How to Fill Out and Submit a TWS Warranty Cancellation Form

To cancel a Total Warranty Services vehicle service contract, download the TWS warranty cancellation form, complete your contract and odometer details, sign it with a witness, and mail it to PO Box 810187, Boca Raton, FL 33481.1Total Warranty Services. Cancellation Request Form TWS then calculates a pro-rata refund using the lower of your unused mileage percentage or your unused time percentage, multiplied by what you paid, minus the cancellation fee listed in your original contract.

Where To Get the Form

The form is a one-page PDF on the Total Warranty Services website.1Total Warranty Services. Cancellation Request Form You can also request a copy by calling TWS at (800) 870-6856 Monday through Friday, 8 AM to 6 PM, or by emailing info@totalwarrantyservices.com.2Total Warranty Services. Contact The finance office at the dealership that sold you the contract usually has copies as well. Going directly to TWS is the more reliable path if the dealership has closed or won’t help.

Pull your original service contract before you start. The form itself points you to the “Your Right to Cancel” section on the first page of that contract, which spells out your cancellation fee and any conditions.1Total Warranty Services. Cancellation Request Form You will also need the contract to fill in term length, mileage limit, and the price you paid.

What To Gather First

Have these items in front of you before you start writing:

  • Your service contract, including the contract number, contract date, total months and miles in the term, and the customer charge you paid.
  • Your current odometer reading. The form asks for the mileage at cancellation and the mileage on the original issue date, then uses the difference as miles driven.
  • The 17-character VIN from the lower-left corner of the dashboard or the sticker inside the driver-side door jamb.
  • The dealer name and dealer number from your original contract. If the dealer number is missing, TWS can look it up by contract number.
  • Lienholder name and loan account number if the vehicle is still financed. The refund goes to the lender, not to you, while a balance is owed.3EFG Companies. Vehicle Service Contract Cancellation Request Form
  • A lien release or payoff letter if the loan is paid off. That document shows TWS the refund should come to you directly.3EFG Companies. Vehicle Service Contract Cancellation Request Form

Completing the Three Sections

The form is divided into three sections.1Total Warranty Services. Cancellation Request Form

Section I is contract and vehicle information. Enter the contract number, contract period in total months and miles, vehicle year, VIN, the customer charge you paid, and both odometer readings. Subtract the issue-date reading from the cancellation reading to fill in “Miles Driven.” Write the cancellation date and the original contract date, then calculate elapsed months. Check the reason for termination: “Repossession” or “Other.”

Section II is the refund math, covered below.

Section III is the signature block. It requires your printed name, your signature, the date, and a witness signature. An unsigned form, or one without a witness, will not be processed. Fill in the dealer name and number here even if you are submitting on your own; TWS uses it to identify which dealer sold the plan.

How the Refund Is Calculated

TWS runs two separate pro-rata calculations and applies whichever produces the smaller percentage.1Total Warranty Services. Cancellation Request Form

Mileage Remaining

Subtract the miles you have driven from the total mileage in your contract, then divide that result by the total contract mileage. If your contract covers 60,000 miles and you have driven 21,000, the unused mileage is 39,000 ÷ 60,000 = 65%.1Total Warranty Services. Cancellation Request Form GAP contracts skip this step because mileage does not factor into GAP coverage.

Time Remaining

Subtract the months that have passed from the total months in your contract term, then divide by the total months. On a 48-month contract with 12 months elapsed, time remaining is 36 ÷ 48 = 75%.1Total Warranty Services. Cancellation Request Form

Applying the Lower Percentage

TWS multiplies the lower of the two percentages by the customer charge you paid. In the example above, 65% from mileage would be used rather than 75% from time. On a $2,000 contract, the gross refund would be $2,000 × 0.65 = $1,300. TWS then subtracts the cancellation fee from your original agreement to arrive at the final amount.1Total Warranty Services. Cancellation Request Form Check the “Your Right to Cancel” section of your contract for the exact fee figure.

Free-Look Period for a Full Refund

Most vehicle service contracts include a free-look period during which you can cancel for a full refund minus any claims already paid. The length depends on your state and the specific contract. Some states set it as short as 15 days; others require 30 or 60 days from the date you received the contract. The exact window appears in your contract’s cancellation clause. Once that window closes, the pro-rata formula above takes over.

Submitting the Completed Form

Mail the completed form and any supporting documents (odometer statement, lien release if the loan is paid off) to:

Use certified mail with return receipt. The postmark date establishes your cancellation date, which feeds directly into the pro-rata math. Keep the receipt and a photocopy of everything you sent. You can also fax to (561) 537-7215 or email info@totalwarrantyservices.com for faster delivery, though certified mail gives you the strongest proof if a dispute comes up later. Processing takes roughly six to eight weeks. If a loan balance remains, the refund goes to your lender and reduces what you owe. If the loan is paid off and you have provided a payoff letter, the refund comes to you. When the refund is larger than the remaining loan balance, the surplus comes to you after the lender is paid.

After a Total Loss or Theft

If your vehicle is totaled or stolen and not recovered, you can still cancel and collect a pro-rata refund using the same form. You will need two extra documents:

  • A lien release letter from your lender, confirming the loan balance is zero once insurance proceeds have paid it off.
  • An odometer statement showing the last recorded mileage, typically documented when the vehicle was transferred to the insurance company or salvage yard.

The dealership and insurance company usually will not initiate this for you. The refund belongs to you, or to your lender if a balance remains, and you have to request it.

If Your Refund Is Delayed

Call TWS at (800) 870-6856 to confirm your form was received and ask for a status update.2Total Warranty Services. Contact Incomplete forms are the most common cause of delays: a missing witness signature, a blank dealer number, or an odometer reading that does not match your supporting documents. If TWS says the form is complete and processing is simply pending, write down the date and the name of the person you spoke with.

If eight weeks pass with no refund and no clear answer, escalate:

Your certified mail receipt, a copy of the completed form, and notes from any phone calls with TWS are what a regulator needs to act on your behalf.
