How to Fill Out and Submit the GED Accommodations Request Form

To complete and submit the GED accommodations request form, create an account on GED.com, indicate during registration that you need modified testing conditions, fill out the form that matches your disability category, and upload it with your evaluator’s report through the online portal.1GED. Accommodations Under the ADAA The GED Testing Service reviews each request individually and aims to decide within 30 days of receiving a complete submission.2GED. Pick the Right Disability Category First

Before you touch the form, identify which of the four disability categories your condition falls under. Each has its own PDF with its own evaluator instructions, and submitting a form or documentation that doesn’t match the category is one of the fastest ways to get kicked back.1GED. Accommodations Under the ADAA

  • Learning and Cognitive Disorders (LCD) covers conditions such as dyslexia, dyscalculia, and intellectual disabilities.
  • Attention-Deficit/Hyperactivity Disorder (ADHD) has its own separate form because the documentation calls for specific evidence of attention-related functional limitations.
  • Psychological and Psychiatric Disorders (EPP) covers anxiety disorders, depression, bipolar disorder, PTSD, and similar conditions.
  • Physical Disabilities and Chronic Health Conditions (PCH) covers mobility impairments, chronic pain, visual or hearing impairments, diabetes, and conditions needing physical modifications to the testing environment.

Download the PDF that matches, and have your clinician read it before writing anything. The form you fill out and the evaluation you attach have to point at the same category.

What Your Evaluator’s Report Must Contain

The request lives or dies on your clinical documentation. The report has to come from a qualified professional (a licensed psychologist, physician, psychiatrist, or other specialist qualified to diagnose your condition) and must tie the diagnosis directly to the modifications you are requesting.1GED. Accommodations Under the ADAA

It must be typed on the professional’s official letterhead and include their name, title, credentials, address, and phone number.4Grossmont Union High School District. GED Test Accommodation Guidelines Beyond that, make sure the report covers:

  • A clear diagnostic statement with the appropriate DSM or ICD code, not a general description of symptoms.
  • Standardized testing results. For learning disabilities and cognitive disorders, this typically includes intelligence testing (such as the Wechsler Adult Intelligence Scale or Stanford-Binet) and achievement testing (such as the Woodcock-Johnson Tests of Achievement). The evaluator should list the specific instruments, scores, and dates.
  • A full educational and developmental history showing how the disability has affected schooling and daily functioning over time.
  • A narrative on functional limitations explaining how the disability impairs you under standard GED testing conditions and why each requested modification addresses that impairment.

Recency matters. Documentation for learning disabilities should be no more than three years old. Psychiatric and other condition documentation should be no more than one year old, because those conditions can change more quickly.4Grossmont Union High School District. Filling Out and Submitting the Form

Everything moves through GED.com. The sequence is straightforward once your documentation is ready.

  • Create your GED.com account. During registration, the system asks whether you need accommodations or modified testing conditions. Select “Yes.”5GED Testing Service. Testing Accommodations
  • Follow the system’s prompt to the GED Testing Service accommodations website, which gives you the detailed application instructions and the correct forms.2GED. Exhibit 16 – Accommodated Testing Procedure
  • Complete the Accommodations Request Form. Fill in your personal information and list the specific modifications you are requesting. The modifications you list must match what your evaluator’s report recommends; a mismatch between the two is a common reason for denial.
  • Gather your supporting materials. Convert the evaluator’s report, medical records, and any other documentation into clear digital files. PDF or JPG work best.
  • Upload the completed form and all supporting documentation through your GED.com account.1GED. Accommodations Under the ADAA

You cannot schedule an accommodated test until the review is complete, so submit well before the date you want to sit for the exam.2GED. If Your Request Is Denied

You can appeal by completing the “Request for Testing Accommodations Appeal” form, downloadable from the GED Testing Service website, or by clicking “Request an Appeal” inside the Accommodations System on your GED.com account.1GED. Accommodations Under the ADAA

Resubmitting the same paperwork that was already denied is unlikely to produce a different result. The GED Testing Service encourages you to add new supporting documentation.2GED. Exhibit 16 – Accommodated Testing Procedure Useful additions include:

  • School records showing participation in special education services
  • Individualized Education Plans (IEPs) from your school years
  • Middle school or high school transcripts
  • Any other records showing a history of academic difficulties related to your disability

Disability experts review every appeal and respond directly to you.2GED. Exhibit 16 – Accommodated Testing Procedure The GED Testing Service does not publish a specific appeal deadline, so file promptly while your documentation is still within the recency window.

When You Don’t Need the Form at All

Not every medical item requires an accommodation request. Common comfort aids and medical devices, including foam earplugs (no strings), inhalers, EpiPens, glucose tablets, braces and slings, insulin pumps, continuous glucose monitors, canes, crutches, wheelchairs, hearing aids, service animals, and medical masks, are allowed in the testing room after a visual inspection by the test center administrator, with no paperwork needed in advance. One caveat: if a device makes alert noises and cannot be silenced, you need to file a formal accommodation request for it.7PSI Exams. Pre-approved Comfort Aids
