How to Fill Out and Submit the ECFMG Transcript Request Form

For the 2027 ERAS season opening June 4, 2026, an ECFMG transcript request for USMLE scores is handled by the Federation of State Medical Boards, not by ECFMG or NBME.1United States Medical Licensing Examination. FSMB to Process USMLE Transcripts for ERAS Starting in June 2026 You pay a one-time $70 fee at FSMB’s USMLE portal, then initiate score retrieval inside MyERAS. Residency programs cannot see your scores until both steps are done.

What to Have Ready Before You Start

Pull together your identifiers first. You need your USMLE Identification Number, which FSMB now issues to international medical graduates when they register for their first Step exam.2Intealth ECFMG. MyIntealth Identification Number A USMLE ID issued by ECFMG before the January 2026 transition is still valid, and you can find it under My Profile in MyIntealth. You also need your MyIntealth Identification Number, formerly called the ECFMG ID, which was issued when you created your MyIntealth account.

Your name, gender, and date of birth must match your passport exactly across every system. When you set up MyIntealth, ECFMG reconciled your submitted information against your passport data and changed any field that didn’t match.3Intealth ECFMG. MyIntealth Applicant Account Establishment and Identity Verification Before requesting your transcript, confirm the name in MyERAS’s Personal Information section matches the name tied to your USMLE ID. A mismatch between those records will block ERAS from fetching your transcript.

Pay and Authorize at the FSMB Portal

The first step happens outside MyERAS. Log in to FSMB’s USMLE Portal at usmle.fsmb.org, pay the transcript fee, and authorize release of your scores to ERAS.4AAMC Students and Residents. USMLE Transcript MyERAS no longer collects the transcript processing fee, so payment must be complete at FSMB before you move on. Without that authorization on file, the retrieval step inside MyERAS will not work.

The fee for the 2026–2027 cycle is $70, charged once per residency application cycle.1United States Medical Licensing Examination. FSMB to Process USMLE Transcripts for ERAS Starting in June 2026 That single fee covers every residency program you apply to through ERAS that season; there is no per-program charge. Once an order is submitted, it cannot be canceled, changed, or refunded. Do not place an order before your scores have officially posted, because FSMB will not hold a request for scores that aren’t yet available.5Federation of State Medical Boards. Transcripts Overview

Retrieve Your Transcript in MyERAS

Once payment is complete at FSMB, switch to MyERAS and open the Additional Documents section. In the row for USMLE Transcripts, select “Initiate Score Retrieval” under the Actions column.4AAMC Students and Residents. USMLE Transcript The system will ask you to enter or confirm your USMLE ID. Check the number carefully. If the USMLE ID in MyERAS is wrong, ERAS cannot fetch your transcript from FSMB. Select “Yes” to confirm, then “Save.”

After saving, select “Get Scores” to pull your authorized exam results into ERAS.4AAMC Students and Residents. USMLE Transcript The transcript then becomes available to assign to the programs you’ve selected. FSMB typically processes transcript orders within 24 hours of submission.5Federation of State Medical Boards. Transcripts Overview

Confirm Programs Received It

After initiating retrieval, open the USMLE Transcript Request Report inside MyERAS. Each row shows one program and the current status of your transcript for that program.4AAMC Students and Residents. USMLE Transcript You will see one of three statuses:

  • Not Received: ERAS has not yet retrieved a transcript from FSMB for this program.
  • Received: The transcript was retrieved and is available to the program.
  • No Scores: No score data is available at FSMB at this time.

When a row updates to “Received,” the report also shows the Most Recent Score Transfer Date with a timestamp, which Step exams are included, and the associated exam dates. That report is your definitive confirmation that a program can see your scores.

Sending Updated Scores Later

If a new Step score posts after your initial transcript has already gone out, you have to trigger the update yourself. Transcripts do not refresh automatically. Open the Additional Documents page in MyERAS and select “Resend My Scores” under the Actions column.4AAMC Students and Residents. USMLE Transcript You can do this as soon as the new score appears in FSMB’s system.

One thing to know before you click it: a resend goes to every program that previously received your transcript. You cannot pick and choose programs.4AAMC Students and Residents. USMLE Transcript After the resend is fulfilled, the Request Report refreshes the Most Recent Score Transfer Date and adds the newly released exam to the score columns.

Fixing a Name Mismatch

If the name on your medical diploma or transcript differs from the name in your MyIntealth record, ECFMG wants documentation proving both names belong to the same person.6Intealth ECFMG. Name on Medical Diploma and Transcripts Acceptable documents include an expired passport (photograph and expiration date pages), a birth certificate, a marriage certificate or license, a court order or official name change document, a U.S. immigration document (Resident Alien Card, Naturalization Certificate, or Permanent Residence Card), or a driver’s license.

If none of those are available, ECFMG may accept a letter from an authorized official at your medical school. The letter must be on official letterhead with the school’s seal, signed by someone whose name appears on ECFMG’s list of authorized officials, and must include the statement: “This certifies that the names [name on document] and [name in Intealth record] belong to one and the same person.”6Intealth ECFMG. Name on Medical Diploma and Transcripts Documents in a language other than English need an official English translation. Self-attestations are not accepted, and failing to provide documentation will result in rejection of your certification application.

When the Status Will Not Move

The most common cause is a USMLE ID mismatch. If the ID in MyERAS doesn’t match what FSMB has on file, retrieval fails silently; there is no error message, just a status that never moves past “Not Received.” Verify your USMLE ID in the My Profile section of MyIntealth and make sure MyERAS shows the same number in both the Personal Information section and the Additional Documents page.

Transmission glitches between FSMB and ERAS do happen, especially around peak application dates. If your status is stuck, AAMC recommends using “Resend My Scores” in Additional Documents to trigger a fresh retrieval attempt.4AAMC Students and Residents. USMLE Transcript If that doesn’t resolve it within a few business days, contact FSMB and ERAS directly.

State Medical Board Requests Are Separate

The process above is for residency applications only. If a state medical board or licensing authority needs verification of your ECFMG certification status, that is a different service, the Certification Verification Statement, requested from the MyIntealth Applicant Portal under Services, with a $66 fee and $5 more for a paper report.7Intealth ECFMG.
