To clear the Northeastern University immunization form, fill out the University Health Report with the dates of your required vaccines, complete the Massachusetts TB Risk Assessment, have your healthcare provider sign it, and upload the signed document to Sentry MD before your term’s deadline.1Northeastern University. Vaccination and Health Requirements Miss the date and a registration hold goes on your record, blocking class enrollment until your paperwork is reviewed and approved.2Northeastern University. Gather Your Records First
Before opening the form, pull together what the form will ask for. You will need official immunization records from your pediatrician, family doctor, or previous school showing the exact dose dates. Vaccinations from outside the United States should be translated, with dates written month/day/year.
If you cannot find your records, ask your doctor for an antibody titer. A titer is a blood test that confirms existing immunity to measles, mumps, rubella, varicella, and Hepatitis B without hunting down old paperwork. At Northeastern’s own health center, titers run $25 each for students on the Northeastern University Student Health Plan, and the vaccines themselves are covered at no cost under that plan.3Northeastern Health and Wellness. Have your Northeastern ID number ready. Sentry MD uses it to match your submission to your student record.
What the Form Requires
Massachusetts law requires every student at a postsecondary institution to present a physician’s certificate of immunization against the diseases specified by the state Department of Public Health.5General Court of Massachusetts. Massachusetts General Laws Part I, Title XII, Chapter 76 For Northeastern students, those specifications translate to:1Northeastern University. Vaccination and Health Requirements
- MMR (measles, mumps, rubella): two doses, both after the first birthday.
- Tdap (tetanus, diphtheria, pertussis): one dose within the last ten years.
- Hepatitis B: a complete three-dose series.
- Varicella (chickenpox): two doses, or a physician-certified history of the disease.
- Meningococcal (MenACWY): one dose on or after the 16th birthday, for students age 21 or younger living in on-campus housing.
Health science students with direct patient contact may face additional immunization requirements under Massachusetts General Laws chapter 76, section 15C, which lets the Department of Public Health specify extra vaccines for that population.6General Court of Massachusetts. Massachusetts General Laws Part I, Title XII, Chapter 76 – Section 15C
Tuberculosis Screening
Every student completes a TB screen, not just international students. The Massachusetts Tuberculosis Risk Assessment on the form asks three risk-factor questions. If you check any of them, you need a TB blood test or TB skin test to confirm whether you carry the infection, and the result goes on the form with the rest of your documentation.1Northeastern University. Vaccination and Health Requirements Try to get tested before you arrive on campus so the results are ready to upload. Answering the risk questions inaccurately, or falsifying any of them, can trigger a hold the same way a missing vaccine does.
Filling Out and Uploading the Form
All incoming students receive the University Health Report by email from Sentry MD. Check your spam folder if you do not see it. A blank copy is also posted on the Northeastern Health and Wellness website, and you can reach the Sentry MD platform through the Student Hub at me.northeastern.edu under Health and Wellness.1Northeastern University. Vaccination and Health Requirements2Northeastern University. Immunization Requirements
Enter the vaccination dates from your records and complete the TB Risk Assessment section. Then bring the form to your healthcare provider. Massachusetts requires the health form and immunization records to be signed by your provider before submission, and an unsigned form will be rejected.1Northeastern University. Vaccination and Health Requirements Once signed, upload the document to Sentry MD. Make sure the scan is legible. A blurry photo of a crumpled form is the fastest way to get sent back.

Deadlines
Northeastern ties the submission deadline to your term:1Northeastern University. Vaccination and Health Requirements
- Fall 2026 undergraduates: July 15, 2026.
- Spring 2027 undergraduates: December 15, 2026.
- Graduate students: no later than one month before entering the university.
Past semesters have not offered extensions. If your provider’s office is slow to return signed paperwork, start the process well before the cutoff.
Medical and Religious Exemptions
Massachusetts law allows students to bypass vaccination requirements on medical or religious grounds.5General Court of Massachusetts. Massachusetts General Laws Part I, Title XII, Chapter 76 Northeastern accepts both, but written documentation must be uploaded to Sentry MD by the same deadline that applies to everyone else.1Northeastern University. Vaccination and Health Requirements
- Medical exemption: a letter from your healthcare provider stating your diagnosis and explaining why you cannot receive the vaccine.
- Religious exemption: a signed, dated letter you write yourself explaining the sincerely held religious belief that prevents you from being vaccinated.
There is no state-issued exemption form; each school manages its own process.7Mass.gov. Massachusetts Law About Vaccination and Immunization Submitting an exemption does not automatically clear a hold. The university reviews the documentation before updating your compliance status, and students granted exemptions may face additional health and safety protocols, such as extra testing, depending on campus conditions at the time.8Northeastern Global News. If You Miss the Deadline or Arrive Incomplete
The moment the deadline passes without a compliant submission, a registration hold goes on your student record and blocks you from enrolling in classes.2Northeastern University. Immunization Requirements The university does not list separate monetary fines, but losing the ability to register can delay your start date and disrupt financial aid. The hold stays until your documentation is reviewed and approved.3Northeastern Health and Wellness. Registration Holds
If you arrive with an incomplete series or a non-compliant titer, the campus health center can catch you up. For students on the Northeastern University Student Health Plan, vaccines for Hepatitis B, MMR, and varicella and the Quantiferon Gold TB blood test are available at no out-of-pocket cost; individual titer blood draws are $25 each.4University Health and Counseling Services. Vaccines and Labs Students not on the university plan should expect to pay more at a retail pharmacy or outside clinic. Getting any missing doses through the campus health center after arrival is often the fastest way to clear a hold, since the health office can update your compliance status internally without waiting on uploaded paperwork from an outside provider.
