A housing withdrawal form is the written notice you give your landlord or university housing office to formally end your lease or dorm contract. It records your intent to vacate, sets your move-out date, and starts the clock on your deposit refund. Getting the fields right and delivering the form through an accepted channel is what separates a clean exit from months of disputed charges.
Whether you use a template provided by your housing office or draft your own letter, the same basics apply: identify yourself and the unit, state when you’re leaving, attach any required proof, and submit through a method that creates a dated record.
What the Form Must Contain
Every withdrawal notice needs a core set of details. Miss one and the housing office can treat the submission as incomplete, which means the notice period doesn’t start running when you thought it did.
- Your full legal name as it appears on the lease or housing contract, plus the names of any co-tenants or co-signers.
- Your student ID for campus housing, or your lease account number for a private rental.
- The property address, including building name, floor, and unit or room number.
- Your intended move-out date. This single field determines how your final charges are prorated, whether you owe a termination penalty, and when the deposit-return countdown begins.
- The reason for withdrawal, if your contract requires one. University housing offices typically ask you to pick from a defined list — graduation, academic withdrawal, transfer, marriage, medical need, military deployment, financial hardship — because the reason affects whether a cancellation fee applies.1Minnesota North College. - A forwarding address where the housing provider should mail your deposit refund and final account statement.
- A phone number and email where you can be reached after you leave.
- Your signature and the date. Some institutions also require a housing official to sign acknowledging receipt.
Campus forms tend to be more structured than private-rental notices. Minnesota North College’s version asks students to check one approved cancellation reason and reminds them that the submission date, assignment status, and actual move-out date all affect deposit eligibility.1Minnesota North College. A Template You Can Adapt
If no form is provided, this layout covers what housing providers expect. Trim or expand it to fit your situation.
Header: Housing Withdrawal Notification
Section 1 — Resident Information
- Full legal name
- Student ID or lease account number
- Building name, unit or room number, and full address
- Phone number and email address
Section 2 — Withdrawal Details
- Intended move-out date
- Reason for withdrawal, if required by your contract
- Forwarding address for deposit refund and final statements
Section 3 — Acknowledgment
Add a short declaration along these lines: “I am providing formal notice of my intent to vacate the above unit on [date]. I understand this withdrawal is subject to the terms of my lease or housing contract, including any applicable cancellation fees or deposit forfeitures. I agree to complete all required checkout procedures and return all keys and access devices before my departure date.”
Section 4 — Signatures
- Resident signature and date
- Housing official signature, date, and received-by notation, if applicable
Read the acknowledgment carefully before signing. It usually references cancellation penalties, prorated charges, and forfeited deposits that your signature commits you to.
Supporting Documents to Attach
The form by itself may not be enough. If you want a penalty-free release, you’ll likely need backup proof.
- Military orders. Service members ending a lease under federal law must submit written notice along with a copy of their PCS orders, deployment orders, or orders to active duty of at least 90 days.3Office of the Law Revision Counsel. 50 USC 3955 – Termination of Residential or Motor Vehicle Leases
- Medical documentation. If you’re leaving for health reasons, most housing offices want a letter from a licensed healthcare provider confirming that your condition prevents you from staying in the unit. The letter doesn’t have to disclose your diagnosis.
- Proof of academic status change. Campus housing often requires an official withdrawal letter from the registrar, a graduation confirmation, or a transfer acceptance letter.
- Marriage certificate. Some college contracts list marriage as an approved cancellation reason and will ask for a copy.
Attach everything in one submission. Sending the form first and the proof later often means the office treats the request as incomplete until the full packet arrives.
How and When to Submit
The form only works if it reaches the right person through an accepted method. Delivery date is what establishes whether you met your contractual notice deadline.
Online Portals
Most university housing offices and large property management companies accept withdrawal forms through a secure portal. You upload a signed PDF or complete the digital form directly. The system usually generates a confirmation email with a submission ID and timestamp. Save it. That email is your proof of on-time delivery.
Certified Mail
For private rentals with no portal, USPS Certified Mail with Return Receipt Requested is the standard. The signed return receipt gives you a dated record that the landlord received the notice, which holds up in court if the delivery is ever disputed.4Click2Mail. Certified Mail for Property Management – Notices, Lease Letters, and Documentation As of 2026, Certified Mail costs $5.30 and a return receipt adds $4.40 for a physical green card or $2.82 for the electronic version.5USPS. Shipping Insurance and Delivery Services Under $15 for solid delivery proof is cheap insurance.
Hand Delivery and Email
Some leases allow hand delivery to the management office or a timestamped email to the property manager. If you hand-deliver, ask the office to sign and date a copy for your records. If email is permitted, send the form as a PDF attachment and ask for a reply confirming receipt. A text message or a verbal conversation does not satisfy a written-notice requirement under most leases.
Notice Period Timing
Check your lease for the required notice period before you pick a submission date. Most month-to-month agreements require at least 30 days of written notice before the intended move-out date. Fixed-term leases may have different notice windows at the end of the term. University housing contracts often tie deadlines to the academic calendar, and at some schools canceling after classes begin means you owe the full semester regardless of when you actually leave.1Minnesota North College. When You Can Cancel Without a Penalty
Breaking a housing contract early usually costs money. Two situations are the main exceptions.
Active-Duty Military
The Servicemembers Civil Relief Act lets active-duty military personnel terminate residential leases without an early-termination penalty. The protection covers leases signed before entering active duty and leases signed during service when the member later receives PCS or deployment orders of 90 days or longer. It extends to dependents on the lease and to the spouse or dependent of a service member who dies during service.3Office of the Law Revision Counsel. 50 USC 3955 – Termination of Residential or Motor Vehicle Leases
To use it, deliver written notice with a copy of your orders. The statute accepts hand delivery, private carriers like FedEx or UPS, USPS mail with return receipt requested, or electronic delivery to an address the landlord has designated.3Office of the Law Revision Counsel. 50 USC 3955 – Termination of Residential or Motor Vehicle Leases The lease ends 30 days after the next rent payment is due. If you’ve already paid for the month, you may be entitled to a prorated refund. Be cautious about any SCRA waiver a landlord asks you to sign — signing one can forfeit these protections entirely.6Military OneSource. Military Clause – Terminate Your Lease Due to Deployment or PCS
Domestic Violence, Sexual Assault, or Stalking
The Violence Against Women Act protects tenants in federally subsidized housing who are victims of domestic violence, dating violence, sexual assault, or stalking. A covered housing provider cannot evict you or end your assistance based solely on the abuse, and eligible tenants can request an emergency transfer to a safe unit or ask that the abuser be removed from the lease while the victim stays. Many states extend comparable early-termination rights to tenants in private housing, though requirements like whether you need a police report or protective order vary by state.
What You’ll Owe If You Don’t Qualify
If no legal exception applies, leaving early almost always costs something. Knowing the likely number helps you plan the exit and negotiate where you can.
Private Rentals
Early-termination fees in private rentals commonly run one to two months’ rent, though the exact figure depends on your lease. Some leases don’t include a buyout clause at all, which technically leaves you on the hook for rent through the end of the term. In most states, landlords have a legal duty to mitigate damages — they must make a reasonable effort to re-rent the unit instead of leaving it empty and billing you for every remaining month. If a replacement tenant moves in quickly, your liability shrinks to the gap period plus any re-leasing fees.
University Housing
College cancellation penalties typically follow a sliding scale tied to the start of the semester. Canceling well before move-in may cost nothing; canceling after classes begin can mean the full year’s rent. At the University of South Carolina, new students who cancel before May 1 owe nothing, the fee climbs to $2,000 by August, and students who never show up owe the full contracted amount.7University of South Carolina. Cancellations and Refunds – Housing Minnesota North College’s contract is binding for the full academic year, and a mid-year withdrawal without an approved reason can result in charges for the remaining term.1Minnesota North College. Subletting or Assignment
If your lease allows it, finding a subtenant or assigning the lease to someone else can reduce what you owe. Subletting means a new person takes over part of the remaining term while you stay on the lease as the responsible party. Assignment transfers the entire remaining lease to someone new. Either path almost always requires the landlord’s written consent. Moving ahead without it is a lease violation in most agreements and gives the landlord grounds to terminate the tenancy.
After You Submit
Filing the form is the starting gun, not the finish line. A few practical steps follow, and skipping any of them chips away at your deposit refund.
The Move-Out Inspection
Most housing providers schedule a walk-through near your move-out date. The inspector goes room by room, checking walls, floors, appliances, plumbing fixtures, windows, and doors against the condition recorded at move-in.8HUD. Appendix 5 – Move-In/Move-Out Inspection Form Damage beyond normal wear and tear — holes in walls, stained carpet, broken blinds — comes out of your deposit. Normal wear like minor scuff marks or faded paint should not.
Some states give you the right to a pre-move-out inspection, which lets you identify and fix problems before the official walkthrough. Use it if you have it. Patching a nail hole yourself costs a few dollars; having the landlord hire someone after you leave costs much more.
Keys and Access Devices
Turn in every key, electronic fob, garage remote, mailbox key, and parking permit to the management office on or before your move-out date. Unreturned items usually generate replacement charges deducted from your deposit. Document the handoff. Ask for a signed receipt listing each item, or take a dated photo of the items on the office counter.
Deposit Refund and Itemized Statement
After you leave, the housing provider must return your deposit, minus any legitimate deductions, within a window set by state law. Deadlines range from 14 to 60 days depending on the state. Campus housing follows institutional timelines; Minnesota North College processes refunds within 60 days of checkout.1Minnesota North College. Housing Withdrawal Form
Along with the refund, or in place of one if deductions exceed the deposit, you should receive an itemized statement listing each charge, the cost, and the reason. Review it carefully. If deductions look inflated or include normal wear, you can dispute them. Many states penalize landlords who miss the refund deadline or fail to provide the itemized list, sometimes by forfeiting the right to withhold any portion of the deposit at all.
The forwarding address on your withdrawal form is where the refund and statement get mailed. Double-check it before you submit. A wrong digit in a zip code can delay your money by weeks.
