How to Fill Out and Submit the Car Seat Check Form (NDCF)

To fill out the Car Seat Check Form, log in at carseatcheckform.org or open the NDCF mobile app with your approved technician account, enter the caregiver’s location and vehicle information, work through the five-step inspection (child on arrival, car seat on arrival, findings, departure, and caregiver sign-off), capture the caregiver’s electronic signature, and tap Submit. You can complete checks offline and sync them once you’re back on Wi-Fi.

Set Up Your Account and App First

Registration is not instant, so handle it before your first event. At carseatcheckform.org, click “Create New Account,” complete the fields, and upload a copy of your current Safe Kids wallet card from your online certification profile. No other verification is accepted. Approval takes one to three business days, after which you receive a username and temporary password by email. The Car Seat Check Form app is on Google Play and the App Store. It uses the same account, but you can’t sign in on the app until your web account has been approved. Your first app login must be on Wi-Fi. After that, the app is built to work entirely offline, and the user guide recommends switching to airplane mode during inspections so the app doesn’t glitch while hunting for signal. Reconnect to Wi-Fi before each new event so the app can verify your agency’s liability statement, confirm your technician information, and pull updates.5National Digital Car Seat Check Form. Open a New Record: Caregiver and Vehicle

Each inspection begins with a new caregiver entry. The opening fields are situational, not personal: city, state, county, zip code, inspection date, and event name. You also note whether the caregiver has attended a seat check before and what prompted this visit. The form intentionally skips names, addresses, and other personally identifiable information.6National Digital Car Seat Check Form. NDCF Frequently Asked Questions

Next, record the vehicle’s year, make, model, and trim level. The form sorts vehicles into two groups, 2011 or newer and 2010 or older, because that split changes which anchor and belt configurations the form expects you to evaluate. Keep the vehicle owner’s manual on hand to confirm lower-anchor and top-tether locations, especially on older vehicles where those features can be limited or missing.

Work Through the Five Inspection Steps

Once caregiver and vehicle info are in, the form moves through five sections covering arrival conditions, your findings, and what the caregiver leaves with.5National Digital Car Seat Check Form. NDCF Digital Form User Guide

Step 1: Child on Arrival

Record where the child is seated, the child’s age (in months if under one year), weight in pounds, and height in inches. Note how you obtained those measurements and what currently restrains the child. If the child is in a seat belt rather than a car seat, the form flags whether that belt use is correct for the child’s size.

Step 2: Car Seat on Arrival

Capture the seat’s state before you touch it. Document the restraint type, what it’s installed with (lower anchors, seat belt, or both), and whether it faces the correct direction per the manufacturer. Fill in recline angle, harness position, tether use, handle position, and any non-approved aftermarket products attached. This “before” snapshot is what feeds national misuse data, so complete it before you start correcting anything.

Step 3: Findings on Arrival

Find the manufacturer label on the seat and enter the brand, model name, model number, manufacture date, and expiration date. Record whether the seat is expired, has been recalled, or was involved in a crash. If the history is unknown, say so honestly. Note whether the caregiver has registered the seat with the manufacturer for recall notifications.

Step 4: On Departure

After you’ve made corrections, enter the child’s new seating position, the restraint type now in use, installation method, and whether the departure seat is the same one that arrived. If you supplied a replacement, record who provided it and whether the caregiver met the program’s eligibility rules. Use the “difficulties encountered” field for tricky vehicle configurations or compatibility issues.

Step 5: Caregiver Sign-Off

The caregiver confirms their own role. Check the boxes for whether they physically harnessed the child or a doll and whether they installed the seat themselves during the check. They review the liability release statement, add their initials, and sign electronically.

Record the Topics You Discussed

Before submitting, mark the safety topics you covered with the caregiver. The options include airbag risks, unused seat belts, projectiles, premature transition to a larger seat, heatstroke prevention, bulky clothing, vehicle recalls, and safe sleep. These checkboxes create a record of the education you delivered, which agencies use when applying for grants or reporting to funders.

Submit Online, or Save and Sync Later

If you have Wi-Fi or data, tap the green Submit button after sign-off. The record uploads straight to the NDCF database and shows as “Submitted” under the vehicle info section of the app, at which point you can delete it from the device.5National Digital Car Seat Check Form. At events with no connectivity, completed forms sit inside the app and don’t eat extra device storage until uploaded. Once you’re back on Wi-Fi, open the app, turn off airplane mode, and submit the stored forms. The app will prompt you to reconnect before it lets you submit, so you can’t accidentally lose data by tapping Submit while still offline.

Check Your Records in the Dashboard

Submitted records live in a secure online dashboard. Technicians can review and download their own data in real time. Program managers can pull agency-wide data, build custom reports, track misuse trends, and use the numbers for grant applications and reporting.3CPS Board. National Digital Car Seat Check Form After an event, use the dashboard to verify every form actually synced. If one shows incomplete or unsubmitted, open it through the web platform and fix it before the data ages out of the app’s local storage.

What the Form Does Not Collect

The NDCF does not collect personally identifiable information from caregivers. Geographic data stops at city, county, and zip code, and the caregiver’s electronic signature confirms participation without being linked to a name or contact record. All data is stored on a HIPAA-compliant server.7Car Seat Check Form.
