How to Fill Out and Sign the BCaBA Supervision Meeting Form

The BCaBA Supervision Meeting Form is the monthly record that you and your supervisor sign to confirm supervision happened, that it met the BACB’s standards, and that the right details were captured for each contact. Download it from the BACB’s Supervision, Assessment, Training, and Oversight page, fill in your service hours and each supervision contact’s required fields, and have both parties sign by the last day of the month after the supervision occurred.1Behavior Analyst Certification Board. Documenting Fieldwork: Helpful Answers to Your FAQs You do not submit the form to the BACB unless asked. You and your supervisor each keep a signed copy for at least seven years in case of an audit.

Where to Download the Form

The BACB hosts the form on its Supervision, Assessment, Training, and Oversight page at bacb.com, under “Documents and Resources,” labeled “BCaBA Supervision Meeting Form.”2Behavior Analyst Certification Board. Supervision, Assessment, Training, and Oversight You can complete it digitally or print it and fill it in by hand. Open the current BCaBA Handbook alongside it the first time you use it, since the handbook defines what your documentation system has to contain and how supervision hours are calculated.3Behavior Analyst Certification Board. What You Record Each Month

The form has two kinds of entries: your service delivery and your supervision contacts.3Behavior Analyst Certification Board. Board Certified Assistant Behavior Analyst Handbook

Service Delivery Hours

For every day you deliver behavior-analytic services, enter the date and the hours worked. These daily totals drive the supervision percentage calculation at the end of the month. Log them as you go. Reconstructing hours from memory at month’s end is where rounding errors creep in.

Supervision Contacts

For each supervision contact, record five things:3Behavior Analyst Certification Board. Board Certified Assistant Behavior Analyst Handbook

  • Date of the contact.
  • Time (start time or duration).
  • Format: in person or online (for example, live video conference).
  • Supervision type: individual (one-on-one) or group.
  • Observation date: the date your supervisor observed you providing services to a client.

The observation date is the field people miss. Your supervisor has to observe you working with a client during each supervisory period, and that date belongs on the form. In-person observation is preferred, but asynchronous formats like recorded video and synchronous options like live video conferencing are allowed.

Minimums the Form Has to Show You Met

Before you sign, check the entries against the monthly standards. If the numbers do not meet the minimums, the hours for that month do not count.

At Least 5% Supervised

At least 5% of your monthly service delivery hours must be supervised.4Behavior Analyst Certification Board. 2027 BCaBA Requirements Divide supervised hours by total service hours. Eighty service hours in a month means at least four supervised hours.

At Least One Contact Per Calendar Month

You need at least one real-time supervision contact, individually or in a group, every calendar month.3Behavior Analyst Certification Board. Board Certified Assistant Behavior Analyst Handbook A month with no contact is a gap you will have to self-report.

Group Supervision Capped at 50%

Group supervision (two to ten supervisees with at least one supervisor) cannot exceed 50% of your supervised hours in any month. At least half must be one-on-one.5Behavior Analyst Certification Board. Board Certified Behavior Analyst Handbook Six supervised hours with four of them in a group puts you over the cap, and you need to add individual time before the month closes.

A Client Observation Each Period

At least one supervisor must observe you working with a client during each supervisory period. Under current standards the period is monthly. Record the observation date on the form for the month it happened.

How to Sign It

Both you and your supervisor sign the completed form by the last day of the month following the supervision month. Supervision in March, signatures by April 30.1Behavior Analyst Certification Board. Documenting Fieldwork: Helpful Answers to Your FAQs There is no grace period after that date.

Several signature types are accepted. A digitally signed document through a third-party service like Adobe Sign or DocuSign is preferred because it authenticates the signer. Any electronic signature is acceptable, including a pasted image of a handwritten signature, and you can also sign by hand and scan the result. On a handwritten version, any correction, white-out, or strike-through has to be initialed by both parties.6Behavior Analyst Certification Board. Terms of Use

Who Can Sign as Your Supervisor

The signature only counts if your supervisor qualifies. They must be an active BCBA with no current BACB disciplinary sanctions, and they must be current on the BACB’s ongoing supervision continuing education requirement.4Behavior Analyst Certification Board. 2027 BCaBA Requirements If you do not have a qualified supervisor on record with the BACB, your certification is considered inactive, and during that period you cannot practice, use the BCaBA credential, supervise RBTs, provide RBT training, or complete RBT competency assessments. Replacing a departing supervisor quickly is what keeps your credential active.

Keeping Copies and Audits

You and your supervisor each keep a signed copy for at least seven years from the date of supervision.1Behavior Analyst Certification Board. Documenting Fieldwork: Helpful Answers to Your FAQs A cloud folder with a backup is more durable than a single hard drive or a paper file.

The BACB can audit your documentation at any time if it has reason to question accuracy, which may include requests for additional documentation or other methods of confirming the records are authentic.7Behavior Analyst Certification Board. Code-Enforcement Procedures Missing a documentation request deadline can lead to a summary suspension of your certification, during which you cannot practice, supervise RBTs, or represent yourself as an active certificant. Continued nonresponse after suspension can lead to revocation.

If You Missed a Requirement

If you came in under 5%, missed a monthly contact, or exceeded the group cap, self-report the shortfall. Go to the BACB’s Self-Reporting page and complete the Ethics Self-Reporting Form, choosing “Did Not Meet RBT/BCaBA Supervision Requirements” under “Type of Self-Report.”8Behavior Analyst Certification Board. Self-Reporting You will also download, complete, and upload a Self-Reporting Statement confirming the report is truthful and complete and that you followed the BACB’s formatting and content requirements. Reporting a gap yourself tends to carry less severe consequences than one the board finds during an audit.

2027 Changes to Watch

Several updates take effect in 2027 and will change how the form is completed:4Behavior Analyst Certification Board. 2027 BCaBA Requirements

  • The 50% cap on group supervision is removed.
  • Client observation frequency drops from once per month to once per quarter.
  • The separate supervision percentage for BCaBAs in their first 1,000 hours after certification is eliminated.

Until those changes are in effect, the current rules in the BCaBA Handbook govern every form you sign. When the 2027 edition of the handbook is released, read it against your current workflow to see exactly which fields and calculations change.
