How to Fill Out and Submit a 311 Service Request Form

To file a 311 service request, pick a channel (phone, website, or mobile app), choose the service category that matches your problem, enter a precise location, describe the issue in plain factual terms, and save the confirmation number the system gives you. That is the whole process, and it takes a few minutes once you have the details in front of you. The sections below walk through how to fill out a 311 service request so the right crew actually shows up.

Pick Your Channel

All three channels feed into the same database, so the one you choose is a matter of convenience, with one exception noted later.

  • Phone: dial 3-1-1 from within the city. A live agent asks the questions and enters everything for you. Best if you’re unsure how to categorize the problem.1City and County of San Francisco Careers. Customer Service Agent – 311 Call Center – Office of the City Administrator (1324) – (134604)
  • Website: your city’s 311 portal (portal.311.nyc.gov in New York, 311.chicago.gov in Chicago, and similar in other cities). Open around the clock, no hold time.
  • Mobile app: free 311 apps in cities including New York, Chicago, and Los Angeles. The app uses your phone’s GPS to drop a location pin and lets you attach a photo on the spot.2NYC311. NYC311 App

Gather the Information First

Have these ready before you open the form or make the call:

  • Exact location. A street address is best. If there isn’t one, use the nearest cross streets or a specific landmark. Vague locations stall requests: in one New York case, a complaint about a broken pedestrian ramp was never investigated because the reporter didn’t give an address or specify which corner of the intersection, so the transportation department couldn’t find it.3New York Codes, Rules and Regulations. • Service category. The form presents a menu of issue types. Scroll through the options before you start typing. Some cities list over 1,500 service types, and picking the right one routes your request to the correct department the first time.4City of Los Angeles. MyLA311
  • A factual description. “Large pothole about two feet wide in the left lane, with tire-damage debris nearby” tells a crew what to bring; “road is terrible” does not. Note how long the problem has been there if you know.
  • Photos or video (optional). Most 311 systems accept attachments. They aren’t required, but a clear image removes ambiguity about what you’re reporting.5NYC311. Service Requests
  • Contact information (optional in many cities). You can file anonymously. Chicago, for example, allows anonymous submissions by phone, text, web, or app, even if you have a registered account. The trade-off: without contact info, the department can’t reach you for clarification, and you won’t get status updates by email.6City of Chicago 311. Working Through the Form

The web and app forms follow a similar flow across cities. You pick a service category from a dropdown, type a description into a text box, enter the address or drop a pin on a map, optionally attach photos, and optionally add your name and contact details. The form won’t submit until the required fields are complete, which at a minimum means a location and an issue type.

On the phone, the agent asks the same questions and types the answers in for you. Philadelphia’s 311 agents, for instance, record your name, address, phone number, and the nature of the request into the city’s customer relationship management software during the call.7City of Philadelphia. 3-1-1 Contact Center Agent

Review before you submit. A wrong digit in the address sends a crew to the wrong block. If you’re on the phone, repeat the address back to the agent before they close the call.

Save the Confirmation Number

Once you submit, the system displays a service request number. Phone callers get the number read to them.7City of Philadelphia. 3-1-1 Contact Center Agent Write it down. Behind the scenes, the system routes your request to the responsible department based on the category you picked. You don’t need to know which agency handles what.4City of Los Angeles. MyLA311 If you supplied an email, expect automated updates: New York City sends a confirmation first, then a new email each time the assigned agency acts on the request, including when a technician is dispatched, when an inspection happens, and when the request is closed.9NYC311. If the Request Gets Closed Without a Fix

Sometimes a crew inspects, can’t find the problem, and marks the request closed. Or the repair is partial. If the issue is still there, file a new service request.5NYC311. Service Requests Put your original service request number in the new description so the department sees it’s a recurring complaint. Repeated filings signal that the problem genuinely warrants attention.

When Online Beats the Phone

Pick the website or app over the phone when you’re reporting a physical hazard that could injure someone, such as a broken sidewalk, a dangerous street defect, or a damaged ramp. In cities like New York, you generally cannot sue the city for injuries caused by a dangerous street or sidewalk condition unless the city had “prior written notice” of the problem and failed to fix it within a set window. New York courts have held that a 311 phone call alone doesn’t satisfy that written notice requirement, while a report submitted through the city’s online portal likely does, because it qualifies as an electronic record under state law.10Justia Law. Sheikh v City of New York – 2016

Save the confirmation email and the service request number. If the hazard goes unfixed and someone is hurt, that written record is what matters in a liability claim. Many cities have prior written notice laws, though the specific rules differ by jurisdiction.

What Not to Report Through 311

311 is for non-emergencies. If someone’s life is in danger, a crime is in progress, or there’s an active fire, call 911.11City of Cincinnati. What Is the Difference Between 311 and 911 A rough test: if the response needs to be measured in minutes, it’s 911. If it can wait hours or days, like a dead tree, a missing street sign, or a clogged storm drain, it’s 311.
