How to Fill Out and Submit the iFLY Waiver Form

The iFLY waiver form is a liability release and assumption-of-risk agreement every participant must sign before entering the wind tunnel, and the fastest way to complete it is through the link in your booking confirmation email. You can fill it out from any device at home, or use a digital kiosk in the facility lobby when you arrive. The form asks for basic personal details, a set of health questions, and initials on several risk acknowledgments. Finish it before you show up: iFLY asks guests to arrive ten minutes before their reservation, and an unfinished waiver can cost you your flight time.

Where to Find the Waiver

Your booking confirmation email contains the direct link. Clicking it opens the form in your browser, and you can complete it well before your session. If you prefer to sign in person, every iFLY location has lobby kiosks that pull up the same form.

Doing it early is worth the few minutes. Anyone in your party who waits until the lobby eats into the arrival window and holds up the group, and if you are flying with children, each minor needs a separate signature from a parent or guardian.

Who Signs

If you are 18 or older, you sign for yourself. Participants between three and seventeen need a parent or legal guardian to sign on their behalf. iFLY will not accept a signature from an older sibling, aunt, or family friend who does not have legal guardianship.

Three is the minimum age to fly. Below that, no signature will get a child into the tunnel.

The waiver states that your electronic initials and signature are legally binding under the Electronic Signatures in Global and National Commerce Act and the Uniform Electronic Transactions Act, so the digital form carries the same weight as a paper release.

What the Form Asks

The waiver is shorter than most people expect. It collects your name, date of birth, and an electronic signature drawn with a mouse or touchscreen. The bulk of the form is health and safety disclosures, each a yes-or-no question, with some answers triggering follow-up acknowledgments you must initial before you can continue.

Health Questions

Three medical questions sit at the core of the form:

  • Neck, back, or heart problems. A yes answer asks whether a doctor has cleared you for this kind of physical activity. Without clearance, the form will not let you proceed.
  • Prior shoulder dislocation. A yes answer triggers a warning that previous dislocations significantly increase the risk of re-injury during flight. You have to acknowledge that your instructor may decline to let you fly and that you accept the risk if you proceed. iFLY recommends against flying with any history of shoulder injury.
  • Pregnancy. A yes answer ends the process. Pregnant guests cannot fly under any circumstances.

iFLY’s broader guidance is that you should not fly if you have a current or prior head, neck, back, or shoulder injury, a heart condition, or anything else that limits your ability to handle vigorous physical activity.

Risk Acknowledgments

After the health section, the form presents several statements you initial one by one. You acknowledge that you assume the risk of injury, disability, or death; that you release iFLY from all liability claims; and that you understand the waiver is a full release of your right to bring legal action. If you are signing for a minor, those acknowledgments extend to your child or ward. Skipping any initial blocks submission.

Limits the Waiver Cannot Override

Signing does not guarantee you fly. iFLY enforces a hard weight ceiling of 300 pounds with no exceptions. Between 260 and 300 pounds, staff will evaluate you at check-in to decide whether the tunnel can safely accommodate you, and there is no guarantee of clearance. High flights, the add-on where an instructor takes you higher and faster, are only offered to guests weighing 260 pounds or less.

Guests with hard casts or hard prosthetics that cannot be removed cannot fly. Soft braces and removable prosthetics are handled case by case at check-in.

iFLY also requires every participant to arrive unimpaired. If staff suspect you are under the influence of alcohol or drugs, they will not let you into the tunnel, and you will forfeit your session.

Submitting and Checking In

Once every question is answered and every acknowledgment initialed, submit the form. The system sends a confirmation email that serves as your proof of completion. Keep it accessible on your phone.

Bring a valid government-issued photo ID or passport to check-in. Staff will match your ID to the name on your waiver before issuing flight gear. For minors, the parent or guardian who signed should be present with their own ID.

Sessions start on time. If you arrive late without a completed waiver, you may miss your slot and lose the booking. Treat the ten-minute early arrival as a deadline.
