To cancel with the F45 membership cancellation form, contact the studio where you signed up and ask for its cancellation paperwork; every F45 is an independently operated franchise, and there is no single corporate form.1F45 Training. How Do I Cancel or Pause My F45 Membership? Your studio may hand you a paper form, direct you to a form embedded on its local page of the F45 website, or accept a written request by email.2F45 Training. FAQs Whatever the format, the form is only half the job. The other half is submitting it in a way that gives you proof, so a lost request doesn’t turn into months of extra charges.
Where to Get the Form
Start with your home studio. F45’s own help center says the studio will provide the relevant paperwork or walk you through its process.1F45 Training. How Do I Cancel or Pause My F45 Membership? Some locations post a cancellation request form on their studio page at f45training.com; completing it sends the request to the studio team, who then verifies it against your contract.2F45 Training. F45 studios typically run their billing through Mindbody or Glofox.3F45 Training. How Can I View My Purchase History in Mindbody If yours uses Mindbody, you may be able to manage auto-renewal contracts through the Mindbody web app under the Passes & Memberships screen.4Mindbody. Passes and Memberships Screen (Mindbody Web App) Don’t assume the app alone finishes the job. Confirm with the studio that the cancellation was actually processed on their side.
What to Put on the Form
Whether the studio hands you a printed form or asks for a written request, include enough information for staff to find your account and act on it without follow-up:
- Full legal name as it appears on your membership agreement
- Member ID number, found on your original contract, in the Mindbody or Glofox app, or on a billing receipt
- Studio location where you signed up
- A clear cancellation statement, such as: “I am requesting cancellation of my F45 membership effective [date].”
- Your desired final date, taking into account any notice period your contract requires
- Current mailing address, email, and phone number so the studio can send written confirmation
- Signature and date
You don’t have to give a reason. If the form asks, something like “personal reasons” is enough. What matters is that the request is unambiguous: you want the account closed and billing stopped.
Check Your Contract Before You Submit
Because each F45 studio writes its own contract terms, the notice period and any early exit penalty depend on what you signed. Some studios require 14 days’ written notice after the minimum term is completed.5F45 Training. F45 Website Terms and Conditions of Use Others require 30. Pull out your agreement and find two things: the minimum commitment length (often six or twelve months) and the notice period once that term ends. The “effective date” you write on the form should reflect both.
Cancelling before the minimum term is over usually triggers an early termination fee. One F45 studio defines it as 50 percent of the remaining fees owed through the end of the agreement.6F45 Fort Collins. F45 Membership Terms and Conditions Yours could be a flat amount, a different percentage, or the full remaining balance. Ask the studio manager to confirm the figure in writing before you submit.
If You Just Signed Up
The cancellation form is the wrong path if you’re inside a state cooling-off window and want a full refund. Many states give you a short period after signing (New York, for example, guarantees three days) to cancel a new gym contract under consumer protection law rather than under the studio’s own terms.7New York State Attorney General. Health Clubs and Gyms Check your state attorney general’s site for the deadline where you live, and send that cancellation in writing right away.
Medical and Relocation Cancellations
Even inside a minimum term, most state fitness-club laws let you cancel penalty-free in two situations: a qualifying medical condition, and moving too far from the studio to reasonably attend.
Medical
If a doctor determines you shouldn’t exercise for an extended period, you can typically cancel without an early termination fee. Studios and their billing partners generally want a physician’s note on official letterhead stating the doctor does not recommend you continue exercising for six months or longer. The note should show the date, your name, the practice name, and the physician’s name. It can’t be backdated to claim a refund for months already billed. Attach the note to your cancellation form.
Relocation
If you move beyond a set distance from the studio, state law often gives you the right to cancel. New York sets the threshold at 25 or more miles from any location operated by the same seller.7New York State Attorney General. Health Clubs and Gyms Other states set it lower. Expect to include proof of the new address, such as a lease or utility bill, with your form.
How to Submit It With Proof
However you deliver the form, create a record that proves the studio received it and when. Disputes almost always come down to whether the studio can claim it never got your request.
In Person
Hand the completed form to a manager during staffed hours. Ask them to sign and date a copy for your records, or take a photo of them holding the document with a visible date. Get the manager’s name.
Certified Mail
Sending your cancellation by USPS Certified Mail with a Return Receipt (PS Form 3811, the green card) creates a legally useful paper trail: the postal service records when the letter was delivered and who signed for it. In 2026 this runs about $10.48 for a one-ounce letter, combining $0.78 for First-Class postage, $5.30 for the Certified Mail fee, and $4.40 for the Return Receipt.8Simple Certified Mail. 2026 USPS Certified Mail Rates
Email or Member Portal
If your studio accepts a digital form, save the sent email with its timestamp or screenshot the “request submitted” confirmation. Follow up within a few days if you don’t get an acknowledgment. An unanswered portal submission is easy for a studio to overlook or later deny.
Confirming It Went Through
Wait for a written confirmation that spells out your last charge date and when facility access ends. If nothing arrives within a week, follow up in writing and keep a copy. A verbal “you’re all set” over the phone is worth nothing in a dispute.
Compare the confirmation against your contract. If your agreement requires 14 days’ notice and you submitted on the 1st, your final billing date shouldn’t fall two months later. Flag any mismatch immediately, in writing, before the next charge posts.
If You’re Still Being Charged
Watch your statements for at least two billing cycles after your cancellation date. If a charge appears after the membership should have ended, contact the studio first with your proof of cancellation attached. Most billing errors at the studio level are clerical, and a manager with documentation in hand can issue a refund quickly.
If the studio refuses or goes silent, dispute the charge with your credit card issuer. Under the Fair Credit Billing Act, you have 60 days from the date the billing statement containing the error is sent to dispute it in writing. Your notice must give your name and account number, identify the charge you believe is wrong, and explain why.9Office of the Law Revision Counsel. 15 U.S. Code 1666 – Correction of Billing Errors The issuer must acknowledge the dispute within 30 days and resolve it within two billing cycles (no more than 90 days). During the investigation, the issuer cannot try to collect the disputed amount or report it as delinquent.
Keep everything in one folder: the signed form or letter, the certified mail receipt or screenshot, the studio’s confirmation, and any follow-up correspondence. That file is your entire case if a chargeback or a collections claim comes later.
