How to Fill Out and Submit an Equitable Withdrawal Form

To take money out of an Equitable (formerly AXA Equitable) policy, annuity, or retirement account, you need to submit an Equitable withdrawal form that matches your specific contract. Most of these forms aren’t posted publicly. You get them by signing in to the Equitable Client Portal, downloading the version tied to your contract, completing it, and sending it back by mail or fax. Equitable generally processes a valid request within five business days.1Equitable. Hardship Withdrawal Request

Finding the Form That Matches Your Contract

Equitable’s public forms page shows only a handful of downloadable documents. The withdrawal paperwork you actually need sits behind your account login. Sign in at the Equitable Client Portal, open “Manage your account” on the dashboard, and click “Forms” in the middle “View” column. The “Service Forms” page then lists every form tied to your specific contract.2Equitable. Customer Service and Support Forms

Which form you pick depends on your product and what you want to do:

  • For a partial withdrawal from a life insurance policy, you use a Policy Owner Service Request form. It pulls a dollar amount from your cash value while keeping the policy in force, subject to any contract limits and withdrawal fees.
  • For an annuity, Equitable publishes product-specific lump sum distribution forms for the Accumulator Series, Investment Edge, Retirement Cornerstone, and Structured Capital Strategies lines. The same form handles a full surrender or a one-time partial distribution.2Equitable. Customer Service and Support Forms
  • For a 403(b) hardship withdrawal, there is a separate Hardship Withdrawal Request form. You have to certify an immediate and heavy financial need and confirm no other resources are available.1Equitable. Hardship Withdrawal Request
  • To move money from an Equitable 403(b) or IRA to another carrier, you use a Direct Transfer/Contract Exchange or Direct Rollover form, not a withdrawal form.

Picking the wrong form is the quickest way to get a request bounced back. If you aren’t sure which document applies, call Equitable’s customer service line at (866) 444-6001 before you submit anything.

What You’ll Need to Fill In

The header section on every version asks for the same core information: your full legal name, Social Security number, and your contract or certificate number. That contract number is the unique identifier printed on your policy documents and shown in your online account.1Equitable. Hardship Withdrawal Request A missing or mismatched contract number is one of the most common reasons a form gets rejected, so check it against the portal before you write anything down.

Next you specify the dollar amount you want (or mark the box for a full surrender) and choose how you want to receive the money. If you want an electronic deposit, have your bank’s nine-digit routing number and your account number ready for the direct deposit section. If you leave the banking fields blank, Equitable defaults to a paper check, which takes longer to arrive.

Signatures and Medallion Guarantees

A routine withdrawal usually needs only your signature. For larger transactions or ownership changes, Equitable may ask for a notarized signature. For a transfer of securities or certain high-value annuity surrenders, the company may require a Medallion Signature Guarantee instead. A Medallion guarantee is not a notary stamp. It’s an insurance-backed certification from a participating bank or brokerage that the institution stands behind the authenticity of your signature. Most banks provide them free to existing customers; non-customers may pay up to $100. If the form or the instructions call for a Medallion guarantee, a regular notary won’t satisfy the requirement.

Tax Withholding on the Form

Every nonperiodic distribution from an annuity or retirement account triggers federal income tax withholding. The default rate is 10% of the taxable portion unless you elect a different percentage on IRS Form W-4R.3Internal Revenue Service. Form W-4R – Withholding Certificate for Nonperiodic Payments and Eligible Rollover Distributions Equitable typically builds the W-4R election directly into the withdrawal form, so you don’t submit a separate document. Fill in the withholding section with the rate you want. Skip it and Equitable will withhold 10% automatically.

The 10% default is a withholding rate, not your final tax rate. Your actual tax bill depends on your income bracket and how the withdrawal is classified. Overwithholding ties up money until you file your return; underwithholding can leave you owing in April. If you know your marginal rate, electing that rate in the W-4R section is the simplest way to stay roughly even.4Internal Revenue Service. About Form W-4R, Withholding Certificate for Nonperiodic Payments and Eligible Rollover Distributions

Withholding is separate from the 10% additional tax the IRS charges on distributions from qualified retirement accounts or modified endowment contracts taken before age 59½.5Office of the Law Revision Counsel. 26 USC 72 – Annuities; Certain Proceeds of Endowment and Life Insurance Contracts If you’re under that age, expect both to apply unless you qualify for an exception. Meeting a plan’s hardship criteria gets the money released; it does not by itself waive the 10% penalty.1Equitable. Hardship Withdrawal Request

Surrender Charges and the Free Withdrawal Allowance

Before you settle on a dollar amount, check what the form will actually deduct. Most Equitable annuity contracts let you withdraw up to 10% of your account value each year without a surrender charge.6Equitable. EQUI-VEST Variable Deferred Annuity Anything beyond that free amount is subject to a percentage-based charge on the excess, calculated by how long each contribution has been in the contract. Schedules vary by product:

  • EQUI-VEST Series 201 applies a 5% charge on contributions made within the current and five prior contract years. Charges disappear after 12 contract years.6Equitable. EQUI-VEST Variable Deferred Annuity
  • EQUI-VEST Series 202 applies a 6% charge on contributions made within the current and four prior contract years. Charges also end after 12 contract years.6Equitable. EQUI-VEST Variable Deferred Annuity

Other Equitable products use a declining schedule, for example 6% in years one and two stepping down to 3% in year six and nothing from year seven onward. Your specific schedule is in the policy document and should appear on any withdrawal form that applies a charge. If you’re inside the surrender period and your withdrawal exceeds the free amount, the charge is deducted from the proceeds before the money reaches you.

What the Withdrawal Does to Your Contract

A withdrawal is not free of consequences beyond taxes and charges. Equitable’s own FAQ states that “policy loans and/or withdrawals also reduce the cash surrender value and policy death benefit and increase the chance that a policy will lapse.”7Equitable. Common Questions About Life Insurance A smaller death benefit means less for your beneficiaries, and if cash value drops below the level needed to cover policy charges, coverage can end entirely, potentially creating a taxable event on top of the lost protection.

On the annuity side, partial withdrawals cut your account value and the benefit bases tied to any optional riders. If your annuity carries a guaranteed minimum withdrawal benefit or a death benefit rider, read the rider terms before you set the withdrawal amount. Some riders reduce the guaranteed amount proportionally; others reduce it by more than the withdrawal.

Where to Send the Completed Form

Equitable accepts completed forms through several channels:

  • Standard mail: Equitable Financial Life Insurance Company, Retirement Service Solutions, P.O. Box 1547, Secaucus, NJ 07096-1547
  • Fax: (816) 701-8040
  • Online portal: some account-specific forms can be initiated directly through the Client Portal after logging in.
  • Phone: call (866) 444-6001 for help identifying the right channel for your product.

Use fax or overnight delivery when standard mail is too slow. The P.O. Box handles regular USPS mail but won’t accept overnight packages from FedEx or UPS. If you need to overnight the form, call customer service for the physical street address. Keep a copy of everything you send, along with any fax confirmation page or tracking number.

Processing Time and How the Money Arrives

Equitable’s hardship withdrawal form states that the company has five business days to process a valid request once received.1Equitable. Hardship Withdrawal Request Standard distribution requests run on a similar timeline. Complex transactions, such as full surrenders across multiple sub-accounts or transfers to an outside carrier, can take longer.

If you elected direct deposit, the funds usually land in your bank account within a few business days after processing finishes. Paper checks add mailing time on top of that, so expect roughly a week to ten business days from approval to delivery. Direct deposit is faster and removes the risk of a lost check.

If the review team spots a problem, whether a missing signature, an incorrect contract number, or an unsigned W-4R section, they’ll send a notice explaining what needs to be corrected. The clock restarts when you return the corrected paperwork, so responding quickly matters.

Tax Reporting After the Withdrawal

Equitable issues a 1099-R for every taxable or reportable distribution in a calendar year. The form is available through the Client Portal by the end of January and is also mailed. Box 7 carries a distribution code that tells the IRS what happened: Code 1 for early distributions (under 59½), Code 7 for normal distributions (over 59½), and Code 6 for a Section 1035 tax-free exchange into another insurance or annuity product.8Equitable. 2025 Tax Year Overview

One trap catches people at tax time. If you moved money into your Equitable contract through a 1035 exchange from a previous carrier and Equitable never received your original cost basis, the company reports the entire distribution as fully taxable. Fixing that means tracking down the basis documentation from the prior carrier and sending it to Equitable to correct the reporting.8Equitable. 2025 Tax Year Overview Confirm your basis is on file before you submit the withdrawal form, not after the 1099-R arrives.
