The form commonly called the “EBF-1” for the NYC Management Benefits Fund is actually Form 1060, the Application for Membership used by the NYC Office of Labor Relations.1NYC.gov. Management Benefits Fund Home If your agency told you to complete an EBF-1, Form 1060 is almost certainly what they mean. You get it from your agency benefits office, you have 31 days from your appointment or qualifying life event to submit it, and the same form covers new enrollments, re-enrollments, dependent changes, agency transfers, and name changes.
When You Need to File
New employees must submit a completed Form 1060 to their agency personnel office within 31 days of their appointment date.1NYC.gov. Management Benefits Fund Home The clock starts on your first day. Miss it and you lose access to MBF benefits until the next available enrollment window.
The same 31-day deadline applies to dependent changes. File a new Form 1060 when you gain a dependent through marriage, domestic partnership, birth, or adoption, and when you lose one through death, divorce, legal separation, or the end of a domestic partnership.1NYC.gov. Management Benefits Fund Home You also use Form 1060 when transferring between city agencies or legally changing your name.
One point to confirm before you file: MBF membership is limited to managerial and confidential employees who are not represented by a union, in positions approved as ineligible for collective bargaining by the Department of Citywide Administrative Services, working at least 20 hours a week.2NYC.gov. What to Gather Before You Start
Partial submissions get rejected, so pull everything together before you open the form. You will need:
- Social Security numbers for yourself and every dependent you plan to list.
- Your agency code and pension or BAR number. Both appear on your pay stub, or your agency personnel officer can give them to you.
- A marriage certificate when adding a spouse.
- A domestic partner registration certificate when adding a domestic partner.
- A birth or adoption certificate for each dependent child under age 26.
- Legal documentation for any name change, such as a court order showing the new legal name.
New members attach all relevant documentation at the time of enrollment. Current members adding dependents later must include the appropriate certificates for each new dependent.4H+H Internet. Management Benefits Fund Membership Form 1060
How to Complete Form 1060
The form has to be filled out in full every time you submit it, even for a small dependent change. You cannot leave prior sections blank and send in only the updated piece.4H+H Internet. Management Benefits Fund Membership Form 1060
At the top of the form, check the box that matches your reason for filing: new enrollment, re-enrollment, agency transfer, adding or dropping a spouse or domestic partner or dependent children, or a name change. The member sections then ask for your personal information, agency details, and dependent information. List every eligible dependent you want covered and attach the corresponding documentation for each one.
Give the beneficiary designation section extra attention. It covers life insurance and accidental death and dismemberment coverage, and it needs full legal names along with each person’s relationship to you. This is where errors most often appear, and an incomplete designation can create real problems for your family if a claim is ever filed.
Section VIII, Agency Certification, is not yours to fill in. Your authorized agency personnel officer completes it to certify your eligibility for MBF membership.4H+H Internet. Coordinate with your benefits office and confirm the form was actually transmitted. Handing it to someone is not the same as it arriving.
