How to Fill Out and Submit the Amazon Prime Settlement Claim Form

The Amazon Prime settlement claim form is at www.SubscriptionMembershipSettlement.com, and it must be submitted on or before July 27, 2026. Eligible customers who did not already receive an automatic refund from Amazon in late 2025 can claim a refund of Prime subscription fees up to $51.1Federal Trade Commission. Amazon Refunds

Who Can File

You qualify if you signed up for Amazon Prime through one of the enrollment flows the FTC challenged, between June 23, 2019 and June 23, 2025. Those flows are the universal Prime decision page, the shipping selection page, single-page checkout, and the Prime Video enrollment page. You also qualify if you tried to cancel Prime through Amazon’s online cancellation process during that same window and were unable to do so.1Federal Trade Commission. Amazon Refunds

There is a usage limit that matters. You must have used no more than three Prime benefits in any 12-month period after enrolling. Prime Music and Prime Video content offered free to Prime subscribers count as benefits for this purpose.1Federal Trade Commission. Amazon Refunds Heavy users who regularly took advantage of Prime shipping, streaming, and music will likely fall outside the threshold. The rule targets people who paid for a subscription they weren’t really using.

You Don’t Need an Active Membership

Eligibility turns on when and how you enrolled, not on whether you are still a Prime member. If you canceled years ago but were originally signed up through one of the challenged flows during the eligible window, you can still file as long as you meet the benefit-use limit.

If You Already Received an Automatic Refund

Amazon sent automatic refunds to some eligible customers between November 12 and December 24, 2025. If you received one, you do not need to file a claim form. The claims process that opened in January 2026 is for eligible customers who did not get that automatic payment. If you’re not sure, check the email address on your Amazon account for messages from Amazon about the settlement sent during that period.

How to Fill Out and Submit the Form

The claim form lives at www.SubscriptionMembershipSettlement.com. The settlement administrator began contacting eligible customers by email or mail no later than January 28, 2026, and a notice you received should link you to your claim. You can also reach the administrator at admin@SubscriptionMembershipSettlement.com or 1-888-999-8094.2Amazon. Amazon Settlement

Complete every requested field, sign the form electronically, and submit it before the deadline. Have the email address tied to your Amazon account ready, since the administrator uses it to match your claim against Amazon’s records. If your notice includes a unique code or claimant ID, enter it too; verification is faster with it.

Choosing How You Get Paid

You pick the payment method on the form. The options are PayPal, Venmo, or a paper check by mail.1Federal Trade Commission. Amazon Refunds For a digital payment, you’ll need an active account with that service. Paper checks go to the mailing address you provide and must be cashed within 60 days of receipt.

How Much You’ll Get

The refund covers Prime subscription fees you paid during the relevant period, up to a maximum of $51.1Federal Trade Commission. Amazon Refunds Someone enrolled for only a few months will receive less than someone billed over a longer stretch.

The Deadline and What Comes Next

Every claim must be completed, signed, and submitted on or before July 27, 2026. Missing that date means forfeiting the refund. If you think you’re eligible and haven’t received a notice, don’t wait for one. Go to the settlement website directly and check whether you can file.

After you submit, save any confirmation number or email the system sends back. The administrator reviews claims against Amazon’s customer database to verify eligibility, and verification takes time given the volume. Once your claim is validated, payment goes out through the method you chose. Status updates come to the contact information you gave on the form.

Avoiding Scams

The only official website for this settlement is www.SubscriptionMembershipSettlement.com, and the only official contact email is admin@SubscriptionMembershipSettlement.com.1Federal Trade Commission. Amazon Refunds Ignore text messages, social media posts, or emails from other addresses asking you to file a claim or hand over financial information.

The real claim form will never ask for your full Social Security number, your bank account login, or an upfront fee. You do not pay anything to file. Any site demanding a processing fee or a payment to “unlock” your refund is fraudulent. When in doubt, go straight to the FTC’s page at ftc.gov/enforcement/refunds/amazon-refunds instead of clicking links in emails.

Filing Won’t Affect a Current Prime Membership

Submitting a claim does not cancel or suspend an active Prime account. The settlement addresses how Amazon enrolled people in the past, not your ability to keep using the service. If you want to cancel Prime, do that separately through your Amazon account settings.
