The Life Time parental consent form, officially the Sponsoring Member Permission Agreement, is an online waiver a parent or legal guardian submits before a minor child visits a Life Time club with an adult who isn’t the child’s parent or guardian. You complete it on Life Time’s Formstack page, cover up to three children per submission, and choose a start date; the agreement then stays valid for six months.1Life Time. Sponsoring Member Permission Agreement It has to be on file before the child walks through the door.2Life Time. Guest and Club Policies
When You Need to Submit It
The form is required any time a minor guest visits with a supervising adult who is not the child’s parent or legal guardian. That covers both age brackets Life Time uses: children under 12 and teens 12 through 17. Either way, the parent or guardian submits the online consent before the visit.2Life Time. Guest and Club Policies
The supervising adult has to be at least 18, has to stay on-site the whole visit, and has to sign the electronic guest register at check-in, even if that adult isn’t planning to work out.2Life Time. Guest and Club Policies
A related but separate document, the Parent/Guardian Consent Form, is used when a grandparent wants to add grandchildren ages 3 months through 13 to their membership. The non-member parent can complete that one online or print it and drop it off at the grandparent’s club.3Life Time. Help and FAQs
Where to Find the Form
The direct link is lifetime.formstack.com/forms/sponsoringmemberform. It opens in any browser, and you don’t need to log in or hold a Life Time membership to use it. Every field is on one page, and you sign digitally right there.1Life Time. Sponsoring Member Permission Agreement
A printable PDF exists for parents who would rather fill it out on paper. You can download it, sign by hand, and hand it in at the club’s front desk. Some Life Time club pages for kids programming link straight to the PDF.4Life Time. Parental Guardian Consent Form PDF
Information to Have Ready
The form is short, but having everything in front of you keeps it to a couple of minutes. Each section asks for the following:1Life Time. Sponsoring Member Permission Agreement
- Parent or legal guardian details: your first name, last name, email address, and phone number.
- Supervising adult details: the first name, last name, and email address of the member or adult guest who will be with your child at the club.
- Club and dates: the specific Life Time location, chosen from a dropdown; the date of visit; and the start and expiration dates of the agreement. The agreement type is preset to a six-month window.
- Child information: each child’s first name, last name, date of birth, and age. Up to three children fit on one form. For a fourth child, submit a second form.
- Rock wall usage: a yes-or-no question on whether the children will use the climbing wall. Selecting “yes” adds a climbing waiver with its own terms checkbox.
- Acknowledgments and signature: a terms-and-conditions checkbox and a digital signature field for the parent or legal guardian.
You don’t need your child’s membership number or a guest pass number. The form connects the child to the supervising adult by name and email, not by account.
Signing and Submitting
The online version accepts a digital signature you draw or type on the Formstack page. Check the terms box, sign, and submit. There’s no separate upload; it goes directly to Life Time’s system.
If you use the PDF, sign it by hand and bring it to the front desk of the club your child will visit. The supervising adult can deliver the signed paper form for you. Either way, the agreement has to be on file before your child checks in.
How Long the Agreement Lasts
The online Sponsoring Member Permission Agreement is valid for six months from the start date you pick on the form.1Life Time. Sponsoring Member Permission Agreement When it expires you have to submit a new one. It does not renew on its own.
The separate PDF Parental/Guardian Consent Form used to add a household member to someone else’s membership works differently. That authorization stays active until the parent contacts Life Time to remove it or the primary member takes the child off the membership.4Life Time. Parental Guardian Consent Form PDF
What Your Child Can Actually Do After You Sign
Submitting the form gets your child in the door with the supervising adult. What happens next depends on the child’s age, and the accompanying adult carries real responsibilities.
Under 12
Children under 12 have to stay with the supervising adult at all times, unless a parent or legal guardian personally checks them into the Child Center and stays on-site. Guests under 12 can use the Child Center, indoor and outdoor aquatics during designated hours, and racquet courts or gymnasiums during family hours.2Life Time. Guest and Club Policies
Eleven-year-olds have a narrow exception to use the fitness floor and free weight area, but only under the direct supervision of a parent, legal guardian, or authorized adult. Anyone under 11 cannot use the fitness floor at all.2Life Time. Guest and Club Policies
Ages 12 Through 17
Teens 12 and up get much broader access: cardio and resistance training areas, group fitness classes, racquetball and squash, LifeSpa, LifeCafe, the climbing wall, the gymnasium, and aquatics during scheduled programming or open hours. The accompanying adult still has to be at check-in and stay on-site the entire visit.2Life Time. Guest and Club Policies
For 12- and 13-year-olds, direct fitness-floor supervision is recommended but not required. Once a junior member turns 14, Life Time no longer classifies them as a junior member.2Life Time. Guest and Club Policies
Child Center Access
One boundary to know before you assume the form covers it: use of the Child Center by guests accompanied by someone other than their parent or legal guardian is subject to applicable law. In practice, a parent or legal guardian has to be the one who checks the child in, and that parent has to stay on-site. A supervising adult who is not the child’s parent generally cannot drop the child off at Kids Academy.2Life Time. Guest and Club Policies
Climbing Wall
If you marked “yes” for the rock wall, the separate climbing waiver is built into your submission. Climbers must be at least five to enter the climbing area. No climber under 12 can top-rope belay, lead belay, or lead climb. A parent or legal guardian has to directly and actively supervise minor children in the climbing area at all times, unless the child is in a Life Time-supervised climbing program. Direct and active supervision means visually watching and physically attending to the child the whole time, including double-checking equipment before climbing. Every climber new to Life Time also has to complete a staff-led orientation, including the auto-belay system; manual belaying requires a separate certification.1Life Time. What Else Your Signature Authorizes
Life Time’s kids participation agreement includes a medical release. By signing, you authorize Life Time to make decisions about medical and survival procedures for your child, including arranging emergency transportation, if you or another parent or guardian can’t be reached. You also agree to handle hospital admittance arrangements if you’re available during an emergency.5Life Time. Participation Agreement It’s standard language for youth activity waivers, but read it before you sign.
A separate document, the Image and Likeness Release, often comes up during the same enrollment process. When a parent or guardian signs it, Life Time gets a non-exclusive, worldwide, perpetual, royalty-free right to use photos or video of the child in marketing and social media, including corporate and employee social channels, without additional consent or compensation.6Life Time. Image and Likeness Release It’s not the same document as the parental consent form. If you don’t want to grant those image rights, ask club staff whether you can opt out of the image release while still completing the parental consent agreement.
