The LA Fitness cancellation form asks for your full name, membership or barcode number, mailing address, contact information, the club where you enrolled, a reason for canceling, and your signed date. Once it’s filled out, you submit it by certified mail, in person at any club, or through your online account, and it needs to reach LA Fitness at least five business days before your next billing date to stop the next charge.1LA Fitness. Membership Questions
What the Form Asks For
Pull out your membership card or log into your online account first. Your barcode number is the identifier the billing department uses to find your specific contract, and without it your request can stall.1LA Fitness. Membership Questions Enter your name exactly as it appears on the original agreement. A mismatch — a middle initial on one, none on the other — is the kind of small thing that gets a cancellation kicked back.
The fields to complete:
- Full legal name
- Membership or barcode number
- Mailing address
- Phone number and email
- The club location where you signed up
- Reason for canceling
- Signature and date
Also have your most recent billing statement handy so you know exactly when the next payment posts. That date decides whether you need to move quickly or can take your time.
Where to Send the Completed Form
LA Fitness accepts the form through three channels. Pick one and keep proof.
By Certified Mail
Send the signed form to:
LA Fitness Member Services
P.O. Box 54170
Irvine, CA 92619-4170
Use certified mail with return receipt. You get a postmarked date and a signature confirming delivery, which is your strongest evidence if the company later says nothing arrived.1LA Fitness. Membership Questions Keep the tracking number and the physical receipt. A phone call or a casual mention to staff is not a cancellation, regardless of what you’re told at the counter.
In Person at a Club
You can bring the form to any club location and hand it to the front desk.1LA Fitness. Membership Questions Ask for a signed, dated copy back before you leave. The FTC alleged in its August 2025 lawsuit that LA Fitness sometimes routed all cancellations through a single designated employee even when other staff could enroll new members.2Federal Trade Commission. FTC Sues LA Fitness for Making it Difficult for Consumers to Cancel Gym Memberships If you’re told that person isn’t in, leave the form with a dated acknowledgment and mail a backup copy to the P.O. Box the same day.
Through Your Online Account
Log in on the LA Fitness website and go to member services. The site offers a digital cancellation submission, and depending on your state of enrollment, email may also be an option.1LA Fitness. Membership Questions Screenshot the confirmation page. Save any acknowledgment email in a folder you won’t accidentally delete.
Timing: The Five-Business-Day Rule
Your form must be postmarked (for mail) or received (for in-person and online) at least five business days before your next billing date. If it is, you should not be charged again.1LA Fitness. Membership Questions
Submit inside that window and the billing system may run one more payment. LA Fitness says it will refund that charge, but you have to follow up if the refund doesn’t show.1LA Fitness. Membership Questions Payment files often go to banks a few days before the actual draft, which is why cutting it close backfires. Give yourself two weeks of cushion.
One other timing point: the annual fee is billed 14 days after enrollment and every year after that, at $69 at some locations.3LA Fitness. Online Membership – Rate The company’s FAQ doesn’t say whether that fee is prorated or refunded on cancellation. If your annual billing date is close, getting the form in before it hits could save you the charge.
If You’re Still Inside an Initial Term
Most LA Fitness memberships are month-to-month with no minimum commitment and no penalty for leaving.1LA Fitness. Membership Questions If yours is, the form ends things after the current billing cycle and you owe nothing else.
If your agreement has an initial term of three months or more, check where you stand:
- Past the initial term: submit the form like any month-to-month member.
- Still inside the term: you can submit now, but the cancellation only takes effect when the term ends, and you keep paying until then.
- Leaving before the term ends: an early termination fee applies. LA Fitness doesn’t publish the amount because it varies by agreement, so read the termination clause on your contract or ask your local club.1LA Fitness. Membership Questions
What Happens After You Submit
You keep your gym access for whatever time is already paid for. Most LA Fitness enrollments include prepaid last month’s dues, and those get applied to the month after your final recurring payment. Membership expires at the end of that prepaid month.1LA Fitness. Membership Questions
A concrete example: your final recurring draft covers October, your prepaid last month covers November, and your access ends November 30.
Watch your bank or card statements for at least two billing cycles after you submit. If charges continue past when the cancellation should have taken effect, contact member services with your proof. The FTC alleged that some members who tried to block charges through their bank were then rebilled under new account numbers.2Federal Trade Commission. FTC Sues LA Fitness for Making it Difficult for Consumers to Cancel Gym Memberships Unresolved balances can go to a third-party collection agency, so keep the certified mail receipt or your online confirmation until every charge stops.
Personal Training Uses a Different Process
The gym cancellation form doesn’t end a personal training agreement. Those are separate contracts, and the online portal won’t process a training cancellation — the website directs you to your local club instead.4LA Fitness. Cancel Membership The initial-term rules are the same: free to cancel after the term, wait it out if you’re still in it, pay a termination fee to leave early.1LA Fitness. Membership Questions
You can cancel training and keep your gym membership. The FTC noted that LA Fitness failed to clearly tell members these two agreements were independent.2Federal Trade Commission. FTC Sues LA Fitness for Making it Difficult for Consumers to Cancel Gym Memberships
If You Hit Obstacles
The FTC’s August 2025 lawsuit alleged that LA Fitness violated the FTC Act and the Restore Online Shoppers’ Confidence Act by forcing members through a complicated cancellation process, requiring in-person visits or certified mail from people who enrolled online, and training staff to reject phone and email requests.2Federal Trade Commission. FTC Sues LA Fitness for Making it Difficult for Consumers to Cancel Gym Memberships The case is ongoing, and the cancellation process may change as a result.
If you’re told only one specific person can process your form, pressured to stay, or told your cancellation was never received despite having proof, you can file a complaint with the FTC at ftc.gov. In the meantime, submit through a second channel with documentation, and hold on to every receipt until your last charge clears.
