How to Fill Out and Submit a Medical Withdrawal Form

To fill out a medical withdrawal form, you gather a clinical letter from a licensed provider that ties your condition to the specific semester, complete your university’s petition (through the registrar, dean of students, or student portal) with your ID, the term, the courses, and a short explanation, attach the provider’s letter and any release form, and submit everything before your school’s deadline. The sections below walk through each piece so your petition does not stall on vague documentation, missed dates, or an eligibility rule you did not know applied.

Before You Fill Out the Form, Confirm It Is the Right One

A full medical withdrawal drops every course for the semester. If you are passing a class and the medical issue hit late, your professor can assign an incomplete and give you until the end of the next term to finish. If only some courses are affected, many schools offer a selective course load reduction (sometimes called a partial medical withdrawal) that keeps you enrolled in the rest; Auburn University processes these through the same petition system as a full withdrawal.

F-1 visa holders have a separate option. A designated school official can authorize a reduced course load for medical reasons for up to 12 months with documentation, which keeps you below full-time without breaking your status.1Study in the States. Understanding Reduced Course Load for F-1 and M-1 Students An academic advisor or dean’s office can tell you which form fits your situation before you start filling anything out.

Get the Medical Documentation First

The provider’s letter is the single most important attachment. A generic note saying you are “under my care” almost always draws a denial. The reviewer needs enough clinical detail to see that your condition genuinely kept you from finishing the semester.

Who Can Write the Letter

Schools generally require the letter from the licensed provider currently treating the condition. Physicians and psychiatrists are accepted everywhere. Many universities also accept nurse practitioners, psychologists, licensed clinical social workers, and licensed counselors. The University of Alabama accepts recommendations from a “licensed medical provider (physician, nurse practitioner, or mental health professional).”2The University of Alabama. Medical Withdrawal and Return to Campus Check your own school’s list first. If it only accepts an MD, DO, or NP, a letter from your therapist alone will not clear the review.

What the Letter Needs to Say

Strong documentation covers four things:

  • The nature of the condition and how it affected your ability to attend class, study, or complete assignments. A specific diagnosis is not required if the functional impact is clear.
  • Dates of treatment, including when the condition began, when you came under the provider’s care, and any hospitalizations. These dates must overlap with the semester you are withdrawing from. A gap between the treatment timeline and the academic term is one of the most common reasons for denial.3University of Colorado Denver. Necessary Documentation to Submit for Medical Withdrawal Request
  • A clinical rationale stating that completing the semester was not medically feasible.
  • The provider’s contact information and license number so the health office can verify the letter.

Mental health documentation follows the same standards. The provider should describe how concentration problems, inability to leave home, hospitalization, or similar symptoms blocked you from functioning academically. A DSM-5 code is not usually required, but the clinical picture has to be detailed enough that a reviewer unfamiliar with you understands why withdrawal was necessary.

A Note on Privacy

Once your documentation reaches the university, it becomes part of your education records under FERPA rather than HIPAA. The U.S. Department of Health and Human Services has confirmed that student health records at postsecondary institutions, including campus clinic records, are “excluded from coverage under the HIPAA Privacy Rule” because FERPA applies instead.4U.S. Department of Health and Human Services. Does FERPA or HIPAA Apply to Records on Students at Health Clinics Your professors and classmates will not see the medical details. Many schools also ask you to sign a release so they can contact your provider directly with follow-up questions; the University of Colorado Denver uses a separate “Authorization for Release of Medical Information” form naming the Office of Case Management.3University of Colorado Denver. Necessary Documentation to Submit for Medical Withdrawal Request Have that release ready to submit with the petition.

Find the Form and Complete Every Field

The petition is usually hosted by the Office of the Registrar, the Dean of Students, or the campus health center. Some schools post a downloadable PDF; others launch the process inside the student portal. At the University of Wisconsin–Madison, students start in their Student Center by selecting a “medical” reason code and then finish with the academic dean’s office.5Office of the Registrar. Medical Withdrawal At UCF, you submit a separate Medical Withdrawal Request petition to the registrar.6University of Central Florida. Registrar’s Office – Medical Withdrawal Form

Whatever the format, the form will ask for:

  • Your university ID number
  • The academic term you are withdrawing from
  • Each course by subject, number, and section
  • A short written statement of the medical circumstances
  • Your signature (electronic or wet) and the date

Fill in every field. Blank sections create processing delays and sometimes trigger an automatic bounce-back for correction. If the portal lets you upload attachments, convert the provider’s letter to PDF so the signature and letterhead survive. Your written statement does not need to repeat the medical detail from the letter; a short paragraph identifying the condition in general terms and the dates it disrupted your coursework is enough.

Watch the Deadline, and the “Final Exam” Rule

Filing during the semester is straightforward. Once the term ends, you are asking for a retroactive withdrawal, and the windows are strict. UCF requires the petition within six months of the end of the semester.6University of Central Florida. Registrar’s Office – Medical Withdrawal Form UW-Madison allows up to one year after the term ends, but warns that for late requests “transcript adjustments may be the only possible remedy; tuition adjustments may not be possible.”7UW-Madison Policy Library. Medical Withdrawal

One eligibility rule applies almost everywhere: you cannot have already taken the final exam or received a final grade in a course and then ask to medically withdraw from it. The University of Alabama states eligibility is “limited to students who have not taken a final exam or otherwise completed coursework for a final grade.”2The University of Alabama. Medical Withdrawal and Return to Campus If you sat the final, that specific course is likely off the table even when the rest of your request is approved. If you are too incapacitated to file on time, have a family member or academic advisor contact the dean of students office to preserve your options.

Submit the Form and Track the Review

Send the completed petition and all attachments through the channel your school designates: online portal, email to the dean of students, or hand-delivery to the registrar. If you submit by email, request a confirmation reply so you have proof of the date.

A review committee then evaluates the request. Composition varies, but it usually pulls in staff from the health or counseling center plus an academic administrator. They cross-check the provider’s letter against the academic calendar to confirm the timeline. Florida State approves full medical withdrawal “only for severe illnesses or injuries of an acute emergency nature that incapacitates the student.”8Florida State University. Medical (M)/Mental Health Withdrawal (MHW) That language is typical. The committee wants evidence that you could not reasonably have continued, not that the semester was harder than expected.

Decision timing runs about two to three weeks. UT Austin notifies students within 10 business days.9University of Texas at Austin. Medical Withdrawals and Course Load Reductions UT Arlington sends notification within 14 calendar days.10The University of Texas at Arlington. Medical and Compassionate Withdrawal and Selective Course Load Reduction (SCLR) The notice typically goes to your official university email, so keep checking it even if you have left campus.

If approved, your transcript shows a neutral code in place of grades. UCF uses “WM,”6University of Central Florida. Registrar’s Office – Medical Withdrawal Form Florida State uses “WD,”11Florida State University. Withdrawal Information on Transcript and many schools simply use “W.” None factor into your GPA, and future schools and employers see the withdrawal but not the medical reason.

If the Petition Is Denied

Most schools allow at least one appeal. UT Arlington gives students 14 calendar days from the denial to submit an appeal with “new documentation or new facts previously unavailable at the time of submitting the original application,” and offers no further appeals after that.10The University of Texas at Arlington. Medical and Compassionate Withdrawal and Selective Course Load Reduction (SCLR) The University of Illinois sets a deadline and allows additional documentation with the appeal.12Office of the Dean of Students, Illinois. Medical Withdrawal

Denials usually come down to vague documentation, dates that did not align with the semester, or a letter that failed to explain why finishing was not possible. Ask your provider to write a supplemental letter that addresses the specific gap the committee flagged, and attach it to the appeal.

Know the Financial Consequences Before You File

Tuition refunds and financial aid adjustments work independently of each other, and both can produce a bill you were not expecting.

Most schools use a sliding refund scale: close to 100 percent in the first weeks of the semester, dropping to as little as 30 percent by the final weeks. UW-Madison’s standard schedule for a 15-week term runs from 100 percent in weeks one and two down to 30 percent from week thirteen onward.7UW-Madison Policy Library. Medical Withdrawal Some schools apply a more generous refund schedule to medical withdrawals specifically, so ask the bursar’s office about the medical schedule, not just the general chart.

If you received federal financial aid (Pell Grants, Direct Loans, FSEOG), the school must perform a Return of Title IV Funds calculation. The percentage of aid you keep equals the percentage of the payment period you completed. Past the 60 percent point, you have earned all of your federal aid for the term.13Federal Student Aid. General Requirements for Withdrawals and the Return of Title IV Funds The school returns its share first, and you are responsible for any unearned grants or loans. Unearned grant funds in particular can create an immediate balance due.

Withdrawn courses also count as attempted-but-not-completed credits for Satisfactory Academic Progress, which federal rules require schools to measure.14Federal Student Aid Knowledge Center. Satisfactory Academic Progress If a medical withdrawal pushes your completion rate below the school’s threshold, file a SAP appeal before your next registration window so the hold clears in time.

If the withdrawal drops you below half-time enrollment, your six-month federal loan grace period starts the day after you leave. Returning to at least half-time before it runs out pauses and resets it, so the grace period is not used up by shorter gaps.15Federal Student Aid. Grace Periods, Deferment, and Forbearance in Detail If you do not expect to return within six months, contact your loan servicer about deferment before the clock runs down.

Housing and meal plans are separate contracts. An approved medical withdrawal does not cancel them automatically. Wichita State requires residents to complete an online contract cancellation form through the housing portal even after notifying the registrar.16Wichita State University. Contract Cancellation Expect a prorated charge and possibly an early termination fee. Attach a copy of your approved medical withdrawal to any housing or dining cancellation appeal.

Extra Steps for F-1 Students and Veterans

F-1 Visa Holders

A full medical withdrawal breaks the full-time enrollment condition of F-1 status. Talk to your designated school official before filing. If a reduced course load with medical documentation can keep you in at least one course, that is almost always better; F-1 students can carry a reduced load for medical reasons for up to 12 months.1Study in the States. Understanding Reduced Course Load for F-1 and M-1 Students If a full withdrawal is unavoidable, your DSO will update your SEVIS record, and you may need to return home and apply for reinstatement. USCIS may reinstate F-1 status when the violation resulted from circumstances beyond the student’s control and the application is filed within five months of falling out of status.17USCIS. Chapter 8 – Change of Status, Extension of Stay, and Length of Stay

Veterans on GI Bill Benefits

A withdrawal while using Post-9/11 GI Bill or other VA education benefits can create a debt: the VA may seek repayment of housing allowance, and your school may need to return tuition the VA paid on your behalf. A medical condition qualifies as a “mitigating circumstance,” and if the VA accepts it, you will not owe the full amount. Report the circumstances to your School Certifying Official or directly to the VA as soon as you file the withdrawal. Without an explanation, you will receive a letter requesting one, and if the VA does not get it, you owe the full debt.18U.S. Department of Veterans Affairs. How Your Reason For Withdrawing From a Class Affects Your VA Debt

One Last Thing to Plan For

Coming back is a separate process with its own form. Most schools require medical clearance before you can re-enroll; the University of Alabama requires documentation from a provider “who is knowledgeable of the student’s full health history and has treated the student since the effective date of Medical Withdrawal.”2The University of Alabama. Medical Withdrawal and Return to Campus Contact the dean of students office at least two months before you plan to return so a slow clearance review does not cost you a registration window.
