How to Fill Out and Submit the Optum Provider Change Form

The Optum provider change form is a PDF that network clinicians and groups use to report updates to their demographic, financial, and practice information. You download it from Optum, fill in only the sections that apply to your change, attach any required documents, and send it to pdmops@optum.com or fax it to 1-855-202-4313.1Optum. What the Form Covers

The change form handles the updates that affect how Optum identifies, credentials, and pays your practice. The most common ones:

  • A new practice name after a rebrand or reorganization, with its effective date.1Optum. Optum Physician Provider Change Form
  • A new Tax Identification Number following a merger, acquisition, or restructuring. A current W-9 must be attached.1Optum. Optum Physician Provider Change Form
  • Address or contact changes, including moves, added locations, and new phone numbers.
  • Adding or removing a clinician from the group.
  • Banking changes for Electronic Funds Transfer, so payments don’t land in a closed account.

The form lets you flag whether a change applies to every clinician under one TIN or only to specific providers in the group.3Optum. Optum Provider Change Form

What to Gather Before You Start

Incomplete submissions get rejected. The form itself states that Optum cannot process incomplete forms.1Optum. Optum Physician Provider Change Form Pull these together first:

  • The individual NPI for each affected clinician, and the Type 2 organizational NPI if the change involves the practice entity. The NPI is the 10-digit identifier required of covered healthcare providers under HIPAA.4Centers for Medicare & Medicaid Services. National Provider Identifier Standard
  • Both the current and new nine-digit TIN if you’re reporting a tax ID change, plus a completed W-9.1Optum. Optum Physician Provider Change Form
  • Your CAQH provider ID.
  • A copy of the current state medical license, typically asked for when a clinician is joining a new group or changing locations.
  • The malpractice insurance face sheet showing coverage limits and effective dates.
  • For EFT updates, a voided check or a letter on the bank’s letterhead confirming account and routing numbers.

Verify every identifier against your original enrollment paperwork rather than from memory. Optum can’t pay correctly if the NPI, TIN, or banking information on file is wrong,5Optum. Updating Your Practice Information and a single transposed digit is enough to get a submission bounced back.

Portal or PDF: Which Route to Use

Provider Express is the faster path for routine demographic updates. Log in with your One Healthcare ID, choose “My Practice Info” from the top menu, and pick the right category: Clinician Information, Practice Information, Licenses and IDs, or Directory Attestation. Most portal changes take effect in real time, and directory listings refresh overnight.2Optum. Provider and Practice Demographic Updates

The PDF form is the right tool when the change involves financial restructuring (new TIN, new banking details) or when you need to attach supporting documents like a W-9, license copy, or malpractice face sheet. You can download it from the Optum support site at business.optum.com or from the Provider Express forms library. Optum keeps separate forms for specific situations, including a Clinician Add/Change Form for contracted clinicians and an Agency Roster Update Form for community mental health center agencies, and some states have their own versions, so confirm you’re on the right form for your state and program before filling anything in.6Optum. Filling Out the PDF Correctly

The form separates individual clinician details from group-level practice information. Check the box at the top that identifies the type of change, then complete only the sections that apply. Leave the rest blank. Filling in fields that aren’t relevant to your change just slows the processing team down.

For a practice name change, enter the new name, the effective date, and whether the change applies to all practitioners under the TIN or only to specific clinicians.3Optum. Optum Provider Change Form For a new TIN, write the number clearly and attach the W-9; submissions missing the W-9 get returned. For an address change, give the full street address, suite number, city, state, and ZIP for every affected location.

Before you send it, read every identifier against the source document one more time.

Where to Send It

Submit the completed form, with all attachments, one of two ways:

  • Email to pdmops@optum.com
  • Fax to 1-855-202-4313

Both are printed on the form.1Optum. Optum Physician Provider Change Form Save a copy of the completed form, every attachment, and your fax confirmation or sent-email receipt. If anyone later disputes when you reported the change, that record is your proof.

After You Submit

Simple portal updates show up almost immediately, with directory listings refreshing the next day.2Optum. Provider and Practice Demographic Updates PDF submissions take longer because they’re processed manually, and changes that involve credentialing verification, like adding a new clinician, take longer still.

Watch your claims during the wait. If denials or payment holds appear right after you submit, the usual cause is a mismatch between the old information still on file and the new details on your claims. Call Optum provider services if denials start piling up rather than waiting the change out. Once you get confirmation the update is live, check the public provider directory to confirm your name, address, specialty, and contact information look right.

Why Timely Filing Matters

Federal law requires healthcare providers to have business processes in place for timely reporting of directory information to health plans. Under 42 U.S.C. ยง 300gg-139, providers must submit updated information when they begin or end a network agreement, when there are material changes to their directory data, and at any other time a plan requests it.7Office of the Law Revision Counsel. 42 USC 300gg-139 – Provider Requirements to Protect Patients and Improve the Accuracy of Provider Directory Information

The practical consequences stack up quickly. Patients get sent to the wrong address, referrals go to clinicians who have left, and claims are processed against the wrong TIN. Optum says outdated information is one of the biggest reasons for delayed claim payments and that your directory status can be affected if you don’t maintain and attest to your records.5Optum.
