Illinois DHS Form 2540, the “Verification of Living With” form, is how the Illinois Department of Human Services confirms that the people on your benefits case actually live together at the address you gave.1Illinois Department of Human Services. WAG 14-06-04-a – Proof of Meeting the Requirement If a caseworker sent it to you, fill it in, sign it, and return it to your Family Community Resource Center by the date on your Verification Checklist. In most cases that is 10 calendar days from the date the request was issued, and missing it means your application is denied the first workday after the deadline.2Illinois Department of Human Services. What the Form Is and Why You Got It
Form 2540 is a verification document, not an application. When you apply for SNAP, cash assistance, or medical coverage in Illinois, a caseworker reviews who you listed as part of your household in order to set eligibility and benefit amounts. If the worker needs written proof that those people actually share your address, they issue Form 2540.1Illinois Department of Human Services. WAG 14-06-04-a – Proof of Meeting the Requirement
You might receive it at your initial eligibility interview, during a mid-certification review, or any time the caseworker has reason to question your household composition. It usually arrives alongside a Verification Checklist (Form IL444-0267), the cover sheet that lists every document DHS wants, who needs to provide it, and the last day it is due.3Illinois Department of Human Services. WAG 02-07-02 – Obtaining Verifications
If you landed here looking for the actual application for benefits, that is a different form: IL444-2378B, available at any Family Community Resource Center or online through the Application for Benefits Eligibility portal at abe.illinois.gov.4Illinois Department of Human Services. How to Fill It Out
Print clearly and complete every field the form asks about. List each person who lives at your address and your relationship to each of them. Do not leave anyone off, even people who are not part of your benefit household. The caseworker needs to see the full living situation to calculate your benefits correctly, so if someone shares the home but is not applying with you, write them in and note that.
Sign and date the form before you send it back. An unsigned form is not treated as returned, and the clock keeps running against you.
The Deadline and How to Return It
The last day to return Form 2540 is printed on your Verification Checklist, and it will always fall on a workday.3Illinois Department of Human Services. WAG 02-07-02 – Obtaining Verifications In most cases you have 10 calendar days from the date DHS issued the request. Check the checklist for your exact date rather than counting on your own.
You can return the form three ways:
- Mail it to your assigned Family Community Resource Center.
- Fax it to the office handling your case.
- Hand-deliver it to the office.
If you are close to the deadline, hand-delivery is the safest option because you can ask for a date-stamped receipt confirming the office logged it. Mail and fax can be delayed, and the burden of proving you returned the form on time is on you.
If you do not know which office serves you, use the DHS Office Locator at illinois.gov and search by county, or by zip code if you are in Cook County.6Illinois.gov. Human Services Office Locator
What Happens If You Miss the Deadline
If Form 2540 is not returned by the due date on your Verification Checklist, the caseworker is required to deny your application the first workday after the deadline passes.2Illinois Department of Human Services. WAG 02-08-03-b – Other Verification The denial is for failure to provide requested verification, not a decision on the merits of your eligibility.
That distinction matters, because it shapes your next step. You do not have to wait for a hearing to try again. You can simply file a new application and submit the Form 2540 along with it. If you prefer to challenge the denial itself, you have 90 days from the date on the notice to request a fair hearing on a SNAP decision, and 60 days for a cash assistance or Medicaid decision. If DHS failed to act on your application at all, there is no deadline for requesting a hearing.7Illinois Department of Human Services. Appeals and Fair Hearings For Those Receiving Cash, SNAP, or Medical Assistance
For most people whose only problem was a late form, reapplying is faster than appealing. The trade-off is that your benefit start date resets to the new application date, so file the new application the same day you realize the first one was denied.
If Someone Else Needs to Fill It Out for You
You do not have to complete Form 2540 yourself. Another household member, or any adult who is familiar with your situation, can fill it out and return it on your behalf.8Illinois Department of Human Services. SNAP households can also designate an approved representative — an adult outside the household authorized to handle applications and paperwork, report changes, and use benefits. The designation has to be in writing, signed by the head of the household, and kept in the case file.9Illinois Department of Human Services. Related Documents DHS May Request
Form 2540 is one of several verifications DHS can ask for. The Verification Checklist that came with it may also list proof of residency such as a lease, rent receipts, a mortgage book, or a utility bill, along with Social Security numbers for household members, pay stubs or benefit award letters for income, bank statements for assets, and shelter and utility costs.10Illinois Department of Human Services.
