How to Fill Out and Submit the Melaleuca Return Order Form

The Melaleuca Return Order Form is a one-page PDF you download from Melaleuca’s website, fill out, and pack inside the box with the products you’re sending back.¹ If you can’t get to the PDF, call customer service at (208) 534-3000 and ask for one. The form covers all three outcomes under the 90-day satisfaction guarantee: a refund to your original payment method, a credit on your Melaleuca account, or a replacement product.²

You have 90 days from the purchase date to return the unused portion of any product. The same deadline applies whether you want cash back, an account credit, or an exchange.²

Filling Out the Form

At the top, write your name, your Customer Member Number, and a daytime phone number. Your member number is on your purchase receipt, on the packing slip, and inside your online account. Enter it exactly as shown.¹

Then check one of three boxes for how you want the return handled: Credit My Account, Refund Purchase Price, or Replace the Product. Pick one. This is the choice that decides what happens after the warehouse opens your box.¹

The middle table is where you list each item you’re returning. For every product, write the item number (from your receipt or packing slip), the quantity, and the product name. Then check the reason: Damaged, Wrong Item Sent, Poor Results, or Other. If you check Other, write a short note in the space provided.¹

If you asked for a replacement at the top, a separate table below lets you list the item numbers, quantities, and names of the products you want instead. Two things to know before you commit to that option: exchange products ship with your next monthly order unless you request otherwise, and if the replacements cost more than what you sent back, Melaleuca charges the difference to the payment method on your account.¹

Where to Ship the Package

This is the step that trips people up. The package has to go to the warehouse your order originally shipped from, not to Melaleuca’s general corporate address. Check your packing slip or order confirmation for the shipping origin and use the matching address:¹

Put the completed form inside the box along with a copy of your purchase receipt.² You pay for return shipping; Melaleuca does not reimburse it. Use a carrier with tracking so you have proof of delivery, and pack the items well enough that they won’t be damaged in transit.

What You’ll Get Back

Once the warehouse processes the package, your refund goes to the original payment method or your account credit balance updates, depending on the box you checked. Refunds cover the product price plus any sales tax you paid. Shipping and handling from the original order are not refundable, no matter which option you chose.²

To check the status of a return or your credit balance, call (208) 534-3000. If you already received an account credit and later decide you want cash instead, request the switch in writing by faxing (888) 528-2090 or mailing the request to 4609 West 65th South, Idaho Falls, ID 83402.²

Limits to Know Before You Send

A few situations don’t qualify for a cash refund. Products from a Home Conversion Pack, Value Pack, or other special bundle priced below the standard member price can only be exchanged unless you return the entire pack together. Anything bought with the $100 in free Loyalty Shopping Dollars new members receive is also exchange-only.²

Melaleuca also tracks return volume. If you return more than half the product points you purchased over any rolling three-month period, future returns become eligible only for exchange credit, and your account can be canceled.²
