To file a Ply Gem warranty claim form, first identify which product line you own, because the form and the submission address are different for each. Siding claims use a downloadable PDF submitted by email or postal mail. Window and patio door claims go through a separate online portal hosted by Cornerstone Building Brands. Mastic and Variform siding each have their own brand-specific submission pages. Sending a siding claim through the window portal, or the reverse, stalls the case before anyone reviews it.
Find the Right Form for Your Product
Check your original invoice, product packaging, or the label on a leftover piece before you start. Ply Gem operates several product lines, and each has its own claim path.

- Simonton windows: a Ply Gem brand routed through the same Cornerstone Building Brands portal used for other window claims.4Simonton Windows. Warranty Support and Information
If you can’t tell which brand you own, the stamp on the back of a siding panel, the spacer bar between window glass panes, or the frame edge of a window usually carries a manufacturer name and production code. Your original purchase invoice should also identify the brand.
What to Have Ready Before You Open the Form
Gather everything first. Going back and forth mid-claim is how fields get left blank, and incomplete submissions are the easiest for a claims department to set aside.
- Full name, phone number, email, and the street address where the product is installed.
- The original installation date. Warranty duration and any prorated reimbursement are calculated from this date.
- Proof of purchase: original sales receipt, invoice, or bank statement showing payment.
- Product identification. For siding, the profile name, color, and any batch or lot numbers on the back of a leftover piece. For windows, the label on the glass spacer bar or frame, which often includes an AAMA or IGCC certification number Ply Gem can use to trace the manufacturing run.
- A written description of the problem. Note what’s failing, when you first noticed it, and how widespread the damage is. “Siding is buckling on the south-facing wall, three panels affected” is far more useful than “siding looks bad.”
Photo Requirements
The siding claim packet spells out exactly what photos Ply Gem wants, and skipping any of them reliably delays the claim. Photos need to be clear, in color, and high quality.5Ply Gem. Consumer Damage Report
- One photo of the entire structure, similar to a real estate listing shot.
- One full-view photo from each corner of the structure (front, right, back, left), each labeled with the direction shown.
- Close-up photos of the damage, also labeled by direction.
- A photo showing your house number on the property.
If you print photos for a mailed claim, use photo-quality paper and write the direction on the back of each one. Ply Gem does not accept instant-developing photos like Polaroids. If you’re submitting digitally, you typically email the photos after receiving a claim number and specific email instructions from the claims department.5Ply Gem. Consumer Damage Report Window claims filed through the online portal have their own upload interface. The same principles apply: show the whole window, get close-ups of the defect, and make the images sharp enough that someone who has never seen your house can tell exactly where and how bad the problem is.

Windows and Patio Doors
Window and patio door claims run through the online portal at prtwarrantyclaims.cornerstonebuildingbrands.com. You can reach the portal through the link on plygem.com’s window warranty page or by calling the Ply Gem Warranty Claims Department at (800) 635-6253.1Ply Gem. Submit a Warranty Claim Ply Gem’s warranty documents recommend contacting the claims department promptly after discovering a covered issue, so the phone line is worth using if you want to get the process moving.
What Happens After You Submit
Expect a review period before anyone contacts you. A claims representative will reach out to discuss next steps or request more information. If your photos or documentation are incomplete, this is where things stall, and the clock effectively pauses until you provide what’s missing.
Ply Gem reserves the right to send a technician to physically inspect the property before approving a claim. The inspection verifies that the damage exists as described and determines whether it stems from a manufacturing defect rather than improper installation, physical abuse, or environmental factors. Refusing the inspection gives Ply Gem grounds to deny the claim.
If Ply Gem approves the claim, the company chooses the remedy. You don’t get to pick between repair, replacement, or refund. The warranty language across product lines is consistent: the remedy is “solely up to” Ply Gem.6Ply Gem. Ply Gem Performance Collection Vinyl Siding Warranty
Reasons Claims Get Denied
A few exclusions come up often enough that they’re worth checking before you file.
Condensation on the interior surface of window glass is caused by indoor humidity, not a product defect, and is not covered.7Ply Gem. Warranty Information for Windows and Patio Doors Condensation between the two panes of an insulated glass unit is different: that usually indicates a failed seal and may be covered.
Improper installation voids coverage. Ply Gem’s window warranty page notes that “even the highest-rated impact window or door can fail if it is not installed correctly.”7Ply Gem. Warranty Information for Windows and Patio Doors Keep your installation records and contractor information in case an inspector suspects an installation issue.
Cleaning damage and neglected maintenance are excluded. The Mastic warranty requires annual cleaning and warns against abrasive cleaners, strong solvents, and excessive scrubbing, and mildew from neglect is not a covered defect.8Ply Gem. Mastic Warranty and Care Information
Musket Brown and Black siding used on sidewalls are excluded from the fade warranty entirely.8Ply Gem. Mastic Warranty and Care Information Ply Gem also does not warrant against cracked window glass because the cause is “practically impossible to determine” after the fact.7Ply Gem. Hail Damage Requires an Insurance Claim First
You cannot go straight to Ply Gem for hail damage. The warranty requires you to file with your homeowner’s insurance first and exhaust that coverage. Only after insurance has paid out will Ply Gem consider product costs that exceed your total insurance reimbursement, not counting your deductible, which stays your responsibility. Ply Gem’s obligation is capped at the original purchase price of the damaged material.6Ply Gem. Ply Gem Performance Collection Vinyl Siding Warranty Keep copies of your insurance claim and settlement, because Ply Gem will want to see what was paid and what gap remains.
Who Pays for Labor
For windows and patio doors, the warranty is unambiguous: Ply Gem does not cover labor of any kind. Removing the defective window and installing the replacement is on you.9Ply Gem. Warranty
Performance Collection siding is more nuanced. The general terms state that Ply Gem has “no responsibility for any costs associated with the labor required to remove, replace or install any Product.” An asterisked exception follows: if Ply Gem elects to repair or replace under warranty, the company “will also cover the reasonable labor costs, as determined by us for the applicable area.”6Ply Gem. Ply Gem Performance Collection Vinyl Siding Warranty The key phrase is “as determined by us.” Ply Gem defines the reasonable rate, and if your contractor charges more, you pay the difference.
Transferring the Warranty to a New Owner
If you just bought a home with Ply Gem products already installed, the warranty can transfer, but you have to act within 30 days of the ownership change. Ply Gem provides a transfer form on its website.10Ply Gem. Transfer Your Warranty You can also initiate the transfer by sending written notice to the Warranty Services Department at 303 W Major Street, Kearney, MO 64060.
The terms change once transferred. A limited lifetime warranty that covered the original owner for as long as they lived in the home becomes a maximum of 50 years from the original installation date, prorated on a declining schedule.11Ply Gem. Homeowner Warranty Missing the 30-day window doesn’t automatically strip you of recourse, but it gives Ply Gem grounds to deny a future claim. If you’re buying a home and the seller mentions Ply Gem products, handle the transfer at closing.
