How to Complete and Submit the Nexplanon Specialty Pharmacy Order Form

The Nexplanon specialty pharmacy order form is a one-page enrollment PDF that routes an implant order through a patient’s pharmacy benefit. Download the editable version from Organon’s Customer Support Center for Nexplanon (CSCN) at organon-cscn.com, complete the patient, prescriber, insurance, and diagnosis fields, and fax it to 844-232-2618.1Organon CSCN. Nexplanon CSCN Enrollment Form If your EMR supports e-prescribing, you can skip the paper form and send the prescription electronically to CSCN instead. Either path starts a benefits investigation, hands the prescription to the specialty pharmacy in the patient’s network, and arranges shipment of the device to your office.

When This Form Applies

Use this form only when Nexplanon is covered under the patient’s pharmacy benefit. Most Nexplanon orders go through the medical benefit instead: the office stocks the device through a specialty distributor, inserts it, and bills the insurer afterward (buy-and-bill). Under a pharmacy benefit, the device is dispensed separately by a specialty pharmacy and shipped to you, and the insertion procedure is billed separately on the medical side.2Organon CSCN. If you don’t know which pathway a given patient falls under, call CSCN at 844-639-4321 and ask for a benefit investigation before you fill anything out.3Nexplanon Customer Support Center. Nexplanon Customer Support Center

What Goes on the Form

The form has three sections. Check each field against the patient’s intake paperwork and insurance card before you send it, because a mismatch is the usual reason the pharmacy calls back.

Patient and Prescriber

Enter the patient’s full legal name, date of birth, address, and phone number exactly as they appear on the insurance card. The prescriber block needs an NPI. You do not need a DEA number: etonogestrel is not a scheduled controlled substance, and the specialty pharmacy does not use that field for this device.4DailyMed. Insurance

Copy the group number, member ID, and pharmacy benefit manager (PBM) information straight from the card. If the patient has both primary and secondary insurance, list both. The PBM entry tells the specialty pharmacy where to route the claim, and leaving it blank or mistyped is a common cause of callbacks and delay.

Diagnosis and Prescription

Use ICD-10-CM Z30.017 for a new insertion, which codes the encounter for initial prescription of an implantable subdermal contraceptive.5ICD10Data.com. ICD-10-CM Diagnosis Code Z30.017 Watch out for Z30.432, which looks similar but codes for removal of an intrauterine device, a different procedure on a different product. That mismatch will trigger a denial. A signed prescription has to accompany the form and must follow the pharmacy law of the state where you practice.

E-Prescribing Instead

If your EMR supports e-prescribing, you can skip the paper form. Pick “CSCN” as the pharmacy in your system and send the Nexplanon prescription electronically.6Organon CSCN. How to ePrescribe The CSCN pharmacy profile uses NCPDP/NABP 5939484 and NPI 1407469471.

CSCN processes the e-prescription the same way as a faxed form: it verifies pharmacy coverage and transfers the prescription to whichever specialty pharmacy is in the patient’s network. Your office receives a fax confirming receipt and a patient ID for tracking. The patient also receives an automated text from 844-620-5904 asking them to submit their insurance information. Tell the patient to expect it so they don’t dismiss it as spam.6Organon CSCN. How to ePrescribe

Submitting the Paper Form

Fax the completed form to 844-232-2618.1Organon CSCN. Nexplanon CSCN Enrollment Form The line feeds directly into CSCN’s digital intake. Save the fax confirmation page in the patient’s chart; it proves the order was initiated if an insurance dispute comes up later. If you submit through a prescriber portal, keep a screenshot or the digital receipt with the timestamp and reference number.

Don’t mail the form. Benefits verification depends on active insurance eligibility, and postal delays can push the order past a coverage window. Fax and e-prescribe both keep the turnaround tight.

After You Submit

CSCN starts a benefits verification as soon as the form arrives. That step confirms formulary status, checks for prior authorization requirements, and determines the patient’s out-of-pocket cost. If the insurer asks for clinical justification, the specialty pharmacy will contact your office for chart notes or other supporting documentation.

Under the Affordable Care Act, most plans must cover at least one option in each of the 18 FDA-approved contraceptive categories without cost-sharing, and the implant is one of those categories.7Office of the Law Revision Counsel. 42 U.S. Code 300gg-13 – Coverage of Preventive Health Services Most patients on ACA-compliant commercial plans pay nothing for the device itself, though some may still owe a procedure fee. The list price for one rod is $1,275.36 as of January 2026, which mainly matters for uninsured patients or non-ACA-compliant plans.8Nexplanon. Nexplanon Cost – Pricing and Coverage

Once financial clearance is complete and any copayment collected from the patient, the specialty pharmacy ships Nexplanon directly to your office by expedited courier. You get a notification with a tracking number and estimated arrival so you can schedule the insertion.

If the Device Arrives but Isn’t Used

If a patient cancels or no-shows after Nexplanon has already shipped, you can’t send it back through normal pharmacy return channels. Organon runs an Abandoned Unit Program for exactly this situation.10Texas Vendor Drug Program. Organon Abandoned Unit Program for Nexplanon To qualify, the device must be unused and in its original sealed packaging with the prescription label still attached, and your office must have attempted to reach the patient at least twice to reschedule. The return window opens 120 days after the dispense date and closes at 180 days. Return the unit to the manufacturer’s third-party return processor inside that window. The program only covers units obtained through a specialty pharmacy, not buy-and-bill stock.

REMS Certification Starting August 2026

Beginning August 23, 2026, providers will need REMS certification to place Nexplanon orders, including buy-and-bill orders.2Organon CSCN. Cost and Insurance Coverage This sits on top of the existing clinical training requirement. If your office orders Nexplanon regularly, verify your REMS status before that date so your ordering doesn’t get interrupted. CSCN takes questions on certification at 844-639-4321.
