The UNICEF USA donation form is a printable PDF available at unicefusa.org that you complete by hand or on-screen, then mail with a check, money order, or credit card details to UNICEF USA, 125 Maiden Lane, New York, NY 10038.1UNICEF USA. What the Form Asks For
Print the PDF after downloading it, or fill it in on your computer before printing. Either way, type or write clearly. Illegible handwriting is the easiest way to delay processing.
The form asks for:
- Your full name, as it appears on your check or credit card.
- Your mailing address: street, city, state, and zip code. If you’re paying by credit card, the zip code needs to match what your card issuer has on file.
- Your phone number.
- Your email address, which is where any electronic acknowledgment will go.
- Your donation amount.
The form also includes a checkbox for email updates about UNICEF’s work. Select the option that matches what you want before you send it in.1UNICEF USA. UNICEF USA Donation Form
How to Pay Through the Mail-In Form
You have two payment choices on the paper form: a check or money order, or a credit card.
If you’re paying by check or money order, make it payable to U.S. Fund for UNICEF. That is the legal payee name, not “UNICEF” or “UNICEF USA,” and getting it right avoids a rejected deposit.
If you’re paying by credit card, the form has fields for the card number, expiration date, and cardholder name. It does not ask for the CVV security code.1UNICEF USA. UNICEF USA Donation Form Since you’ll be putting your card number on paper and dropping it in the mail, consider sending it in a sealed envelope you drop at a post office rather than leaving it in an unsecured mailbox.
One boundary worth knowing: the paper form does not support ACH bank transfers. That option is available only through the online donation portal.1UNICEF USA. UNICEF USA Donation Form
Where to Mail It
Send the completed form, with your check, money order, or credit card details, to:

If you’re trying to have the gift count toward the current tax year, remember that mail takes time. Processing is based on when UNICEF USA receives and records the gift, so send well before December 31 if year-end timing matters.
If You’d Rather Not Mail Paper
The same donation can be made online at unicefusa.org/donate in about two minutes. The online form accepts Visa, Mastercard, American Express, Discover, Apple Pay, Google Pay, and direct bank transfer, and it sends an emailed receipt within minutes.3UNICEF USA. Your Receipt and Tax Record
After UNICEF USA processes a mailed donation, expect a written acknowledgment to arrive within a few weeks. Keep it. That letter is your tax record.
For any single donation of $250 or more, the IRS requires a written acknowledgment from the charity that includes the organization’s name, the cash amount, and a statement about whether you received anything in return for your gift.5Internal Revenue Service. Charitable Contributions – Written Acknowledgments UNICEF USA’s standard receipt covers all three. UNICEF USA is a 501(c)(3) nonprofit, so a cash gift is deductible if you itemize.6UNICEF USA. Checking Your Form Before It Goes in the Envelope
A quick pass before you seal the envelope saves a lot of back-and-forth:
- Name, address, phone, and email are legible.
- Donation amount is written clearly, and if you’re paying by check, the numeric and written amounts on the check match.
- The check is made out to U.S. Fund for UNICEF, not to UNICEF or UNICEF USA.
- If paying by credit card, the card number, expiration date, and name on the card are complete and readable.
- The email-updates checkbox is set the way you want.
- Both the form and the payment are in the envelope.
If something is missing or unclear after you mail it, call donor services at 1-800-367-5437 or email support@unicefusa.org before sending a second form, so your gift isn’t recorded twice.
