How to Fill Out and Submit the Clips4Sale Performer Consent Form

To put a performer in content you sell on Clips4Sale, you need a Clips4Sale performer consent form that the performer has signed and dated, plus a copy of their valid government-issued photo ID, on file before the clip goes live. Clips4Sale does not publish a single official template. You draft or source your own release, as long as it contains four specific consent statements the platform requires, and you pair it with the identity records that federal law under 18 U.S.C. § 2257 demands.1Clips4Sale Creator Help Center. Where Can I Get a Consent Form for My Third Party Performers?

The Four Consent Statements Your Form Must Contain

Whatever release language you use, the performer has to agree in writing to all four of these points:

  • Being depicted in the content.
  • Public distribution of the content.
  • Upload of the content to Clips4Sale.com.
  • Allowing the content to be downloaded.

Each point must appear somewhere in the form. The performer signs and dates it, and for content you produce yourself you need a signed and dated form for every production date, not a single blanket release covering an open-ended period.2Clips4Sale Creator Help Center. Identity Details to Capture on the Form

Federal record-keeping law requires that your records show the performer’s legal name and date of birth, both verified from a photo ID, along with every other name the performer has ever used. That includes stage names, aliases, nicknames, maiden names, and professional names.3Office of the Law Revision Counsel. 18 USC 2257 – Record Keeping Requirements Most producers build these fields directly into the consent form or attach an intake sheet so everything sits in one document.

The performer’s home address is not required by federal rules. Addresses, phone numbers, and Social Security numbers can be redacted from record copies as non-essential.4eCFR. 28 CFR 75.2 – Maintenance of Records Collect an address if your contract or business practice calls for it, but don’t treat it as a legal obligation.

The Photo ID You Have to Keep With the Form

Every performer must provide a valid government-issued photo ID confirming they are at least 18 and have reached the age of majority where the content was produced.5Clips4Sale. Content Rules A driver’s license, state ID card, or passport works. It can’t be expired on the production date, and the photo has to be recognizable as the performer.

Looking at the ID is not enough. Under 28 CFR 75.2, you must keep a legible hard copy or a digital scan of the document you examined. If the ID you used to verify age doesn’t carry a recent, recognizable photo, you need to also keep a copy of a separate picture ID that does. For content published online, your records must also include a copy of the content itself and any associated URL.4eCFR. 28 CFR 75.2 – Maintenance of Records

Submitting the Consent Form Through the Creator Workspace

Clips4Sale handles consent through a Digital Consent workflow in the Creator Workspace, which lets you email a consent request to the performer directly. For a new upload:

  • Log in to the Creator Workspace and open “My Content” from the main menu.
  • Upload your clip and go to the Performers section.
  • Pick the co-performer from the dropdown, or add a new performer.
  • Click “Request Digital Consent” and enter the performer’s stage name and email address.
  • Click “Assign Performer” to send the request, then finish your clip details and submit for review.

If you need to attach performer documentation to clips that are already uploaded, go to My Content, then Content Management, and open the Action Required tab. Select the clips, choose Bulk Action, then Add Performer(s), and run the same assignment and consent-request steps across all of them at once.6Clips4Sale. If You’re Uploading Licensed Content From Another Producer

The paperwork is different when the clip isn’t yours to begin with. For licensed content, you need a licensing agreement from the legal copyright owner, signed and dated model releases from every performer in the clip, and clear copies of each performer’s photo ID showing their date of birth.2Clips4Sale Creator Help Center. The Digital Consent email flow is built for performers you shot with; licensed work travels with its own release and ID copies from the original producer.

Holding on to the Form After Upload

Submitting the consent through Clips4Sale does not end your obligation. Under federal rules, you must keep each performer’s records for seven years from the date they were created or last amended. If you shut your business down, the records still have to survive for five years after you stop operating.7eCFR. 28 CFR Part 75 – Child Protection Restoration and Penalties Enhancement Act When a performer’s details change, update the record. The seven-year clock runs from the last amendment, so keeping the file current also keeps it legally active for the full retention window.

Records must be kept at your business premises and made available for inspection by the Attorney General or an authorized designee at reasonable times.7eCFR. 28 CFR Part 75 – Child Protection Restoration and Penalties Enhancement Act

What Happens If the Paperwork Is Missing or Wrong

Clips4Sale’s compliance team reviews submitted documentation and will flag or restrict stores that don’t have proper performer records on file. Unresolved documentation issues can keep you from uploading or selling new clips.

The federal exposure is heavier. Failing to create or maintain the required records, making false entries, or refusing a federal inspection are each separate violations under 18 U.S.C. § 2257(f). A first offense carries up to five years in federal prison and a fine. A second or later conviction carries a mandatory minimum of two years and a maximum of ten, plus fines.3Office of the Law Revision Counsel. 18 USC 2257 – Record Keeping Requirements If you hit a consent or content-removal problem you can’t fix in the dashboard, the Clips4Sale Creator Experience Team takes email, chat, and phone contact around the clock.
