The Byram Healthcare ostomy order form is a one-page document you or your doctor’s office can fax, mail, or submit online to start home delivery of pouches, skin barriers, pastes, and related ostomy supplies. Download it from Byram’s healthcare professionals page, or request a copy by calling 1-877-902-9726 (1-877-90-BYRAM), Monday through Friday 8:30 a.m. to 7:00 p.m. and Saturday 9:00 a.m. to 1:00 p.m. local time.1Byram Healthcare. What to Have Ready Before You Start
A missing piece on the form will hold up your shipment, so gather everything first. The single most important document is a signed prescription, sometimes called a detailed written order, from your treating physician. For Medicare patients, the Centers for Medicare and Medicaid Services requires every order for durable medical equipment and supplies to include six elements:2Centers for Medicare & Medicaid Services. DMEPOS Order Requirements your name or Medicare Beneficiary Identifier (the number on your red, white, and blue Medicare card), a description of each item, the quantity per month, the treating practitioner’s name or 10-digit National Provider Identifier,3Centers for Medicare & Medicaid Services. National Provider Identifier Standard the date of the order, and the practitioner’s signature.
Most private insurers follow the same pattern, though some also want the ICD-10 diagnosis code for your ostomy type. Keep your insurance card next to you. The form asks for plan name, group number, and member ID so Byram can verify coverage before anything ships.
Filling In Each Section
The top of the form is personal and insurance information: full name, date of birth, shipping address, phone number, and insurance details. Medicare beneficiaries should enter the MBI, not an older Social Security–based number. Check the shipping address carefully. A wrong zip code means a box of supplies at the wrong doorstep.
The middle section is the clinical part. Your physician or the office staff enters the diagnosis, the specific product names or SKU numbers, and the quantity for each item. Byram carries products from the major ostomy manufacturers, including ConvaTec, Coloplast, and Hollister, and will not substitute a different product without your consent. If you use a two-piece system with a custom-cut barrier, note the stoma dimensions; Byram offers barrier custom cutting for two-piece systems.4Byram Healthcare. Ostomy Care Products and Supplies
The bottom section is the prescriber’s information and signature. Without a valid signature and NPI, Byram cannot process the order or bill your insurer. If you are filling out the form yourself rather than having the doctor’s office fax it in, make sure the prescription portion is completed and signed before you submit.
Watch the Quantity Limits
Before you write in a monthly quantity, check whether it fits within your plan’s allowance. Medicare Part B covers ostomy supplies as prosthetic devices for anyone who has had a colostomy, ileostomy, or urinary ostomy, and you pay 20 percent of the Medicare-approved amount after meeting the Part B deductible.5Medicare.gov. Ostomy Supplies Coverage Monthly quantities cannot exceed Medicare’s Local Coverage Determination limits unless your doctor documents a medical reason for a higher amount. Common ceilings are 20 drainable pouches, 60 closed pouches, 20 urostomy pouches, 20 skin barriers with flange, and one 4 oz. tube each of paste and lubricant per month.6Byram Healthcare. Medicare Coverage for Ostomy Supplies Some accessories run on longer cycles, including 150 adhesive remover wipes every three months and 10 oz. of protective powder every six months. If your skin or stoma situation requires more, ask your doctor to document the medical necessity in your chart before the order goes in. Private plans often use similar limits, and the specifics vary; Byram contracts with more than 650 insurance plans across over 2,000 plan types.7Byram Healthcare. How to Submit the Form
Byram accepts the form through four channels, and they all land in the same processing queue.
- Phone: Call 1-877-902-9726 and a representative will walk you through the order. This is the easiest route for first-time patients, who can set up an account on the same call.8Byram Healthcare. Ordering Options for Customers
- Fax: Send the completed form to 1-866-992-6331. This is the most common method when a doctor’s office submits the prescription and order together on your behalf.
- Online portal: Log in at mybyramhealthcare.com to place reorders. The portal generates a confirmation number for tracking. New patients place their first order by phone, then enroll in MyByram through the email Byram sends afterward.9Byram Healthcare. Byram Healthcare Home
- Mobile app: The MyByram app offers the same reorder functionality as the web portal.
Keep a copy of the completed form and any confirmation number. If an insurance question comes up weeks later, the original paperwork saves time.
What Happens After You Submit
Byram first runs an insurance verification, checking coverage, your deductible status, and your copay or coinsurance. Most orders clear this stage and ship within 48 to 72 hours when the prescription is current and every required field is filled in. You will receive a shipment confirmation by your chosen contact method, usually email or text.
If the insurer denies the claim or asks for more information, Byram contacts your prescribing physician directly rather than putting that work on you. Common reasons an order sits on hold include an expired prescription, a missing NPI, or quantities above the Medicare allowance without supporting documentation in the chart.
Setting Up Automatic Reorders
Supplies run out on a predictable schedule, and a missed shipment is more than inconvenient. Byram’s EZ Refill program tracks when your next order is due and ships it automatically.8Byram Healthcare. Ordering Options for Customers Enroll through MyByram or by calling 1-866-540-5293. Once enrolled, you get a reminder 10 days before each shipment. If you need to change products, adjust quantities, or move the ship date, contact Byram before that reminder window closes. You can also switch to 90-day supply orders if you prefer fewer, larger deliveries, and you can set notification preferences, including text, email, automated call, or a live customer service call, through the portal or by phone.
If the Shipment Is Wrong or Damaged
Call Byram at 1-877-902-9726 within 15 days of the purchase date. A representative will issue a returned goods authorization (RGA) number, which must accompany anything you send back; Byram will not accept returns that arrive without one.10Byram Healthcare. Byram Healthcare Return Policy Items need to be in their original packaging. Custom-cut items and sterile packages may not be eligible, so ask the representative before shipping anything back. The 15-day window is firm. If you spot a problem, call right away rather than waiting until your supplies run low.
