Pick the Right Form for Your Situation
There is no single catch-all document. Carnival splits accessibility requests into eleven categories so each one routes to the team that handles it. You can complete more than one if your needs span categories.

What You Need Before You Start
Each form asks for details specific to the accommodation. Having the information ready before you open the form saves a second trip.
Mobility Devices
The Mobility Devices Form asks for the width, length, height, and weight of every wheelchair or scooter you plan to bring, plus the combined weight of the passenger and the device if you also need wheelchair-accessible ground transportation.2Carnival Cruise Line. Aftermarket accessories, baskets, and armrest extensions can add inches that matter at a narrow cabin doorway.
The form also needs to know your battery type. Carnival permits gel, dry cell, absorbed glass mat, and lithium-ion batteries. Wet-cell (flooded lead-acid) batteries are prohibited, and batteries must be charged inside your stateroom. Device dimensions also determine which stateroom types will work for you. Any scooter wider than 21 inches has to be booked into a Fully Accessible Cabin. Oxygen, CPAP, and Other Medical Equipment
The Oxygen, Concentrator, and CPAP Machine Form asks for the device model, the quantity of tanks if applicable, and your required flow rate or pressure settings.5Carnival Cruise Line. Shipboard Medical Services FAQs If you use insulin, need a sharps container, or require refrigerated medicine storage, those requests go on the Diabetes, Sharps, Medicine Storage, and Medical Care Form. Dialysis has its own dedicated form. The ship’s medical center and your stateroom steward both need advance notice to prepare storage space or arrange power, so submit early.
Allergies and Dietary Restrictions
The Allergies Form asks you to flag specific allergens such as gluten, dairy, tree nuts, or shellfish. Filing it pre-cruise lets the galley plan, but the request is not complete on its own. On embarkation day you still need to visit the maître d’ or head waiter during their posted hours to review the next evening’s menu and place your first allergy-safe order.1Carnival Cruise Line. Service Animals
Only working service dogs individually trained to perform disability-related tasks are permitted. Emotional support animals, therapy dogs, and service dogs still in training are not allowed on board.6Carnival Cruise Line. If your itinerary visits non-U.S. ports, your dog may need additional documentation. A USDA-endorsed international health certificate typically requires the original export health certificate from your veterinarian, proof of current rabies vaccination, and, depending on the destination, a rabies titer test or import permit. The USDA waives its endorsement fee for ADA-defined service animals.7USDA APHIS. Deaf or Hard of Hearing
The Deaf or Hard of Hearing Form covers a portable room kit with visual-tactile alerts for the door knocker, telephone, alarm clock, and smoke detector, along with TTY access at Guest Services and assistive listening headsets for live shows. If you need a sign language interpreter, note it on the form. Interpreters are offered on a shared basis for cruises departing from and returning to a U.S. port, covering main production shows, port presentations, and other major organized activities, and Carnival strongly encourages requesting one at least 60 days before sailing.8Carnival Cruise Line. When to Submit
Carnival does not publish one universal deadline. A few accommodations have firm advance-notice windows, and everything else falls under “at the time of booking, or at your earliest possible convenience.”
- Sign language interpreters: 60-day notice strongly encouraged.
- Airport transfers with hydraulic lift, Europe departures: 30 days before sailing.
- Airport transfers with hydraulic lift, North America departures: no later than 14 days before sailing.
- Service dog vaccination records: emailed to access@carnival.com at least one week before sailing.
For everything else, the closer to sail date you submit, the less time the team has to arrange equipment, verify cabin compatibility, or coordinate with the galley. Complete the questions specific to that form and submit. Repeat for any additional forms you need. After You Submit
You should receive an email confirming the request was logged. A coordinator from the Guest Access Department reviews it and may follow up if something is missing. Incorrect device dimensions, unclear allergy details, and an incomplete booking number are the usual reasons for a callback. Carnival’s standard language is that it will make every reasonable effort to accommodate you, meaning most requests are approved but the ship has physical limits.5Carnival Cruise Line. On embarkation day, stop at Guest Services early to confirm your arrangements are in the system. If you filed a dietary form, visit the maître d’ during their posted hours to review your first dinner order. For mobility devices, your stateroom steward should already know about your equipment, but verifying in person catches anything that slipped through.
Pregnancy Uses a Different Process
Pregnancy is not handled through the disability pre-cruise forms. Carnival has a separate policy and a firm cutoff: you cannot sail if you will be beyond your 24th week of pregnancy by the end of the cruise, not the start. Carnival requires a physician’s letter confirming that both mother and baby are in good health, that the pregnancy is not high-risk, and stating the estimated due date.9Carnival Cruise Line.
