How to Complete and Submit the FYSA Guest Player Form: Approvals and Check-In

The FYSA Guest Player Form is a digital authorization in GotSport that temporarily adds a registered youth soccer player to another team’s roster for a single tournament or competition. Either the player’s home club or the borrowing club can start it, both club registrars must approve it, and the completed PDF gets uploaded to the tournament’s online check-in portal.

Who Can Guest Play

Under FYSA Rule 209.1, the player must already be registered with FYSA or another US Youth Soccer affiliate, and the primary club must grant written permission through the guest player form before the player suits up elsewhere. Permission covers one event only. A separate form is needed for each tournament.1Florida Youth Soccer Association. FYSA Rules March 2026 – Rule 209

A player can generally guest up to the next older age group without extra steps. Playing up more than one birth year requires following the separate play-up procedure in the FYSA Rules. The sending club’s agent of record (or their designee) has to approve the request and can deny it at their discretion. Coaches who contact a player before that approval risk a poaching charge.1Florida Youth Soccer Association. FYSA Rules March 2026 – Rule 209

One rule worth knowing before you start: a player who guest plays with another team cannot transfer to that team for the rest of the seasonal year unless the original club gives written permission. If both teams belong to the same club, no guest player form is needed at all.1Florida Youth Soccer Association. FYSA Rules March 2026 – Rule 209

What to Have Ready Before You Open GotSport

The form pulls information from both clubs, and which fields you fill in depends on your role.

If you are the receiving organization (the club borrowing the player), have the loaning club name, loaning team name, the player’s full name, date of birth, and pass ID number, plus the email address linked to the player’s GotSport profile and their zip code. Both the email and zip code are required to add the player’s profile to your organization so they appear on the event roster.

If you are the sending organization (the player’s home club), have the tournament name and dates, the receiving club name, and the receiving team name.2Florida Youth Soccer Association. How To Complete a Guest Player Custom Transfer Form and Add the Guest Player to the Event Roster

Confirm the player’s FYSA membership is active before starting. Passes and rosters only become available once a player is fully registered and paid.3Florida Youth Soccer Association. Player and Coach Membership

Filling Out the Form in GotSport

Either side can start it. The steps are the same:

  • Log in to GotSport and go to Dashboard → Affiliations. Click FYSA Available Forms, scroll to “FYSA Guest Player Form,” and select it.
  • Double-click the blue banner at the top to set your role. Choose Sending Organization if the player belongs to your club, or Receiving Organization if the player is guesting with your club.
  • Complete the required fields, add a digital signature, and click Save.
  • Review the summary screen and click Return.

Setting the role correctly is where most people slip up. The wrong selection shows you fields you can’t fill in and hides the ones you need.

Getting Both Clubs to Approve

Once one side submits, the other has to log in and approve. Go to Club Management → Forms and Requests, select FYSA Guest Player Form from the Form drop-down, and search. Click the ID number for the player, then click the green check mark for your organization.2Florida Youth Soccer Association. The form is complete when both the sending and receiving organizations show green check marks. FYSA does not need to approve the form separately. The receiving club then downloads the approved form as a PDF by clicking the three dots on the far right of the page and selecting View as PDF. That PDF is what gets uploaded for tournament check-in.2Florida Youth Soccer Association. How To Complete a Guest Player Custom Transfer Form and Add the Guest Player to the Event Roster

Coordinate with the other club’s registrar early. You cannot upload the form at check-in until both sides have approved, and a registrar who is slow to act or traveling on tournament weekend stalls the whole request.

Uploading at Tournament Check-In

Most FYSA-sanctioned tournaments use GotSport’s online check-in. With the approved PDF in hand, open the tournament’s check-in portal, click Choose File, and attach the document. Only one file uploads per slot, so merge multiple documents into a single PDF first.4GotSport Support. How Do I Upload Documents for Online Check In

Each event sets its own check-in deadline, so look at the specific tournament’s rules. For events that still require physical check-in, the team manager brings the printed guest player PDF along with the official team roster to tournament headquarters, where staff verify the pass and confirm the player appears on the event roster.

Guest player caps also vary. Some events limit a team to five guests; others set different numbers. Exceeding the cap means the player gets scratched at check-in no matter how clean the form itself is.

Events That Don’t Allow Guest Players

Rule 209.1 conditions guest play on the specific competition’s rules allowing it, and some high-profile events do not.1Florida Youth Soccer Association. FYSA Rules March 2026 – Rule 209 The Florida State Cup prohibits secondary and guest players on State Cup rosters.5Florida Youth Soccer Association. 2025-2026 US Youth Soccer Florida State Cup Rules and Regulations The Florida State Premier League allows only players rostered as Primary or Primary 1 to an FSPL team; secondary and guest players are explicitly banned.6Florida Youth Soccer Association. Florida State Premier League Rules 2025-2026

Check the competition’s rules before you file. Trying to slip a guest player onto a restricted roster carries the same penalties as playing without authorization.

Players Registered Outside Florida

The standard guest player form does not cover a player registered with another state’s youth soccer association. That situation requires an FYSA Interstate Permission form, with written approval from both state associations.7Florida Youth Soccer Association. FYSA Interstate Permission

The player completes the player information section and sends the form to FYSA by mail, fax, or email. FYSA fills in its portion and forwards it to the accepting state association, which completes the final section. Because the form travels between two state offices, start it well ahead of the tournament.7Florida Youth Soccer Association. FYSA Interstate Permission

If You Play Without an Approved Form

Fielding a guest player without a completed and approved form can trigger charges of playing an ineligible player and roster falsification.1Florida Youth Soccer Association. FYSA Rules March 2026 – Rule 209 The practical fallout usually includes forfeiture of any game the ineligible player appeared in and potential fines against the affiliate, and the infraction stays on both the player’s and the club’s record.8Florida Youth Soccer Association.
