How to Fill Out and Submit a Zakat Application Form

A zakat application form is the document you complete to request financial assistance from an Islamic charity or mosque’s zakat fund, and filling it out well comes down to three things: confirming you fall within one of the recognized recipient categories, gathering accurate documentation of your income, debts, and assets, and answering every section honestly and specifically. Each organization uses its own form, but the questions overlap heavily, so the process below applies whether you’re applying through a local mosque, a regional zakat foundation, or a national charity.

Where to Get the Form

There is no single universal application. The most common sources are:

  • Your local mosque or Islamic center. Most keep paper forms at the front office, and some post downloadable PDFs on their websites.1Islamic Foundation. IF Zakat Application Form Template
  • National Islamic charities such as ICNA Relief, Zakat Foundation of America, and the Islamic-American Zakat Foundation, which accept applications by mail or online. ICNA Relief can be reached at (866) 354-0102 for application information.
  • Regional zakat foundations that serve multiple mosques in a metropolitan area. If your mosque doesn’t run its own fund, ask the imam for a referral.

You can apply to more than one organization, but most forms ask whether you’re already receiving or have applied for assistance elsewhere. Answer honestly. Committees share information with each other, and failing to disclose it is a common reason applications are rejected.2Islamic-American Zakat Foundation. Zakat Application Form

Confirm You Qualify Before You Apply

Zakat eligibility comes from the Quran (Surah At-Tawbah, 9:60), which names eight categories of people entitled to receive it:3Zakat Foundation of America. The Eight Kinds of People Who Receive Zakat

  • The poor (al-fuqara), who have little to nothing and struggle for daily meals or shelter.
  • The needy (al-masakin), who earn some income but not enough to cover essentials like rent, food, or healthcare.4Human Concern International. Recipients of Zakat – Who is Eligible
  • Zakat administrators who collect, manage, and distribute the funds.
  • Those whose hearts are to be reconciled, including people new to Islam or in particular hardship related to their faith.
  • Those in bondage, historically freeing enslaved persons and today sometimes interpreted as helping those trapped in exploitative labor.
  • The debt-ridden (al-gharimin), overwhelmed by legitimate debts such as medical bills, family emergencies, or student loans, where repayment would leave them destitute.4Human Concern International. Recipients of Zakat – Who is Eligible
  • In the cause of God, meaning efforts that promote Islamic values and community welfare.
  • The wayfarer, a traveler stranded without funds, provided the travel was for a lawful purpose.

Most zakat committees focus their limited funds on the first two categories and on debtors, since those groups represent the most immediate financial need in U.S. communities. If your situation doesn’t clearly fit, the committee will likely tell you so, but it’s still worth applying when you’re in genuine hardship because interpretations vary between scholars and organizations.

The Nisab Threshold

The Nisab is the minimum amount of wealth a person must hold for a full lunar year before zakat becomes obligatory on them. It is set at the equivalent of 87.48 grams of gold or 612.36 grams of silver.5Islamic Relief. What is Nisab – Zakat If your qualifying wealth falls below the Nisab, you are not required to pay zakat and may instead be eligible to receive it. At mid-2026 precious-metal prices, the gold Nisab translates to roughly $12,700 and the silver Nisab to roughly $1,400, but these dollar figures shift with the market. Organizations typically use the silver standard when evaluating applicants because it qualifies more people for assistance.

How Debt and Assets Are Counted

Committees look at your net financial position, not your income alone. Your primary residence and a personal vehicle used for daily transportation are almost universally treated as basic necessities and excluded. What counts against you are growth assets: cash savings, investments, business inventory, rental properties, and gold or silver holdings.6ISNA. A Comprehensive Guide to Calculation, Wealth, and Charity

Long-term debts are handled carefully. The prevailing scholarly view is that only payments immediately due around the time of your application can be subtracted from your total wealth, not the full outstanding balance of a 30-year mortgage or a six-figure student loan.7Zakat Foundation of America. Can Student Loans Be Deducted from Zakat as Debt If your monthly student loan payment is $400 and it’s due this week, that $400 reduces your zakatable wealth. The remaining $50,000 balance does not. Be prepared to show what is currently due rather than your total loan balance.

Documents to Gather First

Pulling your paperwork together before you sit down with the form saves time and prevents back-and-forth with the committee. Expect to need:

  • Photo identification such as a driver’s license, state ID, or passport. Organizations that accept mailed applications typically require a photocopy attached to the form.2Islamic-American Zakat Foundation. Zakat Application Form
  • Proof of income: recent pay stubs, a tax return, or a benefits statement. Some organizations only require income documentation above a certain threshold. The Islamic Society of Greenville, for example, requires proof of income for requests over $500.8Islamic Society of Greenville. Sadaqa-Zakat Form
  • Recent bank statements showing your account balances, which help the committee verify that your cash reserves fall below the Nisab.
  • Evidence of debts or hardship: utility shut-off notices, medical bills, court-ordered debt repayment schedules, or eviction notices.
  • Household information: names, ages, and relationship of everyone living in your home. Birth certificates or guardianship documents for dependents may be requested.

If any documents are in a language other than English, some committees will ask for a translated version.

How to Fill Out Each Section

The forms vary in layout but follow a predictable pattern.

Personal Information

Expect fields for your full legal name, date of birth, address, phone number, email, and marital status. If you are married, you’ll need your spouse’s name and, in many cases, their employment and income details. The Islamic Foundation form also asks for your driver’s license or ID number.1Islamic Foundation. IF Zakat Application Form Template Enter these fields exactly as they appear on your government-issued ID to avoid problems during verification.

Financial Details

This is where committees spend the most time, so accuracy matters most here. You’ll typically report:

  • Your current employment status and employer information.
  • Total monthly household income, including wages, government benefits such as SNAP or disability payments, and any other regular assistance.
  • Monthly expenses: rent or mortgage, utilities, food, transportation, and medical costs.
  • The value of any gold, silver, or jewelry owned by the household.
  • Savings and investment account balances.
  • Whether you own or rent your home, and whether you own or lease a vehicle.

Be precise with the gold and silver figures. An error here directly affects the Nisab calculation and can lead to denial if the committee believes your precious-metal holdings push you above the threshold. If you’re unsure of current values, weigh the items and multiply by the current market price per gram.

Your Assistance Request

Most forms ask you to state the specific dollar amount you need and what it will be used for. Some also ask how long you expect to need help. Be specific. “I need $1,200 for three months of past-due electric and gas bills” is far more likely to succeed than a vague request for general financial help. Committees operate with limited funds and prefer to solve a defined problem rather than provide ongoing support.8Islamic Society of Greenville. Sadaqa-Zakat Form

References and Declaration

Many forms require two community references who are not immediate family members. These should be people who can speak to your situation, such as a local imam, a community member who knows your circumstances, or a social worker. The committee may contact them to verify your claims.1Islamic Foundation. IF Zakat Application Form Template

At the bottom of the form, you’ll sign a declaration attesting that everything is truthful. Providing false information results in automatic rejection and can disqualify you from future applications.8Islamic Society of Greenville. Sadaqa-Zakat Form Most declarations also state that you agree the committee’s decision is final and that approval depends on available funds. Read this section before signing. Some organizations include conditions such as participating in financial counseling or training programs as a requirement for receiving aid.2Islamic-American Zakat Foundation. Zakat Application Form

Submitting the Application

Submission methods depend on the organization. Local mosques often accept the form in person at the front office or by email to their zakat committee. The Islamic-American Zakat Foundation accepts applications by mail at 4323 Rosedale Avenue, Bethesda, MD 20814, or by hand delivery.2Islamic-American Zakat Foundation. Zakat Application Form Larger national organizations may have online portals for digital submission. Whichever method you use, keep a copy of the completed form and all supporting documents in case something gets lost in review.

For paper submissions, certified mail or a delivery service with tracking gives you proof the packet arrived. Attach scanned or photocopied supporting documents to the form rather than sending them separately.

After You Submit

Processing times vary. A local mosque with an active committee may review applications weekly and respond within a few days. MCC East Bay, for example, targets one to seven business days.9MCC East Bay. Zakat Larger or busier organizations typically take about two weeks, and it can stretch longer during Ramadan when application volume spikes.10South Bay Islamic Association. Zakat Application Form

During review, a committee member may contact you for a brief interview or to clarify specific financial details. They may also reach out to your references and check with other local mosques to confirm you aren’t receiving duplicate assistance.9MCC East Bay. Zakat This is standard due diligence.

The committee notifies you of its decision by letter, email, or phone. If approved, the organization may pay your bills directly to a landlord or utility company rather than handing you cash. This is common and helps the committee ensure zakat funds are used for the stated purpose. The amount approved may also be less than what you requested. Applying does not guarantee approval, and the committee has sole discretion over grant amounts.2Islamic-American Zakat Foundation. Zakat Application Form

Common Reasons Applications Are Denied

  • Incomplete or inaccurate information: missing fields, math errors in your financial summary, or inconsistencies between your stated income and your bank statements.
  • Assets above the Nisab: if your combined cash, investments, and precious metals exceed the silver Nisab, you won’t qualify regardless of monthly cash-flow problems.
  • False or misleading statements, which result in automatic rejection.8Islamic Society of Greenville. Sadaqa-Zakat Form
  • Not falling within the eight categories. Someone with a stable income and manageable debt may simply not qualify, even if money is tight.
  • Fund availability. Committees can only distribute what has been collected, and late in the fiscal year funds may already be committed.

If you are denied, ask the committee for the specific reason. Some organizations allow you to reapply once your circumstances change or after correcting the issue that caused the denial.

Will Zakat Affect Your Government Benefits

Receiving zakat generally does not jeopardize federal benefits, but the details depend on the program and how the funds are delivered. For Supplemental Security Income (SSI), the Social Security Administration excludes food or shelter provided by nonprofit agencies based on need from countable income. Money that a third party spends on your behalf for expenses other than food or shelter, such as paying a medical bill directly, is also excluded.11Social Security Administration. Understanding Supplemental Security Income SSI Income

For SNAP, federal rules exclude unanticipated or irregular income up to $30 per quarter, and certain nonrecurring charitable donations may be treated as exempt or counted as a one-time lump-sum resource rather than ongoing income. In-kind benefits, where the charity pays your landlord or utility company directly rather than giving you cash, are generally not counted as income for SNAP. If you receive government benefits and are concerned about the impact, ask the zakat committee to pay your bills directly to the provider rather than issuing a check to you. That approach is safest across most benefit programs and is the method many committees prefer anyway.
