To claim State Farm’s impact-resistant roof discount, you complete the State Farm roofing installation certification form with your contractor after the new roof goes on, then submit it to your agent. You fill in the property and product details, your roofer signs a certification that the product and installation meet State Farm’s requirements, and the discount is applied to the wind and hail portion of your premium once the insurer verifies everything. One thing to know before you sign: submitting the form is treated as your acceptance of a cosmetic damage exclusion on your policy.1State Farm. Where to Get the Form
The form is a single-page PDF on State Farm’s website. Go to the homeowner insurance discounts page at statefarm.com, find the roofing discount section, and click the link for the Roofing Installation Information and Certification Form.2State Farm. Homeowner Insurance Discounts Your local agent can also send you a copy. Print it before installation starts so your roofer can see which fields they’ll need to fill in and sign while they’re still on-site.
What Qualifies Before You Fill Anything In
Confirm three things before your roofer starts, because a mistake here means no discount even if the paperwork is perfect.
The Product Must Be on State Farm’s List
Only products on State Farm’s Qualifying Roofing Products Listing are eligible. The list is organized by material: asphalt composition, clay tile, concrete tile, metal, slate, and alternative materials.3State Farm. Roofing Premium Credits Product Listing Ask your agent to check the current list against the exact product you’re considering. Impact ratings vary across a manufacturer’s own product lines, so brand name alone won’t tell you whether something qualifies.
Class 3 or Class 4 Only
The form accepts products tested under UL 2218 or FM 4473, but the checkboxes cover Class 3 and Class 4 only.4State Farm. Roofing Installation Information and Certification Class 1 and Class 2 products don’t qualify. UL 2218 is common for asphalt shingles; FM 4473 covers rigid materials like metal panels, slate, and wood shakes. Both standards are treated equally on the form.
Installation Rules
Three installation requirements can quietly kill your discount:
- Full coverage. The impact-resistant material has to cover the entire roof, including hips, ridges, and ridge vent systems. Hip and ridge material must be “of like kind and quality” to the main approved product.4State Farm. Roofing Installation Information and Certification
- No overlay. The new roof cannot be installed over existing material. A full tear-off down to the deck is required. Qualifying metal roofs are the only exception.1State Farm. Roofing Installation Information and Certification for Reduction in Residential Insurance Premiums
- No existing damage. The contractor has to certify the roof is free of defects or damage, including hail damage, at the time of installation.
How to Fill Out the Form
The form has three parts on one page: your information, product details, and the contractor’s certification.
Homeowner Section
Enter the property owner’s name, home and office phone numbers, the address of the home where the roof was installed, and your State Farm policy number. Check the policy number against your declarations page. A transposed digit slows everything down.
Product Details
Identify the exact product: manufacturer, brand name, year manufactured, product color, and installation date. Check the box for the UL 2218 or FM 4473 classification, Class 3 or Class 4.4State Farm. Roofing Installation Information and Certification Pull these details from the product packaging label, not from a brochure or memory.
Contractor Section
Your roofer fills in the company name, address, phone number, and license number if applicable, then signs a certification statement that covers several things at once: the product is on the Qualifying Roofing Products Listing as of the installation date, it was installed to manufacturer specifications, it covers the entire roof including hips and ridges, no overlay was used, and the roof is free of defects.4State Farm. Roofing Installation Information and Certification
By signing, the contractor also confirms that the product packaging shows the proper UL or FM classification label, and that a label from the packaging has been given to you. Each individual shingle, tile, or panel has to carry a separate label showing the classification, manufacturer, manufacture date, and brand name. Ask your roofer to hand you that packaging label before they haul the debris off. You’ll want it in your file.
The Cosmetic Damage Exclusion
Read this part of the form before you sign anything. The fine print states that submitting the form is your “acceptance of the exclusion of damages.”1State Farm. Roofing Installation Information and Certification for Reduction in Residential Insurance Premiums In practice, your policy picks up an endorsement excluding cosmetic hail damage to the roof covering. Cosmetic damage is generally damage that changes appearance without letting water through or shortening the roof’s useful life. Dents in metal panels and minor granule loss that doesn’t affect waterproofing are the typical examples.
Functional damage would still be covered: hail hits that crack the material, allow water penetration, or shorten the roof’s useful life. For metal roofs specifically, State Farm requires you to sign a separate endorsement titled “Exclusion of Cosmetic Loss to Metal Roof Covering Caused by Hail.”3State Farm. Roofing Premium Credits Product Listing Ask your agent to walk you through the exact endorsement language before you commit.
Submitting the Form
Once you and the contractor have both signed, deliver the completed form to your State Farm agent. Handing it directly to the agent or scanning and emailing it are the most reliable options, because the agent can confirm receipt and give you a timeline. State Farm’s discounts page routes all questions on the process through the agent.2State Farm. Homeowner Insurance Discounts
After submission, the insurer verifies the product against the Qualifying Roofing Products Listing and reviews the contractor’s certification. If everything checks out, the discount is applied to the wind and hail portion of your homeowners premium. The amount varies by state and product classification, because each insurance company sets its own credit schedule.3State Farm. Roofing Premium Credits Product Listing Your updated declarations page will show the new premium once the change takes effect.
Records to Keep After the Discount Is Applied
The form doesn’t require attachments, but keep a few things on file for the life of the roof. Save the signed certification form, the packaging label your contractor gave you, and the installation invoice showing the product and contractor. A written warranty or a receipt referencing the specific product model belongs in the same folder.
If a hail event hits and you file a claim, those records shorten the adjustment process. The contractor’s certification that the roof was damage-free at installation also matters: it establishes a clean baseline, so any later damage is clearly tied to a covered event rather than to something that was already there. If you eventually replace the roof with a product that doesn’t meet the impact-resistance standard, you lose the discount, and the cosmetic damage exclusion should come off your policy at the same time. If you sell the home, the new owner applies for the discount separately under their own policy.
