How to Fill Out and Submit the Optimum Authorized User Form

The Optimum authorized user form is a one-page PDF the primary account holder uses to add or remove someone else’s permission to manage the account. Download it from Optimum’s website, fill in your account details and the authorized user’s full legal name, sign and date it, and send it in by mail, fax, or in person at an Optimum store. The authorized user cannot complete the form themselves.1Optimum. Authorized User Form

Add/Remove an Authorized User to/from My Business Account One form covers residential and business accounts, with separate sections for each and checkboxes for either adding or removing a user.

What to Have Ready Before You Start

Gather these items first:

  • Your Optimum account number, which appears at the top of each page of your monthly bill and at the top of the page when you sign in at optimum.net/pay-bill.3Optimum Mobile. Where Can I Find My Optimum Internet Account Number
  • Your account PIN. If you’ve forgotten it, reset it through the MyMobile portal under account settings.4Optimum Mobile. How Do I Change My Account PIN
  • The phone number and email address tied to the account.
  • Your full service address: street, city, state, and ZIP.
  • The authorized user’s full legal name.

The account holder’s signature and the date are both required, and an unsigned form will not be processed. The authorized user form does not need to be notarized; notarization applies only to Optimum’s separate business name change and service takeover form.5Optimum. Filling Out the Residential Section

Start with the account information block at the top: date, account number, phone number, and email address. Below that, enter the full street address where you receive service.

In the residential account section, put your own name in the “Account Holder Name” field, not the authorized user’s. Check “Add Authorized User” or “Remove Authorized User,” then write the authorized user’s full legal name on the designated line.1Optimum. Authorized User Form

Residential accounts allow only one authorized user at a time. To swap one person for another, you’ll need to submit a removal and then a separate add, or call Optimum to handle both in one go.

Sign and date the authorization block at the bottom and print your name. The form states explicitly that the account holder, not the authorized user, must complete and sign it.

Filling Out the Business Section

Business accounts use a separate section on the same form. The process is similar, but a business account can designate up to five authorized users.5Optimum. How to Submit the Completed Form

Three options:

There is no online upload portal for the form itself; mail and fax are the only remote options listed on it. Optimum does not publish a guaranteed turnaround time, and a confirmation goes to the primary account holder once the change is processed.

What an Authorized User Can Do

Once added, the authorized user has permission to access the account and make changes to TV, phone, and internet service levels.1Optimum. Authorized User Form On business accounts, that authority extends to making payments, reviewing billing history and balances, and adding or removing certain services and equipment.2Optimum Business. Add/Remove an Authorized User to/from My Business Account They can call Optimum, walk into a store, or manage service details without the primary account holder being present.

What an Authorized User Cannot Do

The authority has clear limits. An authorized user cannot make changes that would cause a complete loss of a primary service, meaning they cannot cancel the account or terminate all services. They also cannot add or remove other authorized users.2Optimum Business. Add/Remove an Authorized User to/from My Business Account Changing the primary account holder’s name or transferring ownership requires a separate name change or service takeover form, which the authorized user cannot initiate.

The form does not create financial liability for the person being added. It makes no mention of credit reporting or financial responsibility, and the primary account holder remains the person legally responsible for the balance.

Removing an Authorized User

Removal uses the same form. Check the “Remove Authorized User” box, fill in the authorized user’s name, sign and date it, and submit it through any of the same channels.1Optimum. Authorized User Form For business accounts, you can also call an Optimum representative to handle the removal by phone.2Optimum Business. Add/Remove an Authorized User to/from My Business Account

Only the primary account holder can remove an authorized user; the user cannot remove themselves. If you need to revoke access quickly, calling Optimum or visiting a store will be faster than mailing the form.

If the Account Holder Has Died

Authorized user status does not transfer the account when the primary holder dies. Optimum uses a separate Residential Name Change / Account Transfer form for that situation, which asks for a Social Security number or a copy of a government-issued photo ID and makes the new account holder responsible for all equipment and outstanding balances as of the transfer date.9Optimum.
