How to Fill Out and Submit the Ameren Medical Certification Form

The Ameren medical certification form is a one-page document a licensed physician or local board of health signs to confirm that someone at your service address has a medical condition that would get worse without gas or electric service. Once Ameren receives it, your account is placed on a temporary hold that blocks disconnection while you arrange payment. The rules differ between Illinois and Missouri, so the way you get the form, where you send it, and how long you’re protected depend on which state you’re billed in.

How to Get the Form

Ameren Illinois customers can download the fillable PDF directly from Ameren’s website. If you’d rather have a paper copy mailed to your service address, call 800-552-7583.1Ameren. Health, Safety and Financial Assistance Options

Ameren Missouri handles medical shutoff protection through the Critical Medical Need Program, which uses a separate intake form processed by a community action agency. Call United Way at 211 to start that enrollment rather than contacting Ameren directly.2Ameren. Ameren Missouri Critical Medical Need Pilot Program Tariff

Ameren will also accept a letter on your healthcare provider’s official letterhead in place of the standard form, as long as it contains the same required information: the patient’s name, service address, account number, the provider’s license details, and a statement about medical necessity.1Ameren. Who Qualifies and Who Can Sign

You qualify if someone living at your service address has a condition that would be significantly worsened by loss of service, or if losing power would prevent the use of life-support equipment such as an oxygen concentrator, respirator, or dialysis machine. The patient doesn’t have to be the account holder. They just have to live at the address being served.3Ameren Illinois. Ameren Medical Certification Form

In Illinois, the certification has to come from a licensed physician or a local board of health.4Illinois General Assembly. Illinois Admin Code Title 83, Section 280.160 – Medical Certification Missouri’s tariff language refers to a “certified medical professional” or “medical physician.”2Ameren. How to Fill It Out

The form has two sections. One is yours. The other belongs to your healthcare provider.

Your Section

Enter the name on your Ameren account, the patient’s name if that’s a different person, your account number, and the service address where the patient lives. Use the street address where the meter sits, not a separate mailing address. Your account number appears near the top of the first page of any recent bill.3Ameren Illinois. Finish your section before you hand the form to your doctor. Medical offices process these faster when the patient information is already in.

The Provider’s Section

Your healthcare provider signs a statement confirming that the patient at your address needs electric or gas service at all times and that losing service would create a medical emergency or make an existing one worse. The provider also fills in a phone number Ameren may use to verify the certification.3Ameren Illinois. Ameren Medical Certification Form Without the provider’s signature, the form has no effect.

Where to Send It

Ameren Illinois accepts the completed form by fax at 217-424-6496 or by email at amerenilcredit@ameren.com. It should be accompanied by a cover letter on the provider’s official letterhead. If your provider is submitting a letter in place of the form, it goes to the same fax number or email address.3Ameren Illinois. Ameren Medical Certification Form

Ameren Missouri customers enrolling in the Critical Medical Need Program go through the United Way at 211; a community action agency handles the submission.1Ameren. If a shutoff is imminent, submit as early in the day as you can. Protection begins when Ameren receives the certification, not when the doctor signs it.

How Long the Protection Lasts

In Illinois, a valid medical certificate protects your account from disconnection for 60 days after the date Ameren receives it. If your service was already off before the certificate arrived, the 60-day clock doesn’t start until Ameren restores service.4Illinois General Assembly. Illinois Admin Code Title 83, Section 280.160 – Medical Certification

In Missouri, the Critical Medical Need Program protects your account from disconnection for 30 days.2Ameren. Ameren Missouri Critical Medical Need Pilot Program Tariff

These protections are not bill forgiveness. They pause disconnection. Everything you use during the protection period still shows up on the account.

What You Still Owe During the Hold

In Illinois, Ameren automatically places your account on a Medical Payment Arrangement once a valid certification is on file. If the certification arrived before any disconnection, your first bill due 30 days after certification shows one-twelfth of the total balance, and the remainder splits into 11 equal monthly installments on later bills. If you were already disconnected when the certificate arrived, the first payment is one-quarter of the total balance, followed by nine equal installments.6Illinois General Assembly. Illinois Admin Code Title 83, Section 280.160 – Medical Payment Arrangement A valid medical certificate entitles you to this payment plan even if earlier payment arrangements have fallen through.

In Missouri, the Critical Medical Need Program gives you 30 days to secure payment or set up a payment plan. Contact Ameren or your community action agency inside that window; waiting until the end leaves no room to negotiate.2Ameren. Ameren Missouri Critical Medical Need Pilot Program Tariff

When You Can File Again

This is the rule that catches people. In Illinois, once an account has had a valid medical certificate, it isn’t eligible for a new one until either the full balance has been paid or 12 months have passed from the start date of the prior certification, whichever comes first.4Illinois General Assembly. Illinois Admin Code Title 83, Section 280.160 – Medical Certification You get one 60-day window per year unless you clear the balance. Treat the protection period as time to apply for financial assistance, not as a cycle you can repeat.

In Missouri, call United Way at 211 before your 30-day enrollment ends to ask about extension options.

Programs People Confuse With This One

A few Ameren programs sit near the medical certification form and get mixed up with it. The Medical Equipment Registry puts your address on a priority list for outage restoration and planned-interruption notice, but it does not stop a disconnection for nonpayment. The Medical Hardship Extension is a shorter request from your provider for extra time to pay, separate from the full certification process.1Ameren. Health, Safety and Financial Assistance Options If the underlying balance is the real problem, look into LIHEAP and Ameren’s bill assistance programs while the certification hold is in place, because once that window closes the account is eligible for disconnection again.
