How to Fill Out and Submit the CNA Cancellation Request Form

The CNA cancellation request form for a CNA National vehicle service contract starts at the dealership where you bought the coverage. The selling dealer completes and signs part of the form before it goes to CNA National’s processing office in Scottsdale, Arizona.1CNA National. Customers – Section: Changing or Cancelling a Contract The completed form must reach CNA National within 30 days of the cancellation date you write on it, and refunds typically take about 30 days to process after that.2CNA National Warranty Corporation. What to Gather Before You Start

Pull these together before you contact the dealership so you can finish the form in one visit:

  • Your contract number, found in the upper corner of your original service contract (Part I or Part II).
  • The 17-character Vehicle Identification Number from your dashboard plate, driver-side door jamb, or registration.
  • Your current odometer reading on the day you initiate the cancellation. The form calls this the cancellation mileage.
  • The lienholder’s name if you still have an active auto loan. If the loan is paid off, bring proof of payoff instead.3CNA National Warranty Corporation. CNA VSC Cancellation Request – Fillable
  • Supporting documents for your cancellation reason. A repossession needs proof from the lienholder; a total loss needs a statement from your insurance company or lienholder; a voluntary cancellation just needs your signature.

Getting the Form From the Dealer

CNA National routes cancellations through the selling dealership.1CNA National. Customers – Section: Changing or Cancelling a Contract Call the finance department where you bought the contract and ask to start a cancellation. The dealer holds the VSC Cancellation Request form and has to complete their own section, including the selling dealer code and signature.2CNA National Warranty Corporation. VSC Cancellation Request

If the dealership has closed, changed ownership, or won’t cooperate, call CNA National directly at 800-345-0191 during business hours.1CNA National. Customers – Section: Changing or Cancelling a Contract CNA National’s website also carries a contact form with a “Cancel a Contract” option. The company can walk you through an alternative process when the original dealer isn’t available.

Filling Out the Form

The form has four parts: your cancellation reason, your customer and vehicle information, a short questionnaire, and signature blocks for you and the dealer.3CNA National Warranty Corporation. CNA VSC Cancellation Request – Fillable

Cancellation Reason

Check one of four boxes at the top:

  • Customer Request. The most common reason. Your signature, or a signed letter attached, is the only documentation needed.
  • Total Loss. Attach a statement from your insurance company or lienholder confirming the total loss.
  • Repossession. Attach proof of repossession from the lienholder.
  • Other/Flat Cancel. Write an explanation in the space provided and attach supporting documentation. Flat cancellations outside CNA National’s normal guidelines need extra justification.

Customer and Vehicle Information

Enter your name, address, and email. The email field is required for Florida residents. Fill in the contract number, VIN, cancellation mileage, and cancellation date. Add the lienholder’s name if you still owe on the vehicle, or note that the loan is paid and attach your payoff confirmation.2CNA National Warranty Corporation. VSC Cancellation Request

Sign and date your section. The selling dealer then completes theirs with their name, dealer code, signature, and date. The dealer signature isn’t optional. The form is built so both parties sign before submission.

The Customer Questionnaire

The bottom of the form carries five yes-or-no questions. These are disclosures CNA National uses to confirm you understood the terms before processing. They cover:

  • That if the contract was financed with your auto loan, the refund goes to the lienholder, not to you.
  • That a refund sent to the lienholder reduces your loan principal but will not lower your monthly payment.
  • That the refund is calculated on the unused portion of time or miles remaining.
  • That you become responsible for all mechanical repairs beyond the factory warranty once the contract is cancelled.
  • That a cancellation fee may apply, as outlined in Part I and Part II of your contract.3CNA National Warranty Corporation. CNA VSC Cancellation Request – Fillable

Answer all five. Blanks can delay processing.

Submitting the Form

In most cases the dealership submits the signed form to CNA National for you. If you’re sending it directly, because the dealership is closed or CNA National told you to, mail the completed form and all supporting documents to:

Use certified mail with return receipt requested. The form states that all requests must be received by CNA National within 30 days of the cancellation date you wrote on it.2CNA National Warranty Corporation. How Your Refund Is Calculated

CNA National calculates refunds pro rata on the unused portion of your contract’s time or mileage, whichever produces the smaller refund, then deducts any applicable cancellation fee.3CNA National Warranty Corporation. CNA VSC Cancellation Request – Fillable The cancellation fee itself depends on your specific contract. The form directs you to check Part I and Part II of your service agreement and any applicable state requirements. There is no single fee that applies to every CNA National contract.

Where the Refund Check Goes

If you still owe money on your vehicle loan and the service contract was rolled into that financing, the refund goes to the lienholder, not to you. The lender applies it to the loan’s principal. That lowers your total balance but does not reduce your monthly payment. The payment amount stays the same and the loan pays off slightly sooner.3CNA National Warranty Corporation. CNA VSC Cancellation Request – Fillable

If you own the vehicle outright or have paid off the loan, attach proof of payoff so CNA National can issue the refund directly to you. If the refund is going to a lender, contact them after submission so you can track when the credit hits your account.

The Free-Look Period

Most vehicle service contracts include a free-look or cooling-off window early in the contract term. If you cancel within that window and haven’t filed any claims, you’re generally entitled to a full refund rather than a pro-rata amount. The length varies, commonly 30 to 60 days depending on your state and whether the vehicle is new or used. Check Part I of your CNA National contract for the specific language, because once that window closes the pro-rata formula applies and the refund shrinks with every mile you drive.

Total-Loss Cancellations and GAP Coverage

If your vehicle was declared a total loss, cancel the service contract promptly. You’re paying for coverage on a vehicle you can no longer drive. Attach the total-loss statement from your insurance company or lienholder to the form.

One detail catches people off guard. If you also have GAP coverage financed alongside the vehicle, the refund from your cancelled service contract can reduce your GAP payout. GAP waivers typically subtract refunds owed on cancellable products from the amount the GAP provider pays toward your remaining loan balance. The service contract refund still goes toward paying down the loan, but the GAP claim is smaller by that same amount. You don’t lose money, and you don’t double-dip either.

If the Dealer Won’t Help

Dealerships sometimes drag their feet on cancellations because the dealer may have to return a portion of their commission. If the dealer is unresponsive or refuses to process your request, call CNA National at 800-345-0191 during business hours and explain the situation.1CNA National. Customers – Section: Changing or Cancelling a Contract You can also submit a request through the “Cancel a Contract” option on CNA National’s website contact form.

If neither the dealer nor CNA National resolves the cancellation in a reasonable time, your state’s attorney general office or department of insurance handles consumer complaints against service contract providers. Vehicle service contracts are regulated differently from state to state. Some states treat them as insurance products, others regulate them under consumer protection statutes, so the right agency to contact depends on where you live. File a written complaint with supporting documentation, including your signed cancellation form, proof of mailing, and records of any phone calls.
