How to Fill Out and Submit an Annex Form or Continuation Sheet

An application annex form is an extra page you attach to a primary government or financial application when the main form doesn’t give you enough space to answer fully. No single federal document is actually titled “Application Annex Form.” The specific annex you need depends on the application you’re filing: USCIS builds an “Additional Information” section into many immigration forms, the SBA uses Form 2202 (Schedule of Liabilities) as a supplement to its disaster loan application, and the U.S. Copyright Office publishes Form CON as a general continuation sheet for registrations.

Which Annex Goes With Which Application

Start by identifying the primary form, because that dictates the annex. A few of the common pairings:

  • SBA disaster loan applicants use Form 2202, Schedule of Liabilities, to list every debt in detail — creditor, original amount, current balance, maturity, payment, and how the debt is secured. The totals have to balance against the liabilities shown on the main balance sheet.1U.S. Small Business Administration. Schedule of Liabilities – SBA Form 2202
  • USCIS forms such as the I-485 (Application to Register Permanent Residence) include an “Additional Information” section and also let you attach separate sheets when any part of the form runs out of room.2U.S. Citizenship and Immigration Services. • The U.S. Copyright Office publishes Form CON for additional authors, claimants, or other overflow fields.
  • Mortgage borrowers using Freddie Mac’s Uniform Residential Loan Application have a dedicated continuation sheet for extra information.3Freddie Mac. Uniform Residential Loan Application – Continuation Sheet
  • Companies filing securities registrations under Regulation S-K attach exhibits for material contracts, compensation arrangements, and financial schedules that won’t fit the main registration statement.4eCFR. 17 CFR 229.601 – Exhibits

If the agency you’re filing with publishes its own supplemental form, use it. Only attach a plain continuation sheet when the instructions tell you to or when no dedicated form exists.

How to Label and Format the Annex

The reviewer has to be able to match every entry on your annex back to the right spot on the primary form. USCIS lays this out plainly in the I-485 instructions: type or print your name and Alien Registration Number (if any) at the top of each extra sheet, note the page number, part number, and item number your answer refers to, then sign and date each sheet.2U.S. Citizenship and Immigration Services. Form I-485, Instructions for Application to Register Permanent Residence Even when an agency’s instructions are less explicit, that same pattern works: your identifier at the top, a clear pointer to the item being answered, and your signature at the bottom.

Before you fill anything in, gather the records you’ll be drawing from. For a financial annex like Form 2202, keep your balance sheet in front of you so the liabilities on the supplemental schedule match exactly.1U.S. Small Business Administration. Schedule of Liabilities – SBA Form 2202 A mismatch between the primary form and the annex is one of the fastest ways to draw a request for additional information.

Keep the formatting on the annex consistent with the main form. Same date format, same level of dollar precision, same style for names and addresses. Consistency is more than a cosmetic choice; it tells the reviewer the file is reliable and keeps your application out of the manual-review pile.

Signing, Notarizing, and Fastening the Pages

Whether the annex itself needs a signature depends on the agency. USCIS tells you to sign and date each additional sheet.2U.S. Citizenship and Immigration Services. Form I-485, Instructions for Application to Register Permanent Residence Other agencies may only ask for a signature on the primary form. Read the instructions for your specific filing, and when the instructions are silent, signing each extra page is the safer choice.

If the filing requires notarization, federal regulations address multi-page documents. When the notarial certificate sits on a separate sheet from the main instrument, the pages have to be securely fastened together, and the notary’s seal should overlap the junction between the certificate page and the document so pages can’t be swapped later. The same principle applies to annexes: fasten all sheets together in a way that resists tampering.5eCFR. 22 CFR Part 92 – Notarial and Related Services

Notary fees vary by state. Some states cap the charge as low as $2 per signature, others allow $25 or more, and a few set no maximum. If notarization is required, call ahead so the price doesn’t surprise you.

Submitting the Annex With the Main Application

The annex goes in with the primary application. Don’t submit it separately unless the agency’s instructions specifically direct that.

How Long to Keep Your Copies

Hold onto copies of both the primary application and the annex for as long as the filing could matter. For tax-related filings, the IRS says to keep records as long as they’re needed to prove the income or deductions on a return — generally at least three years, longer when underreported income or certain losses are involved, and at least four years for employment tax records.8Internal Revenue Service. Recordkeeping

For non-tax filings such as immigration applications, business loans, or copyright registrations, keep your copies until the matter is fully resolved and any appeal window has closed. Loan-related documents are worth keeping for the life of the loan plus several years after. If a question comes up later about what you disclosed, you want to pull the exact pages you submitted.

What Mistakes and False Statements Can Cost

Errors on an annex carry the same legal weight as errors on the primary form, because the annex is part of the application. On tax-related filings, the IRS can impose a 20% accuracy-related penalty on any underpayment caused by negligence or careless disregard of the rules.9Internal Revenue Service. Accuracy-Related Penalty

Intentional falsehoods are more serious. Knowingly making a false statement or using a fraudulent document in any matter within the jurisdiction of the U.S. government is a federal crime punishable by up to five years in prison, with the maximum rising to eight years when the statement involves terrorism. That statute reaches every federal form and attachment, annexes included.10Office of the Law Revision Counsel. 18 USC 1001 – Statements or Entries Generally Treat the annex with the same care as the main form. The reviewer will.
