To complete Form R002 for US Club Soccer, log in to GotSport, open the player’s dashboard, and fill out the three parts of the form in one sitting: player information and emergency contacts, medical treatment authorization with your health insurance details, and the liability waiver signed by a parent or guardian for any player under 18. Youth competitive players also upload proof of birth, and players turning 18 during the seasonal year complete SafeSport training before a passcard is issued. Registration for the 2026–27 seasonal year opens June 1, 2026, and covers play from August 1, 2026, through July 31, 2027.1US Club Soccer. Registration
Have This Ready Before You Log In
The form moves faster if you gather the information first. You’ll need your health insurance policy number, provider name, and group number; notes on allergies, current medications, and any chronic conditions; and names and phone numbers for at least two emergency contacts reachable during games and travel.
Youth competitive players also need a legible scan of a proof-of-birth document: a birth certificate, passport, driver’s license, or a document issued by the U.S. Department of Homeland Security or U.S. Citizenship and Immigration Services.2US Club Soccer. Player Registration A player photo is required too. Think passport style: full face, hair, and down to the base of the neck, in focus, with good contrast. Not an action shot from the sideline.
Your club administrator sets the player up in GotSport with the correct affiliation and competitive level, and that step is what populates the registration requirements on the player’s dashboard. If you’ve never logged in before, ask your club which email address they used, then use the Forgot Password link to set your credentials.3GotSport Support. Filling Out Form R002
Form R002 bundles three documents into one: the Player Information section, the Medical Treatment Authorization, and the Liability Waiver/Release and Consent. Parents of minor players complete it online in GotSport, or the club registrar can certify that a paper copy is already on file.2US Club Soccer. Player Registration Clubs keep the completed form for at least five years or until the player turns 18, whichever is later.
Player Information and Emergency Contacts
The first section collects the player’s full legal name, date of birth, home address, and contact information for at least one parent or guardian. This is what ties the player to their club in the national database and determines the age group and region assigned to them. Check the spelling of names and the date of birth carefully. Any mismatch with the proof-of-birth document will stall verification.
The emergency contact fields take names and phone numbers for people who can be reached during practices, games, and away tournaments. Listing a single contact is possible, but two is far more practical. Coaches and tournament staff use these numbers when a parent can’t be located, so pick people who actually pick up the phone.
Medical Treatment Authorization
The medical section captures your health insurance policy number and provider name along with any allergies, medications, or chronic conditions. That information feeds the Medical Treatment Authorization clause, which gives designated adults such as coaches and club officials the authority to consent to emergency medical care when a parent or guardian can’t be reached. That authority extends to x-rays, anesthesia, and surgical procedures in urgent situations.4US Club Soccer. Forms and Documents
Incomplete insurance information can hold up the player’s registration status or make them ineligible for sanctioned events. If the player is uninsured, note that on the form. Registration includes secondary accident insurance through US Club Soccer, but it is an excess policy that pays after a primary health plan has processed a claim.5US Club Soccer. Youth Summary
Liability Waiver and Consent
The final section acknowledges the inherent risks of competitive soccer, including serious injury, and releases US Club Soccer and its member organizations from certain negligence claims. For any player under 18, a parent or legal guardian must be the one to sign. Read the language before you sign. Once executed, it limits your ability to bring certain legal claims related to injuries sustained during sanctioned activities.
Uploading Proof of Birth
Proof of birth is required only for youth competitive players, and you only need to submit it once. It carries over across seasons.2US Club Soccer. Player Registration Recreational players are exempt.
Accepted documents are a birth certificate, passport, driver’s license, or a document issued by the U.S. Department of Homeland Security or U.S. Citizenship and Immigration Services. US Club Soccer may also accept other documents on a case-by-case basis.2US Club Soccer. Player Registration The scan has to be legible with the full name and date of birth clearly visible.
In GotSport, open the Account menu, select Family, click into the player’s profile, and open the Documents tab. Click New Document, choose Birth Certificate from the document repository, upload the file, and save. Files must be under 20 MB.6GotSport Support. As a Parent – How and Where Do I Upload a Birth Certificate If Birth Certificate isn’t an option in the menu, the club hasn’t created that repository yet. Contact your club administrator to set it up.
US Club Soccer staff verify the uploaded document against the information on the registration form. Altered proofs of birth are flagged to the Safeguarding and Compliance team and result in disciplinary action, including suspension.2US Club Soccer. Player Registration
When SafeSport Training Is Required
SafeSport doesn’t apply to every player, but when it does, skipping it blocks the registration entirely. No passcard is issued and the player won’t appear on an official roster.2US Club Soccer. Player Registration
The training is required for two groups:
- Youth players turning 18 during the seasonal year. For 2026–27, that means anyone born on or before July 31, 2009, and the training must be completed on or after June 1, 2026.2US Club Soccer. Player Registration
- Adult players on a team that includes any minor athlete. If you’re 18 or older and even one teammate is under 18, you need the training.
Only four courses satisfy the requirement: SafeSport Trained – U.S. Soccer Federation (the core course) and SafeSport Refreshers 1, 2, and 3. Other SafeSport courses, including SafeSport for Youth Athletes, SafeSport for Adult Athletes, SafeSport for Kids, and the Parent’s Guide to Misconduct in Sport, do not count.7GotSport Support. US Club Soccer – As a Parent/Player – How to Complete SafeSport
For a minor player who needs the training, a parent or guardian first signs a Parent Acknowledgement in the player’s GotSport profile. Only then can the player access the course. The SafeSport credentials must belong to the player, not the parent. New users create an account at SafeSportTrained.org using the player’s information.7GotSport Support. US Club Soccer – As a Parent/Player – How to Complete SafeSport
Players who meet the SafeSport age threshold also trigger the Sex Offender Registry–Adverse Eligibility List Review, which the club administrator completes on the player’s behalf. Like SafeSport, it must be finished before a passcard is issued.2US Club Soccer. Player Registration
Finishing Registration and Getting the Player Pass
When every requirement shows complete on the GotSport dashboard, the player is fully registered. That means Form R002 signed, proof of birth uploaded and verified, SafeSport completed if applicable, and the registration fee paid. The club administrator handles the membership purchase and fee payment on behalf of the player.2US Club Soccer. Player Registration Fee amounts vary by club, since local organizations usually add their own administrative costs on top of the national US Club Soccer membership fee.
A successful registration generates a player pass, either digital or physical, with the athlete’s photo and a unique ID number. The pass is the player’s credential for match-day check-ins and tournament entry. Passcards and rosters for the 2026–27 season are valid starting August 1, 2026.1US Club Soccer. Registration Keep a copy of the completed form and uploaded documents for your own records.
If the Player Comes From Abroad
Form R002 isn’t the whole picture for a player over 10 who was previously registered with a soccer club in another country, or who has never been registered anywhere and is not a U.S. national. Those players need international clearance through U.S. Soccer before US Club Soccer can complete the registration, regardless of skill level or how seriously they competed abroad.8U.S. Soccer. International Clearance
Players aged 9 and under use a simplified “Prior to Ten” (P10) registration form instead, regardless of citizenship. While a clearance application is pending, the player may take part in preseason scrimmages, team training, and non-affiliated matches, but playing in any sanctioned competition before clearance comes through makes the player ineligible.8U.S. Soccer. International Clearance
If the Player Needs to Guest for Another Club
A registered player who wants to guest play for a different club does not re-register. The player’s existing registration and pass stay tied to the home club, and a temporary loan is processed through GotSport. Starting with the 2026–27 season, all loans must be submitted using the US Club Soccer Player Loan Form inside GotSport. Loan requests sent through Zendesk support tickets are no longer accepted.4US Club Soccer. Forms and Documents Instructions are built into the form itself.
