ERES Form 102 is the nursing license verification form that your foreign licensing authority completes and mails directly to Educational Records Evaluation Service as part of your U.S. nursing credential evaluation. You don’t download it on your own. It generates automatically inside the ERES portal once you submit the online nursing application, and from there you fill in your identifying details before passing it to the government body that issued your license.1Educational Records Evaluation Service. Nursing If you never held a foreign nursing license, Form 102 doesn’t apply to you and you can leave it out of your evaluation package.
How Form 102 Gets Generated
The form doesn’t exist until you create it. You start at the ERES website by submitting a nursing application (Form 100). Once that application is complete, the system issues a reference number and automatically generates both Form 101, which goes to your nursing school for education verification, and Form 102, which goes to your licensing authority.1Educational Records Evaluation Service. Save that reference number. It ties every document in your file together, and ERES uses it to match incoming paperwork to your application. You can track your file through the ERES portal at app.eres.com.2Educational Records Evaluation Service. Foreign Credential Evaluation Services USA Don’t send anything to ERES before you have the reference number.
Your Section of the Form
Your part of Form 102 is short. You enter your full legal name as it appears on your passport or government ID, your contact information, and your ERES reference number. You also fill in the license being verified: the license number, the country or jurisdiction that issued it, and the date it was originally granted.
Check every entry against your official documents before handing the form off. If the name or license number doesn’t match what the licensing authority has on file, the staff there may not be able to locate your record, and that delay sits on top of whatever processing time they already need.
What Your Foreign Licensing Authority Completes
After your section, the form goes to the body that issued your nursing license. In most countries that’s the national nursing regulatory board or the Ministry of Health. They complete the verification portion, which covers:
- License status: whether the license is currently active, expired, lapsed, or revoked.
- Registration dates: when the license was first issued and the dates of any subsequent renewals.
- Disciplinary history: any sanctions, suspensions, or restrictions on the license, with the reasons.
The authority must sign the form and apply its official stamp or seal. ERES will not accept an unsigned or unsealed form. This is the step where applications most often stall, because some licensing bodies take weeks or months to process verification requests. Submit your request to them early and follow up on a regular schedule.
Mailing Form 102 to ERES
The completed form must be mailed directly from the licensing authority to ERES. You cannot carry it, forward it, or mail it yourself. ERES treats the chain of custody as a rejection point: a form that arrives from the applicant will not be accepted.1Educational Records Evaluation Service. Nursing
The ERES office is at 2480 Hilborn Road, Suite 106, Fairfield, CA 94534.3Commission on Teacher Credentialing. Commission Approved Foreign Transcript Evaluating Agencies Confirm the authority has the current address, since mail sent to an outdated one adds weeks. Ask them to write your ERES reference number on the outer envelope or in a cover letter so staff can match the form to your file when it arrives.
How Form 102 Affects Your Timeline
Standard nursing evaluation turnaround is three to four weeks after ERES receives the complete package: application, all documents (including Form 102), and payment.1Educational Records Evaluation Service. Nursing The clock does not start until every piece is in. If your licensing authority is slow with Form 102, nothing else moves.
If you need faster processing, ERES offers a 10-day rush service called R10 for an additional $180 on top of the base evaluation fee, which starts at $480. With R10, the report is ready in 10 to 11 business days after ERES receives the complete package.1Educational Records Evaluation Service. Nursing Rush orders cannot be paid with a regular credit card or personal check; ERES requires money order, cashier’s check, debit card, or cash in person for that service.4Educational Records Evaluation Service. Pricing and Payment Rush service shortens the ERES side of the process. It does nothing to speed up your licensing authority, so the bottleneck usually stays with them.
Where Form 102 Sits in the Full Evaluation
Form 102 is one piece of the nursing evaluation package. ERES also requires your degree certificate or diploma, your transcript or marksheet, English-language course descriptions, and Form 101 completed by your nursing school.1Educational Records Evaluation Service. Nursing Your school mails the certificate, transcript, and Form 101 directly to ERES, the same chain-of-custody rule that applies to Form 102.
Form 102 itself is in English because ERES generates it. If your licensing authority attaches supporting records in another language, those attachments need a word-for-word English translation; paraphrased or summarized translations are not accepted.1Educational Records Evaluation Service. Nursing
Once ERES has everything, it issues an evaluation report and sends it to the state board of nursing you named during your application. The state board uses that report in deciding whether you qualify to sit for the NCLEX. The ERES report is one input, not the full review. Each state board sets its own additional requirements, so contact your target state directly to confirm what else they need alongside the evaluation.
