Already have a recent sports physical from another provider? GCU accepts it as long as the exam happened within the past twelve months, and you don’t need to redo it on the GCU-specific form.1Grand Canyon University. Want to Join a Club Sports Team at GCU? Here’s How

Who Can Complete and Sign the Form

A licensed medical professional has to fill out and sign the clinical section: a physician (MD or DO), a nurse practitioner, or a physician assistant. The provider’s name, signature, and the date of the exam all need to be clearly legible. If any of those are hard to read, the athletic training staff can reject the submission and send you back for a corrected version.

If you are under 18, the form includes parental consent sections that need a parent or guardian signature. Arizona law does let certain minors (emancipated minors and those who are legally married) consent to their own medical care, but the GCU form still requires the parental sections to be completed for underage athletes.

Uploading the Form to Healthy Roster

Healthy Roster is the platform GCU uses to manage all club sports medical clearance, and it’s where the completed physical goes.4Grand Canyon University Club Sports. GCU Club Sports 2025-26 – What You Need to Know Scan the signed form and submit it through your profile. A phone or tablet scan feature produces cleaner results than a plain photo and keeps the text sharp enough for athletic trainers to read. If the physical runs more than one page, use the multi-page scan option so it uploads as a single document. Check that the provider’s name, signature, and signature date are all visible before you hit submit.

The physical is one of several items you have to finish in Healthy Roster:

  • Upload a clear photo of the front and back of your primary insurance card.2Grand Canyon University Club Sports. Club Sports Medicine
  • Complete all required waivers and consent forms electronically inside the platform.
  • Finish the general Healthy Roster onboarding, including your medical history and personal information.

Leave any task unfinished and your profile stays in pending status, which blocks clearance.

Baseline Concussion Testing for Contact Sports

If your sport is on the contact or high-impact list, you also need a baseline concussion test. The athletic training staff and coaches organize and administer it, so you don’t schedule it yourself. Incoming athletes in contact sports are typically tested within the first week of school.1Grand Canyon University. Want to Join a Club Sports Team at GCU? Here’s How

Baseline testing is required for men’s and women’s ice hockey, lacrosse, rugby, club soccer, club basketball, club volleyball, and water polo; combat sports (MMA, wrestling, boxing); and stunt, gymnastics, roller, and Canyon Motorsports.2Grand Canyon University Club Sports. Club Sports Medicine If your sport isn’t on that list, you don’t need the baseline test to clear.

Review, Rejection, and Timing

Once your documents are uploaded, the club sports athletic training staff reviews each one manually. Approved items update your Healthy Roster profile to cleared status. You can’t practice or compete until every required item shows as approved, not simply submitted.2Grand Canyon University Club Sports. Club Sports Medicine

If something is missing or unreadable, the trainers reject the document and send a Healthy Roster notification explaining the fix. Common reasons: an illegible physician signature, a missing exam date, or an incomplete medical history section. Resubmit the corrected version through the same upload process, and keep a digital backup of your signed form so you can turn it around fast.

Don’t wait until the week before your first game. Athletes in contact sports should ask their coach early for the scheduled athletic clearance date and have everything uploaded well before it.1Grand Canyon University.
