How to Fill Out and Submit Knights of Columbus Form 100: Transfer

A Knights of Columbus Form 100 transfer moves your membership from one council to another while preserving your degree history, and the process comes down to four things: check the “Transfer In” box, fill every field, collect three signatures, and send the form to the Supreme Council office. Done cleanly the first time, it’s straightforward. Most delays trace back to a blank field, a missing signature, or the wrong transaction box.

What to Gather Before You Start

Have this information in front of you before you open the form:

  • Your membership number, from your current card or your Financial Secretary.
  • Your current council number (the form labels it “Number of Last Council”).
  • The receiving council’s number. If you’ve moved and don’t yet know which council to join, the council locator at kofc.org will find one near your new parish.
  • The dates you received your First, Second, Third, and Fourth Degrees.
  • Confirmation that your dues are paid in full. A financial hold at your current council will stop the transfer.

You also need a proposer: a current member of the receiving council who agrees to sponsor you and put their name and membership number on the form. If you don’t know anyone at the new council, contact the Grand Knight and ask to be paired.

Where to Get the Form

Ask the Grand Knight or Financial Secretary of either your current or receiving council. A fillable PDF also circulates through state council sites and the Knights of Columbus officer resources page at kofc.org. If the receiving council’s Financial Secretary uses the Officers Online portal, the whole transfer can be handled digitally through Member Management and no paper Form 100 is needed. Otherwise, use a current fillable PDF rather than a photocopied form from a council hall.

Filling Out Form 100 for a Transfer

In the Transaction section, check the box for Transfer In. The form covers several unrelated actions (New Member, Reinstatement, Readmission, Reapplication, Reactivation), and picking the wrong box routes your paperwork into a different process at the Supreme Council.

At the top of the form, enter the receiving council’s number, city, and state. Further down, enter the number, city, and state of the council you’re leaving.

Complete the personal information fields: full name, address, date of birth, marital status, phone numbers, email, occupation, and the last four digits of your tax ID (Social Security Number for U.S. members). Enter your parish name and location.

Answer the two questions near the bottom: whether you are a practical Catholic in union with the Holy See, and whether you were a former Columbian Squire. Then fill in your initiation dates for each degree you have received. Those dates carry your degree status forward, so accuracy here matters.

Read the declaration above the signature line. Signing affirms that the information is correct and that you agree to uphold the charter, constitution, and laws of the Knights of Columbus and of your new council. Date and sign.

Three Signatures Are Required

Your signature alone is not enough. The form needs three: yours, the receiving council’s Grand Knight, and the receiving council’s Financial Secretary.1Knights of Columbus. Membership Document – Form 100 A form arriving at Supreme with any signature missing will be sent back. Don’t mail it in before the two officers have signed.

Where to Send the Completed Form

Once all three signatures are on the form, it goes to the Supreme Council. You have two options:

  • Email: send the completed form to membership@kofc.org or membershiprecords@kofc.org. If you email it, you do not also need to mail the original.1Knights of Columbus. Membership Document – Form 100
  • Mail: send the original to the Supreme Council office at the address on file with your state council. Faxed copies are not accepted.1Knights of Columbus. Membership Document – Form 100

The Financial Secretary keeps a copy for the receiving council’s records and sends a copy to the council’s field agent.

What Happens at the Receiving Council

Before the Grand Knight and Financial Secretary sign, the transfer request goes through the receiving council’s admissions process. The transfer is read at a regular council meeting and members vote to accept it. Once the vote passes and the officers sign, the Financial Secretary submits the form to the Supreme Council (or processes it through Officers Online). When Supreme updates its records, you appear on the new council’s roster, drop off the old one, and your degree status, membership history, and continuity of service carry forward.

If You’re an Online or College Council Member

Online members (sometimes called eMembers) use the same Form 100 process, with an added step: you go through the local council’s admissions process, meet with council leaders, and complete the required ceremonial before the transfer is finalized. Your dues are pro-rated to the local council when you transfer in, so you won’t pay for the year twice.2Knights of Columbus. Graduating college council members follow the same path. As an affiliate member of a college council, you can transfer into any local council at any time using Form 100.4Knights of Columbus. Transitioning Graduating Members

Mistakes That Send the Form Back

Each return trip to the Supreme Council office adds weeks. The recurring problems:

  • Missing signatures. All three are required.
  • The wrong transaction box. “Reinstatement” or “New Member” sends the form down the wrong process.
  • Blank fields. The Supreme Council warns that failure to complete the form fully causes delays and may result in the form being returned to the council.1Knights of Columbus. Membership Document – Form 100
  • Outstanding dues at the old council. Pay any balance before you start.
  • Illegible handwriting. Use the fillable PDF, or print clearly. Faxed copies are not accepted.

The cleanest path is to sit down with the receiving council’s Financial Secretary and fill the form out together. That officer processes transfers regularly and can catch errors before the paperwork leaves the building.
