How to Fill Out and Submit the Drexel Immunization Record Form

Every entering full-time undergraduate and graduate student at Drexel has to complete the Drexel immunization form before registering for classes. The form is a two-part PDF: you fill out Part 1 with your own information, your healthcare provider completes and signs Part 2, and the signed record gets uploaded through the Medicat system inside DrexelOne.1Drexel University. Immunizations Miss the deadline and an administrative hold blocks you from registering for the following term.

Who Has to Submit It

The requirement covers all entering full-time undergraduate and graduate students, whether they live on campus or commute. Fully online students are the exception and do not need to comply with the immunization policy.1Drexel University. Immunizations If you later switch from online to an on-campus program, expect to satisfy the requirement before that change takes effect.

Two groups use a different path. College of Medicine MD students email their records to imm_surv@drexel.edu rather than uploading through DrexelOne. Students in programs with a clinical component — nursing, physician assistant, and similar health-sciences tracks — use the Clinical Compliance Immunization Form, which carries additional requirements beyond the general record.1Drexel University. What Vaccinations the Form Requires

Four vaccinations need to be documented on the general record:

  • Tdap: one dose dated 2005 or later and within the last ten years. If your Tdap is older than ten years, a Td booster is accepted alongside the earlier Tdap record.2Drexel University. Drexel University Immunization Record
  • MMR: two doses, with the second given at least four weeks after the first. A blood test (titer) showing immunity also works; attach a copy of the lab report if you go that route.2Drexel University. Drexel University Immunization Record
  • Varicella: two doses at least four weeks apart, a titer with lab report, or a documented history of having had chickenpox. You check the applicable box on the form.2Drexel University. Drexel University Immunization Record
  • Meningococcal (ACWY): one dose of a quadrivalent conjugate vaccine such as Menactra, Menveo, or MenQuadfi, received since age 16. This is required for all full-time undergraduates age 21 or younger and for all students living in university housing. The Meningococcal B vaccine is not currently required.2Drexel University. The form also includes a Tuberculosis Screening Questionnaire, with directions printed on the form itself.1Drexel University. Immunizations If you are arriving from a country with higher TB prevalence, pay close attention to that section; additional testing such as an IGRA blood test may be needed depending on your risk factors and travel history.

Filling Out Part 1

You complete Part 1 yourself. Enter your last name, first name, middle initial, date of birth, and your eight-digit Drexel student ID.2Drexel University. Drexel University Immunization Record Your student ID was assigned when you were admitted and appears on your acceptance letter and in DrexelOne. Use your legal name exactly as it appears in Drexel’s records. A name mismatch is one of the fastest ways to trigger a processing delay.

Filling Out Part 2

Part 2 is for your healthcare provider. Bring the form to your doctor, a campus health center, or whoever holds your vaccination records. The examiner enters the dates for each required vaccine, verifies your identity, and signs and dates the form. The signature certifies that the provider either administered the vaccines or reviewed documentation of your immunization history.2Drexel University. Uploading Through Medicat

Submission happens online through Drexel’s Medicat portal:

  • Log in to DrexelOne at one.drexel.edu with your university credentials.
  • On the Welcome page, find the Health and Wellness Resources channel in the center column and click through to Medicat.
  • Under the Immunizations tab, select each vaccine from the drop-down in the “All Required” box, enter the dates you received each dose, and hit Submit after each entry.
  • Use the Uploads tab to upload a single clear scan or photo of your signed form along with any lab reports.

Forms take about 48 hours to process. Log back into DrexelOne after that window and check for anything flagged on your record.1Drexel University. Immunizations A successful upload isn’t the same as an accepted record. A missing date, an illegible scan, or a forgotten lab report can leave your file sitting in limbo, and checking at the 48-hour mark gives you time to fix the problem before a hold hits your account.

Exemptions

Pennsylvania law provides three grounds for exemption. A medical exemption applies when a physician provides a written statement that a particular vaccine would be harmful to the student’s health; once the physician determines the risk has passed, the student is expected to get vaccinated.3Pennsylvania Code. 28 Pa Code 23.84

A religious or moral exemption is available when a student objects in writing on religious grounds or on the basis of a strong moral or ethical conviction similar to a religious belief.3Pennsylvania Code. 28 Pa Code 23.84 The written objection should clearly explain the basis for your position. Exemption request forms are available through Drexel’s immunization page.

An approved exemption has a limit worth knowing about. If a vaccine-preventable disease outbreak occurs on campus, students with exemptions may be excluded from campus activities until the outbreak is resolved.

If You Miss the Deadline

Any student who has not met the requirement by the deadline gets an administrative hold on their record, and that hold blocks registration for the following term.1Drexel University. Immunizations It lifts only after you upload a completed and approved record or obtain an approved exemption. If a hold is already on your account, contact the Office of Health Insurance and Immunizations directly rather than the registrar.

Deadlines shift year to year and are communicated through email and DrexelOne ahead of each term, so check the immunization page or your incoming-student messages for the current date.
