Form from the Controller’s Office, write a justification that ties the membership to the university’s work rather than your personal credentials, confirm the organization has a current Certification of Payment with Public Funds on file, obtain your Dean, Director, Department Head, or Chair’s signature, and submit the signed form with a Payment Request Form and the invoice to Accounts Payable.1Florida State University. Gather Three Things Before You Open the Form

Mid-process scrambling is what slows these payments down. Have these ready first:2Florida State University. Creating a Membership Payment Request Form

  • The invoice or dues notice from the organization, showing the exact cost, the membership period, and the organization’s full legal name.
  • Confirmation from the Vendor Certification List on the Controller’s Office Accounts Payable page that the organization has a current Certification of Payment with Public Funds on file. If not, they will need to complete one before any payment goes through.3Florida State University. Accounts Payable – Controller’s Office
  • The correct account code: 741923 for an institutional membership (held by FSU) or 741924 for an individual membership (held by a specific employee).2Florida State University. Creating a Membership Payment Request Form

Filling In the Form Fields

  • Organization name, written out in full exactly as it appears on the invoice.
  • Start and end dates of the membership period.
  • The dues amount, matching the invoice to the dollar.

If the organization’s rates change between drafting and submission, update the form. A mismatch between the form and the invoice is one of the common reasons packets come back.

Writing the Benefit Justification

This is where approvals are won or lost. Reviewers spend their time on this section, and a generic paragraph about professional development will not pass.

Florida Statute 216.345 lets state funds pay membership dues only when the membership is essential to the agency’s statutory duties, and it specifically prohibits using public money to maintain someone’s professional or trade credentials.4Florida Legislature. Florida Statutes 216.345 – Professional or Other Organization Membership Dues; Payment Framing the request as career maintenance is the fastest route to denial.

Write about what FSU gets. Name the research databases the department uses through the membership. Specify conferences where your unit presents and the registration savings members receive. Point to accreditation-related standards or program resources the organization provides. Concrete beats abstract every time.

If your membership happens to carry a licensing component alongside genuine university benefits, lead with the latter and be straightforward about the former. Reviewers recognize a personal credential dressed up as an institutional need.

The statute favors institutional memberships, where FSU itself holds the membership, over individual ones. An institutional membership that several department members can use clears the bar more easily. If you are requesting an individual membership, your justification needs to show one of two things: the organization does not offer an institutional option, or the individual rate is cheaper for the access FSU actually needs.4Florida Legislature. Confirm the Vendor Certification

This step catches people off guard and will stop a payment cold regardless of how good the rest of the packet looks. Because paying membership dues with public funds brings the organization’s related financial and membership records into Florida’s public records law, the organization has to acknowledge that obligation on a Certification of Payment with Public Funds form.5Florida State University. Certifications are valid for five years, and many well-known organizations are already on FSU’s Vendor Certification List. Check the list first. If your organization is not listed, send them the blank certification form from the Controller’s Office forms page, ask them to complete it with their Federal Employer Identification Number, and have it emailed to AccountsPayable@admin.fsu.edu. Nothing moves until that certification is on file.5Florida State University. Certification of Payment with Public Funds

Getting the Signature and Submitting Payment

The FSU President has delegated signature authority on this form to your Dean, Director, Department Head, or Chair. Get that signature before moving to payment processing.1Florida State University. University Membership Justification Form

Two payment paths exist once the form is signed.

Payment Request Form Route

Complete a Payment Request Form and attach the signed Justification Form plus the invoice. Verify the vendor’s certification status one more time against the Vendor Certification List, then submit the full package to Accounts Payable. All three items need to be in hand before you submit.2Florida State University. Creating a Membership Payment Request Form

PCard Route

Memberships are authorized PCard purchases, but only if the approved Membership Justification Form and the approved Certification of Payment with Public Funds are both completed before you make the purchase. If the approvals come after the charge, the purchase may be treated as unauthorized and the cardholder may have to reimburse FSU personally. Keep all documentation with your receipts for reconciliation and audit.6Florida State University. PCard – Procurement Services

Mistakes That Delay or Derail Approval

  • No vendor certification on file. Check the list early, especially for smaller or niche organizations that may never have dealt with a Florida state agency.
  • Justification that reads as personal benefit. “Helps me stay current” gets kicked back. Rewrite around specific FSU-facing resources, cost savings, or program standards.
  • Wrong account code. Using 741923 for a membership held in your own name, or 741924 for one held by FSU, creates an accounting mismatch that slows processing.
  • PCard purchase before approval. Charging the card first and chasing signatures afterward puts the cardholder on the hook for reimbursement.
  • Dollar amount on the form that does not match the invoice. Update the form if rates change before you submit.
