The Delta Identity Verification Form is an online upload tool for proving who you are when you need to change your SkyMiles name, date of birth, gender, email, or (if you’re locked out) mailing address. You reach it at delta.com/contact-us/identity-verification or through the SkyMiles Help page once you’re signed in. You upload a photo of a government-issued ID, add any supporting legal document the change requires, and Delta emails you when the update is finished.1Delta Air Lines. SkyMiles Help
When Delta Requires the Form
Any change to core profile details triggers verification. That covers name changes after marriage, divorce, or a court order; corrections to your date of birth or gender marker; updates to your primary email, particularly when you no longer have access to the old one; and mailing address changes made while you’re locked out of delta.com.1Delta Air Lines. Accepted Photo ID
Delta accepts one of three government-issued IDs, and asks for photos of both sides even when only one side carries your picture:2Delta Air Lines. Identity Verification Form
- Driver’s license (front and back)
- Passport (front and back)
- Military ID (front and back)
Whichever you use, the document has to show both your photograph and your signature, be issued by a local, city, state, or national government, and be currently valid. An expired ID will stall the request.2Delta Air Lines. Identity Verification Form
If you don’t hold a U.S. driver’s license, a passport is the standard option. Delta requires the name on your boarding pass to match the name on your passport, so your SkyMiles profile should carry that same name.3Delta Air Lines. Travel Outside of the U.S.
Extra Documents for Name, Date of Birth, and Gender Changes
A photo ID by itself is enough for simple updates like email or address. Changes to your name, date of birth, or gender need a second document.1Delta Air Lines. SkyMiles Help
For a name change, upload the court document behind the change. Delta lists court orders, marriage certificates, and divorce certificates as examples.1Delta Air Lines. SkyMiles Help Update your driver’s license or passport first, because the new SkyMiles name has to match the ID you’ll use at the airport. If they don’t match, Delta may not process the change.
For a date of birth or gender update, Delta asks for identity verification documentation, such as a driver’s license or passport, that already shows the corrected information.1Delta Air Lines. SkyMiles Help The corrected detail needs to appear on your government ID before Delta will change it on your profile.
How to Reach the Form and Fill It Out
There are two ways in, and they lead to the same upload tool.1Delta Air Lines. SkyMiles Help If you can sign in, go to delta.com, open your SkyMiles account, and follow the Identity Verification Form link on the SkyMiles Help page. If you’re locked out, go straight to delta.com/contact-us/identity-verification; that version doesn’t require an active session, which matters when you’ve lost access to the email on the account.
On the form, enter your SkyMiles membership number and personal details, describe the change you’re asking for, and upload your documents. Make every field match your ID exactly. Small mismatches between what you type and what appears on the uploaded document are a common source of delay.
File Format and Image Quality
The upload portal accepts .pdf, .png, .jpeg, and .jpg files, with a 4 MB cap per file. Beyond the technical limits, Delta warns that documents must be clear, right-side-up, and readable. Blurry, rotated, or dark images get flagged as unreadable and routed to manual processing, which takes longer and may prompt a reservation specialist to call you for additional verification.2Delta Air Lines. A few practical steps head off that outcome. Use your phone’s document scanning mode rather than a casual snapshot. Make sure all four corners of the ID show in the frame. Check that your name and date of birth are legible before you upload. For marriage certificates or court orders, a flatbed scanner produces cleaner output than a phone camera held over a folded page.
After You Submit
Delta says you’ll receive an email once your request is complete.2Delta Air Lines. Identity Verification Form The form page does not publish a specific timeline. Straightforward changes like an email update tend to move faster than name changes that need document review, but Delta does not publicly commit to a set number of business days for either.
Watch the email address on your account for the confirmation, or the new email you asked Delta to switch to. If you submitted through the locked-out path, watch the address you entered on the form itself.
If the Request Stalls or Gets Denied
Unreadable documents are the most common reason a submission slows down. When the automated system can’t process an upload, the request goes to manual review and a reservation specialist may contact you for clearer copies or more information.2Delta Air Lines. Identity Verification Form Expired IDs, missing signatures, and files over the 4 MB limit will also hold things up.
Delta does not publish a formal appeal process for denied verification requests. If you’re stuck, contact a SkyMiles representative: the dedicated SkyMiles line is 800-323-2323, and the Help Center page offers a “Message Us” chat as well as an online comment form.4Delta Air Lines.
