To fill out the Kershaw warranty form, go to kershaw.kaiusa.com/warranty, enter your shipping details and the knife’s model number, select a reason for service, pay the $7 return shipping fee, then print the confirmation and send it in the box with your cleaned knife to the Tualatin, Oregon facility. Turnaround for mail-in service is about two weeks once Kershaw receives it.1Kershaw. Warranty & Service
Check Eligibility Before You Fill Out the Form
Kershaw’s limited lifetime warranty covers defects in materials and workmanship for the original owner. Normal wear, misuse, and accidental damage are not covered, and any modification voids the warranty. Kershaw decides what counts as modification, improper use, or normal wear.1Kershaw. Warranty & Service
If you’ve added aftermarket scales, clips, or hardware, swap the factory parts back on before shipping. Kershaw will replace aftermarket hardware it finds with factory components and is not responsible for damage to or loss of those third-party parts.1Kershaw. Warranty & Service
Damage that falls outside the warranty isn’t automatically the end of the road. A blade replacement outside of warranty costs $25 on top of the $7 shipping fee, and you note the request on the same form.1Kershaw. Warranty & Service
The Shipping Information Section
Open the Warranty/Service form on the Kershaw site. The first section asks for your first name, last name, street address with apartment or suite if applicable, city, state or province, zip code, country, phone number, and email. Kershaw sends status updates to the email you provide, so type it carefully.1Kershaw. Warranty & Service
One address restriction: if you’re sending an automatic or butterfly knife, you cannot use a P.O. Box.1Kershaw. Warranty & Service
The Product Information Section
Next, enter the model number. Look on the back of the blade, opposite the Kershaw logo, near where the blade meets the handle. It’s a four-digit number, sometimes followed by letters.1Kershaw. Warranty & Service
The form also asks where and when you bought the knife, how many knives you’re sending, and whether the knife has sentimental value. Then you pick a primary reason for service from a dropdown:
- Sharpening only
- Blade repair or replacement
- Lock failure
- Closure problem
- Opening or closing difficulty
- Clip or screw replacement
- Weak or broken torsion bar or spring
- Loose blade
- Other
An optional text box lets you describe the problem in your own words. Keep it specific. “Blade won’t lock when fully open” tells the technician more than “knife is broken.”1Kershaw. Warranty & Service
Kershaw sharpens every knife that comes in, as a courtesy, on the plain-edge portion of the blade. Serrated sections are not factory-sharpened. If you want to keep your current edge angle or don’t want the blade touched at all, write “do not sharpen” in the Reason for Service box.1Kershaw. Warranty & Service
Paying the $7 Return Shipping Fee
Kershaw charges a flat $7 return shipping and processing fee no matter how many knives are in the box. You can enter credit card information directly on the form, or include a check or money order inside the shipping box. If you’re requesting an out-of-warranty blade replacement, add $25.1Kershaw. Submitting the form generates a confirmation. Print it. That printout goes in the box with the knife.1Kershaw. Warranty & Service
Extra Steps for Automatic and Butterfly Knives
Automatic (switchblade) and butterfly knives have their own rules. Federal law bars shipping automatic knives through the U.S. Postal Service,2Office of the Law Revision Counsel. 18 USC 1716 – Injurious Articles as Nonmailable so Kershaw will only accept these knives shipped through UPS or FedEx.3Kai USA Ltd. Automatic & Butterfly Knife Release Form
You also need to complete a separate Automatic & Butterfly Knife Release Form, available as a PDF on the warranty page. Signing it acknowledges compliance with the relevant federal statutes and any applicable state or local laws. Mail or fax the signed release to Kai USA before you ship the knife. On the online warranty form, non-Oregon residents must also check the box confirming they’ve read the automatic knife waiver.3Kai USA Ltd. Automatic & Butterfly Knife Release Form
Packing and Shipping
Clean the knife first. Kershaw requires it be free of dirt, oil, food, blood, and other residue. A dirty knife slows service down.1Kershaw. Warranty & Service
Use a sturdy cardboard box, not a padded envelope. A blade tip can puncture a mailer during sorting. Put the printed warranty form or confirmation number inside. Any shipment that arrives without a completed form or order number is sent back without service.1Kershaw. Warranty & Service

Use a carrier with tracking and consider insuring the package for the knife’s retail value. If a package disappears in transit, Kershaw can’t help recover it.
After You Submit
Once the package arrives, the intake team logs it in and adds it to the service queue. Estimated turnaround is about two weeks from the day Kershaw receives the knife, not the day you ship it, so add a few days of transit each way when you’re planning.1Kershaw. Warranty & Service
After repair, the knife goes through a quality check and ships back to the address you entered on the form. You’ll get a notification with return tracking. The $7 fee covers that return shipment.
If You’re Outside the United States or Mexico
Kai USA only accepts warranty and parts requests for knives purchased in the United States or Mexico. Canadian customers should go through Kershaw Canada at kershawcanada.com instead of shipping to Oregon, which cuts down on time, cost, and the risk of a knife being held at the border. For knives bought anywhere else, contact the authorized dealer where you originally purchased the knife.1Kershaw. Warranty & Service
