The Personify Health biometric screening form is a personalized PDF you download from your member account, hand to a licensed healthcare provider to fill in with your blood pressure, cholesterol, glucose, height, weight, and waist measurements, and then submit back to Personify Health by upload or fax to earn your employer wellness reward. Each form carries a unique Member ID and barcode tied to your account, so the copy you use has to be your own.
Download Your Personalized Form First
The form must come from your own Personify Health account because the barcode is what matches the results to your profile.1Personify Health. How to Submit a Biometric Screening Form A blank version from the internet or a printout from someone else’s login will not process.
Log in on the Personify Health website or open the mobile app, go to the Home or Programs tab, and find the biometric screening activity. There you will see a link to download or print the PDF. Your Member ID also appears on the digital health card inside the app. Print on clean white paper. Do not write on the barcode and do not fold the page across it, because the processing center scans it automatically.
Book the Right Kind of Appointment
Most employers let you complete the screening three ways: during a regular physical with your own doctor, at an employer-sponsored on-site event, or at a partner lab. Not every employer offers all three, so check your program details in Personify Health.
Fasting depends on the panel your employer selected. Some panels require a nine-to-twelve-hour fast (nothing but water) to get accurate fasting glucose, triglyceride, and LDL readings. Others accept non-fasting values. Your program materials or the form itself will say which applies. When in doubt, fasting avoids a repeat blood draw.
Bring the printed form and a photo ID. If you are seeing your own doctor, call the office ahead and tell them you need specific biometric values recorded on a wellness form, so they plan for the blood draw and measurements during the visit. Under the Affordable Care Act, most employer health plans cover preventive screenings with no out-of-pocket cost, so a biometric screening done during a routine physical typically has no separate copay. Any additional tests your doctor orders beyond what the wellness form requires may be billed separately.
What Your Provider Needs to Record
The form has a designated box for each clinical value. Your provider fills in:
- Blood pressure, systolic and diastolic, in mmHg
- Total cholesterol and HDL cholesterol, in mg/dL
- Glucose, fasting or non-fasting depending on your employer’s requirement, in mg/dL
- Height and weight, which Personify Health uses to calculate BMI
- Waist circumference in inches, measured at the narrowest point above the hip bones
Some programs also collect triglycerides and LDL cholesterol, though those are less universal. The form itself shows which fields your plan requires.
Only a physician, physician assistant, or nurse practitioner can sign and date the form. The date of service has to reflect the actual day the measurements were taken, not the day the paperwork was finished. Ask the provider to use black or blue ink and print legibly in every box. A missing signature or a blank field is the fastest way to have the form kicked back.
Submitting the Completed Form
You have two main submission methods.1Personify Health. How to Submit a Biometric Screening Form
The faster option is to upload through your account. Log in on the website or app, go to the biometric screening activity, and use the upload tool to submit a scanned PDF or a high-resolution photo. The mobile app has a camera capture feature that aligns the form automatically. Confirm that the whole page is visible, the lighting is even, and the barcode is not obscured before you send it.
The other option is fax. Send the completed form to 508-302-0055, the dedicated number listed on the form header.2Personify Health. How to Submit Your PCP Form Through Personify Health Account Keep the transmission confirmation page as proof of delivery. Some programs also accept mailed copies at an address printed on the form, but mailing is slowest and risks the form getting lost. If you mail it, use a trackable service.
Processing Time and Tracking Your Reward
Expect roughly seven to ten business days from the day Personify Health receives the form for the reward to appear in your account.2Personify Health. How to Submit Your PCP Form Through Personify Health Account Track status in the Rewards or Stats section of your dashboard. An email notification usually arrives once the form enters the review queue.
Once the form processes, your biometric values populate your wellness profile and any associated incentive points or premium credits post to your account. Watch your program’s annual deadline. Most employers set a cutoff for the plan year, and forms received after that date will not count toward the current cycle.
Common Reasons Forms Get Rejected
Most rejections come from a short list of preventable problems:
- Wrong form. A generic doctor’s office printout, a hospital discharge summary, or a PDF downloaded from a different person’s account will not match in the system. Your personalized barcode is what makes it work.
- Missing provider signature. Without a licensed provider’s signature, the form is treated as unverified.
- Blank or incomplete fields. Every required measurement has to be filled in. If the provider skips waist circumference or forgets to mark whether glucose was fasting or non-fasting, the form gets flagged.
- Illegible handwriting or poor image quality. Automated scanning cannot read smeared ink or a blurry photo. If uploading a picture, retake it if any value looks unclear on your screen.
- Date-of-service issues. The screening date must fall within the program’s eligible window for the current plan year.
When a form is rejected, Personify Health sends an alert through the platform explaining what went wrong. Correct the problem, which sometimes means going back to your provider for a new signature or a legible rewrite, and resubmit before the program deadline.
If You Cannot Meet a Required Health Number
Some wellness programs tie part of the incentive to actually hitting certain numbers, such as a blood pressure under 140/90 or a BMI below 30. Federal rules require these “health-contingent” programs to offer a reasonable alternative if you do not meet the target, and the plan must disclose the availability of that alternative in all program materials.3Department of Labor. HIPAA and the Affordable Care Act Wellness Program Requirements
A reasonable alternative might be a health education course, a nutrition plan, or working with a health coach for a set period. You cannot be charged for the alternative; if it involves a class or membership fee, the plan covers it. You can also ask to follow your own doctor’s recommendations instead of the plan’s suggested alternative, and the plan must allow that.3Department of Labor. Who Sees Your Results
When the wellness program runs through a group health plan, the biometric data you submit is protected health information under HIPAA, and Personify Health, as a business associate of the plan, must follow the same privacy and security rules that apply to your insurer. Your employer’s HR department does not receive your individual results unless the plan documents specifically authorize it, and even then only designated plan administrators with HIPAA training can access the data. If the program is offered directly by the employer outside a group health plan, HIPAA does not apply, though other federal and state privacy laws may.4U.S. Department of Health and Human Services. HIPAA Privacy and Security and Workplace Wellness Programs Ask your benefits administrator which structure your program uses if you are unsure.
Separately, the Genetic Information Nondiscrimination Act bars your employer from requesting family medical history through a wellness program unless participation is truly voluntary and specific safeguards are in place. An employer can never use genetic information to make employment decisions.5U.S. Equal Employment Opportunity Commission. Genetic Information Discrimination If your form or health risk assessment asks about family history, answering should be optional.
Keep your own copy of the completed form and the submission confirmation. If a reward goes missing or a value is disputed later, you will want the paperwork on hand.
